Toward European harmonization of national myasthenia gravis registries: modified Delphi procedure-based expert consensus on collectable data.

BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune disorder. Several new treatment concepts have emerged in recent years, but access to these treatments varies due to differing national reimbursement regulations, leading to disparities across Europe. This highlights the need for high-quality data collection by stakeholders to establish MG registries. A European MG registry could help bridge the treatment access gap across different countries, offering critical data to support regulatory decisions, foster international collaborations, and enhance clinical and epidemiological research. Seve…

Comparison of the Quantitative Myasthenia Gravis and Myasthenia Gravis Activity of Daily Living Scores From a Clinical Practice Perspective.

INTRODUCTION/AIMS: Myasthenia gravis (MG) is characterized by fluctuating symptoms, making the assessment of disease activity challenging. Discordance between the clinician-reported quantitative myasthenia gravis (QMG) score and the patient-reported MG activity of daily living (MG-ADL) has previously been observed. We aimed to compare QMG and MG-ADL in a clinical practice setting and to relate to the quality-of-life instrument myasthenia gravis quality of life 15 (MG-QoL-15). METHODS: MG patients simultaneously assessed by QMG, MG-ADL, and MG-QoL-15 (n = 177) identified in the Swedish MG regi…

Efgartigimod as Fast-Acting Rescue Therapy in Very Late-Onset Myasthenia Gravis.

Myasthenic crisis (MC) is a potentially fatal complication of myasthenia gravis (MG), often requiring intensive care and rescue therapies such as intravenous immunoglobulin (IVIG) and corticosteroids. Efgartigimod, a neonatal Fc receptor (FcRn) antagonist, offers a novel approach by selectively reducing circulating pathogenic IgG. We describe a 77-year-old man with anti-AChR-positive generalized MG who developed a severe MC, with little response to IVIG and high-dose corticosteroids. Intravenous efgartigimod was administered as rescue therapy with rapid and sustained clinical improvement. This…

Characterizing Treatment Patterns and Healthcare Use in Patients and Subgroups with Chronic Inflammatory Demyelinating Polyradiculoneuropathy: A Real-World Study.

INTRODUCTION: Chronic inflammatory demyelinating polyneuropathy (CIDP) is a rare autoimmune disorder of the peripheral nervous system. While some patients with CIDP respond to standard-of-care treatments, a significant proportion remains refractory or has partial response. Evidence on the disease burden of this patient group is limited. This retrospective study aimed to analyze the demographics, clinical characteristics, and healthcare resource use of patients with CIDP in the United States, focusing on those who switched therapies within 2 years of initiating first-line treatment (LOT1). Clinical and economic outcomes of myasthenia gravis and myasthenic crisis when treated with PLEX and IVIG.

INTRODUCTION: Myasthenia gravis is an autoimmune disorder characterized by muscle weakness, with myasthenic crisis representing a life-threatening exacerbation requiring respiratory support. Acute treatment options include plasmapheresis (PLEX) and intravenous immunoglobulin (IVIG). Prior studies have reported mixed results regarding the relative effectiveness of these therapies, although recent evidence suggests that PLEX may provide faster and more noticeable clinical improvement. The objective of this study was to evaluate differences in clinical and economic outcomes among U.S. patients ho…

Proteomic and Machine Learning Analysis Predicts Treatment Response Signatures in Myasthenia Gravis.

BACKGROUND: Myasthenia gravis (MG) is a prototypical antibody-mediated autoimmune disease with variable treatment responses with a need for biomarkers to guide therapeutic decision making. Proteomic profiling, coupled with machine learning, offers a powerful approach to identify biomarkers that may predict treatment response. METHODS: We analyzed sera collected at entry (baseline) from participants in a phase 3 trial randomized trial comparing thymectomy plus prednisone versus prednisone alone, along with matched controls using liquid chromatography-mass spectrometry. We derived disease-speci…

Case Report: Fatal myocarditis and myasthenia gravis induced by immune checkpoint inhibitors: concurrent dual adverse events in an older adult.

Immune checkpoint inhibitors (ICIs) have revolutionized the treatment of metastatic renal cell carcinoma (RCC), offering significant survival benefits. Their use, however, can come at the expense of severe immune-related adverse events (irAEs) involving the myocardium and nervous system. We report an 86-year-old male with relapsed clear cell RCC (Fuhrman grade 3, pT3aN0M0) who developed concurrent myocarditis and myasthenia gravis (MG) after receiving Pembrolizumab and Axitinib for multifocal mediastinal relapse. Despite early initiation of high-dose IV methylprednisolone within 24 hours, the …

Identification and validation of lncRNA mutation hotspot SNPs associated with myasthenia gravis susceptibility.

BackgroundMyasthenia gravis (MG) is an autoimmune disorder caused by antibodies that target the postsynaptic muscle membrane. Recent evidence suggests that genetic variants and long noncoding RNAs (lncRNAs) play crucial roles in the pathogenesis of MG. The purpose of this study was to investigate the associations between lncRNA-related single-nucleotide polymorphisms (SNPs) and MG susceptibility in Chinese populations.MethodsFirst, we identified lncRNA mutation hotspot regions based on the improved Kolmogorov‒Smirnov test and the cumulative hypergeometric distribution principle. Next, we furth…

Phenotypic and functional dysregulations of CD8 + T Cells in myasthenia gravis.

Myasthenia Gravis (MG) is a heterogeneous autoimmune disorder characterized by fluctuating muscle weakness caused by autoantibodies targeting neuromuscular junction components. While the role of CD4 + T cells in MG is well established, the contribution of CD8 + T cells remains poorly understood. In this study, we analyze CD8 + T cells in 36 MG patients and 38 age- and gender-matched controls using flow cytometry to evaluate subset distribution, granzyme expression, and cytokine production. MG patients exhibit an altered CD4 + /CD8 + T cell ratio and significant changes in CD8 + T cell subsets,…

Immune Modulatory Effects of PD-1/PD-L1 in Myasthenia Gravis: Insights From Peripheral Blood Mononuclear Cell Analysis.

Myasthenia gravis (MG) is an autoimmune disease characterized by the production of antibodies targeting acetylcholine receptor (AChR)-associated proteins. The disorder involves various immune cells, including T cells, B cells, and dendritic cells (DCs). Programmed cell death protein 1 (PD-1) is one of the immune checkpoints and plays a crucial role in regulating immune cell activation and cell death. PD-L1, the ligand of PD-1, is critical for immune suppression through the transmission of inhibitory signals. These molecules are implicated in the pathogenesis of MG, prompting further investigat…
