Continuous monitoring of the recurrent laryngeal nerve.

OBJECTIVE: The objective of this study was to assess the safety and utility of continuous intraoperative neuromonitoring for the prevention of recurrent laryngeal nerve injury during thyroidectomy. METHODS: A prospective cohort study was conducted in consecutive patients undergoing thyroidectomy. Variations in amplitude and distal latency of the electromyogram (EMG) were assessed. In the case of suspicious events, restorative manoeuvers were performed to attempt to recover the potentials. Associations were explored between findings in laryngoscopy 24 h after surgery and intraoperative neuroph

Surgical Aspects of Opercular Epilepsies.

Operculoinsular epilepsy presents diagnostic and surgical challenges due to the complex anatomy of the opercula, the underlying insula, and … tical vasculature and eloquent cortex. Intraoperative electrocorticography, functional mapping, and neurophysiological monitoring further refine surgical margins and help mitigate postoperative deficits. We also highlight stereotactic laser interstitial thermal therapy as a minimally invasive alternative for well-defined epileptogenic zones, particularly in the dominant hemisp …

A Novel PNDA-MMNet Model for Evaluating Dynamic Changes in the Brain State of Patients with PTSD During Neurofeedback Training.

BACKGROUND: Monitoring and evaluating dynamic changes in brain states during electroencephalography (EEG) neurofeedback training (NFT) for post-traumatic stress disorder (PTSD) patients remains challenging when using traditional methods. METHOD: This study proposes a novel Process Noise Dynamic Adaptation-Mesoscale Mesonetwork Network (PNDA-MMNet) model, which improves upon conventional techniques by establishing a discrete linear dynamic model of the NFT process. The model utilizes a mesoscale intermediate network architecture to create a brain state observation matrix, computes the brain st

Awake craniotomy with dexmedetomidine during resection of brain tumours located in eloquent regions.

INTRODUCTION: An awake craniotomy (AC) is the gold standard for the resection of supra-tentorial brain tumours in eloquent areas. Intraoperative monitoring "on-demand" of essential eloquent brain functions and the increasing need to preserve higher intellectual functions during surgery requires a unique anaesthetic approach during AC. Dexmedetomidine is considered the first-choice pharmacological agent for sedation during AC. MATERIAL AND METHODS: Twenty-six patients with a single brain tumour located in areas of eloquent brain function were enrolled in this prospective study. The patients un

Associations between anaesthetic dose-adjusted intraoperative EEG alpha power, processing speed, and postoperative delirium: analysis of data from three prospective studies.

BACKGROUND: We previously have shown that low intraoperative EEG alpha power is associated with impaired preoperative cognition, a delirium risk factor, and that intraoperative anaesthetic-dose-adjusted EEG bispectral index values were associated with a four-fold increased risk of postoperative delirium (POD). Yet, associations between intraoperative anaesthetic-dose-adjusted alpha power and delirium or delirium risk factors have yet to be quantified. METHODS: We examined cerebrospinal fluid (CSF) Alzheimer's disease (AD)-related biomarkers, cognitive scores, EEG recordings, and delirium data

Intraoperative Electrocorticographic Monitoring for Predicting Post-Traumatic Seizures and Prognosticating Patients With Head Injury: A Prospective Observational Study.

BACKGROUND AND OBJECTIVES: Post-traumatic epilepsy is a major complication of traumatic brain injury (TBI), with limited predictive tools and preventive strategies. Although intraoperative electrocorticography (ECoG) is widely used in epilepsy and tumor surgeries, its role in brain injury remains underexplored. This study evaluates the predictive value of intraoperative ECoG for seizure development and functional outcomes in patients with TBI. METHODS: This prospective observational study included 110 patients with head injury who underwent surgery and intraoperative ECoG evaluation. The ECoG

Sensorimotor Physiological Mapping During Asleep Deep Brain Stimulation Lead Placement With Multichannel Intraoperative Neuromonitoring.

BACKGROUND AND OBJECTIVES: Lead placement during asleep deep brain stimulation (DBS) surgery has relied primarily on intraoperative imaging, forgoing traditional awake neurophysiological testing. We aimed to describe our experience of asleep macrostimulation mapping of implanted DBS leads using intraoperative neuromonitoring (IONM) techniques, which were used to guide electrode placement-in addition to intraoperative computed tomography imaging and in place of awake neurophysiological testing. METHODS: This was a single institution retrospective study of asleep DBS surgery with IONM mapping f

Compound motor action potential guided 240 seconds plus bonus freeze for safe and durable left atrial appendage isolation in patients with recurrent persistent atrial fibrillation: How to isolate the appendage with cryoballoon (the CMAP guided ICE-B protocol).

INTRODUCTION: Pulmonary vein isolation (PVI) is the cornerstone ablation strategy for treatment of atrial fibrillation (AF). Ablation outside the PVs is sometimes needed particularly in persistent or longstanding AF. This study reported left atrial appendage isolation (LAAI) using cryoballoon (CB) focusing on technical data and short-term clinical follow-up. METHODS AND RESULTS: Patients with recurrent persistent AF after multiple AF procedures were enrolled. LAAI was performed using the second generation CB guided by compound motor action potential (CMAP) and 240 seconds plus bonus freeze (I

Somatosensory Evoked Potentials as a Stand-Alone Tool During Spine Surgery: An Egyptian Preliminary Report.

PURPOSE: Monitoring of somatosensory evoked potentials (SSEPs) serves as an early warning system to detect spinal cord injury and is correlated with postoperative sensory findings. It is an indirect indicator of motor function. This study aimed to evaluate the usefulness of intraoperative SSEPs monitoring as a stand-alone tool during spinal surgeries when motor evoked potentials are not available, to prevent and predict new postoperative neurologic deficits. Motor evoked potentials were not used as the equipment needed to record them was not available at the time of this study. METHODS: This

A-train clusters and the intermedius nerve in vestibular schwannoma patients.

OBJECTIVE: EMG "A-train" activity correlates with postoperative facial palsy after vestibular schwannoma (VS) surgery. An intermedius nerve separate from the facial nerve increases A-trains without significant impact on function. We investigate occurrence of A-train "clusters", A-trains over a majority of channels within a short time frame. METHODS: Data from 217 patients with first surgery for VS were evaluated retrospectively. Continuous EMG recorded with 9 channels was evaluated for A-train patterns. "Clusters" of A-trains were identified, i.e. A-trains within 3 seconds over a majority of

Neurophysiological correlates of motor planning and movement initiation in ACL-reconstructed individuals: a case-control study.

INTRODUCTION: Current evidence suggests that the loss of mechanoreceptors after anterior cruciate ligament (ACL) tears might be compensated by increased cortical motor planning. This occupation of cerebral resources may limit the potential to quickly adapt movements to unforeseen external stimuli in the athletic environment. To date, studies investigating such neural alterations during movement focused on simple, anticipated tasks with low ecological validity. This trial, therefore, aims to investigate the cortical and biomechanical processes associated with more sport-related and injury-relat
