Community-acquired pneumonia in the elderly. Clinical and nutritional aspects.

Community-acquired pneumonia (CAP) in the elderly has a different clinical presentation than CAP in other age groups. Confusion, alteration of functional physical capacity, and decompensation of underlying illnesses may appear as unique manifestations. Malnutrition is also an associated feature of CAP in this population. We undertook a study to assess the clinical and nutritional aspects of CAP requiring hospitalization in elderly patients (over 65 yr of age). One hundred and one patients with pneumonia, consecutively admitted to a 1,000-bed teaching hospital over an 8-mo period, were studied

Brief Report: Decreased Bone Health in Children with Autism Spectrum Disorder and Avoidant Restrictive Food Intake Disorder.

PURPOSE: Children with autism spectrum disorder (ASD) and food selectivity are at increased risk for nutritional deficiencies which could affect bone health. METHODS: We report on four male patients with ASD and avoidant restrictive food intake disorder (ARFID) with significant bone conditions including rickets, vertebral compression fractures, osteopenia, and slipped capital femoral epiphyses. RESULTS: Each patient was at risk for at least one nutritional deficiency. Two out of four patients had deficiencies in Vitamins A, B12, E, and zinc. Calcium and Vitamin D deficiency were noted in all

Nutrition in children with exocrine pancreatic insufficiency.

Exocrine pancreatic insufficiency (EPI) is a condition defined as pancreatic loss of exocrine function, including decreased digestive enzymes and bicarbonate secretion, which leads to maldigestion and malabsorption of nutrients. It is a common complication in many pancreatic disorders. If left undiagnosed, EPI can cause poor digestion of food, chronic diarrhea, severe malnutrition and related complications. Nutritional status and fat-soluble vitamins should be carefully assessed and monitored in patients with EPI. Early diagnosis of EPI is clinically important for appropriate nutritional suppo

Effectiveness of gastrostomy for improving nutritional status and quality of life in patients with epidermolysis bullosa: a systematic review.

Individuals with epidermolysis bullosa (EB) may present with a broad spectrum of growth impairment and multiorgan disorders, with compromised nutritional status and quality of life. The provision of nutrients through a gastrostomy tube may minimize EB-related malnourishment but may also result in skin injuries and infections. In this systematic review we consider the current evidence about the effectiveness of gastrostomy in restoring nutritional status and improving quality of life in patients with EB. Seven studies (n = 146) met selection criteria and patients ranged in age from 6 weeks to 3

Catastrophic weight loss after vertical banded gastroplasty: malnutrition and neurologic alterations.

The case of a patient with severe vomiting and very rapid weight loss following vertical banded gastroplasty (VBG) (almost 50% of original weight in 11 months) and who suffered severe neurological and nutritional disorders is reported. All abnormal findings with the exception of nystagmus, disappeared during enteral nutritional support with a complete diet and vitamin supplementation. The alterations found in this case may have been related to the very rapid weight loss, which resulted in protein calorie malnutrition and, probably, in one or more vitamin deficiencies. This complication may be

Feeding ability in Rett syndrome.

Feeding abilities in 20 individuals with Rett syndrome aged 1 1/2 to 33 years were investigated by history and clinical assessment during a meal, followed by videofluoroscopy of feeding. All were shown to have reduced movements of the mid and posterior tongue, with premature spillover of food and liquid from the mouth into the pharynx. They also showed delayed pharyngeal swallow, but otherwise pharyngeal problems were minimal. These findings were noted to be similar to those in Parkinson's disease. Those individuals with the most general neurological impairment tended to have the worst feeding

Malnutrition and the role of nutritional support in alcoholic liver disease.

Malnutrition adversely effects hepatic function and structure. In certain instances, histological changes indistinguishable from alcoholic liver disease may be seen. Alcohol is clearly a direct hepatic toxin and its results are not prevented by normal nutritional intake. The majority of patients admitted to the hospital with alcoholic liver disease are moderately to severely malnourished but find it difficult or impossible to resume an intake sufficient to repair and maintain a normal nutritional state. Malnutrition may be considered a complication of alcohol abuse which occurs simultaneously

Histochemical and biochemical studies of butyrylcholinesterase activity in adult and developing cerebellum. Effects of abnormal thyroid state and undernutrition.

The cellular and subcellular localization of BuchE activity (EC.3.1.1.8) was studied in the developing and adult rat cerebellum at light and electron microscope levels. In the adult cerebellum, BuchE activity was exclusively localized to glial cells, myelin and endothelial cells. In the immature cerebellum, BuchE activity was additionally found transiently localized to the neuroblasts of the external germinative layer and in Purkinje cells of the nodulus. In both the immature and the adult animals, the main part of the activity seemed to be membrane-bound. The developmental pattern of cerebell
