Peripheral facial nerve palsy following ventriculoperitoneal shunting in an infant.

Peripheral facial palsy due to injury to the extracranial segment of facial nerve following ventriculoperitoneal (VP) shunting has been reported in a few adults or adolescents. Herein, we present a case of iatrogenic facial nerve palsy secondary to VP shunt insertion in a 20-month infant. To the best of our knowledge, this is the first infantile case to be ever reported in the literature. Regarding the postnatal development of the mastoid and styloid process, the underdeveloped bony structures may render extracranial part of the facial nerve more vulnerable to blunt trauma. The lesson can be l…

Papillary thyroid carcinoma with metastasis to the temporooccipital skull: a case report.

The patient was admitted to our clinic with the complaint of swelling in his left post-auricular region. The medical history revealed that he underwent thyroid surgery eight years ago and the specimen was reported as papillary thyroid carcinoma. Following required analyses, total thyroidectomy and biopsy from mastoid region were performed. Total thyroidectomy specimen was proved thyroid papillary microcarcinomas at five foci of the thyroid gland, while biopsy samples obtained from the mastoid region were reported as metastatic papillary thyroid carcinoma. The patient underwent radioactive iodi…

Otogenic facial paralysis. A histopathological study.

The histopathological changes of the facial canal and nerve in facial paralysis due to chronic suppurative otitis madia are described in six temporal bones. Bony destruction of the facial canal was found in its tympanic segment and in its upper mastoid part in all cases, but it was particularly notable in the labyrinthine segment as well. The pathological process, cholesteatoma or granulation tissue surrounding the exposed facial nerve was inflamed in all cases. The affected facial nerve showed degenerative and inflammatory changes throughout its tympanic segment in all cases, but its mastoid …

Pott's puffy tumor: a rare complication of acute otitis media in child: a case report.

To describe a rare case of Potts' puffy tumor (PPT) in the zygomatic area, which developed as a complication of acute otitis media in a 6-year-old child. To date, only one case of PPT has been described in the literature as a complication of latent mastoiditis in an adult, and one case of PPT as a complication of acute mastoiditis in a 10-year-old child. Urgent surgical intervention, including evacuation of the purulent lesion, removal of inflamed soft tissue and osteolysis of the involved bone, and antromastoidectomy, intravenous treatment with broad-spectrum antibiotics, including G+, G-, an…

Complicated necrotizing otitis externa.

Necrotizing (malignant) otitis externa (NOE) is a rare and invasive infection originating in the external acoustic meatus seen most commonly in diabetes and other immunocompromised states. After a protracted course, disease can smolder and extend into the mastoid, skull base, dural sinuses, and intracranially. We present a case of NOE complicated by mastoiditis, dural sinus thrombosis, and Bezold's abscess in an uncontrolled diabetic presenting with a prolonged course of facial nerve palsy. We stress the importance of maintaining a high index of clinical suspicion for NOE in diabetic patients …

Septic internal jugular vein thrombosis.

Septic Internal Jugular Vein Thrombosis (SIJVT) is a rare but potentially life-threatening condition which necessitates early clinical recognition and rapid administration of appropriate therapy. Although termed by some authors a "forgotten disease" in the antibiotic era, this entity is still present but frequently overlooked. Deep neck infections, Lemierre syndrome, central venous catheterization and mastoiditis are the etiologies in this series. The diagnosis of SIJVT utilized CT and Doppler ultrasound imaging. The treatment included aggressive abscesses, necrotizing fasciitis, mastoiditis).…

[EBV infection as etiology of otomastoiditis with peripheral facial paralysis in the child].

Case report on a 15-month-old girl with acute bilateral otomastoiditis with facial nerve palsy in the course of an EBV infection. Although the initial mononucleosis blood test was negative the diagnosis was established by the presence of blastic transformed lymphocytes with immunoreaction for the latent membrane protein of EBV, and the presence of EBV-mRNA in the majority of lymphocytes in the biopsy specimen. The occurrence of facial nerve palsy in the course of an EBV infection is relatively rare but well known. The present case involved a viral otomastoiditis with a typical course, which wa…

Five-year postoperative outcomes of modified staged canal wall up tympanoplasty for primary acquired cholesteatoma.

OBJECTIVE: For successful canal wall up tympanoplasty (CWUT) for the treatment of cholesteatoma, the restoration of stable middle ear aeration is also important; however, little is known about the dynamics of such aeration or the optimal surgical procedure. In this study, alternative additional surgical procedure was selected based on the grade of middle ear aeration during the second-stage operation. SUBJECTS AND METHODS: Patients included in this study underwent staged CWUT surgeries with mastoid cortex plasty (MCP) for well-aerated ears (grade 3) and bony mastoid obliteration (BMO) for poo…
