What Factors Are Associated With Open Treatment of Pediatric Mandibular Fractures?

PURPOSE: There is a paucity of data with respect to management of pediatric facial fractures. The purpose of this study was to describe the population of pediatric patients with mandibular fractures at our institution and to assess predictors of fractures requiring open reduction and internal fixation (ORIF). PATIENTS AND METHODS: This was a retrospective cohort study of patients aged ≤17 years presenting with mandibular fractures. The primary predictor variable was age ≥13 years and <13 years. The primary outcome variable was ORIF (yes or no). Epidemiologic factors and complications were als

Open Reduction of a Posteriorly Locked Shoulder Dislocation: A Case Example.

Posterior shoulder dislocations are uncommon but pose diagnostic and treatment challenges, particularly when locked due to humeral head impaction or soft tissue interference. These cases often necessitate surgical intervention, especially when locked or associated with recurrent instability or bony injuries. Preoperative assessment is vital to confirm the diagnosis and guide surgical planning. The surgical procedure begins with patient positioning and anesthesia, followed by a chosen approach, such as the deltopectoral route. In this case, exposure involved identifying and tenodesing the bicep

Meniscal Entrapment After Tibial Plateau Fracture and Fixation: A Case Report.

CASE: A 40-year-old man presented with limited in range of motion, pain, and tenderness over the medial joint line after an open reduction and internal fixation (ORIF) because of a bicondylar tibial plateau fracture (TPF). The cause of his pain was inconclusive on Magnetic Resonance Image (MRI), so arthroscopy was performed and identified an incarcerated medial meniscus at the fracture site. An osteotomy with medial joint elevation was performed followed by a meniscus release, with excellent results at the 1-year follow-up. CONCLUSION: To our knowledge, this is the first case reporting a trap

Classification and Management of Periprosthetic Patella Fractures.

Complications related to the extensor mechanism and patellofemoral joint continue to be the most common cause of pain and indication for surgical revision following total knee arthroplasty. Numerous risk factors related to the patient, implant, and technical performance of the procedure have been identified. The Ortiguera and Berry classification system is widely used for the systematic classification and management of these fractures. Because of the difficult nature of revision surgery for fracture and the high risk of complication, a careful assessment of the fracture and implants is vital t

Biplanar Osteosynthesis for Severely Displaced Proximal-Third Clavicle Fracture: A Case Report.

CASE: We present the case of a 41-year-old man with a displaced extra-articular proximal-third clavicle fracture managed with open reduction and double-plate internal fixation. Twelve months after the procedure, the patient had recovered full range of motion and returned to work. CONCLUSIONS: Proximal-third clavicle fractures are infrequent injuries and are usually treated conservatively. They are associated with thoracic injuries or high-energy dorsal or cervical spinal trauma. In the case of displaced fractures and small medial fragments, orthopaedic management may have unsatisfactory resul

Arthrodesis or Open Reduction Internal Fixation for Lisfranc Injuries: A Meta-analysis.

The purpose of this study is to determine if arthrodesis, compared with open reduction and internal fixation (ORIF), produces favorable American Orthopaedic Foot and Ankle Society (AOFAS) and visual analogue scale (VAS) scores, and to determine if differences in complication, revision surgery, and secondary arthrodesis rates exist for patients with Lisfranc fracture/dislocation injuries. Searches were performed in PubMed using the keywords "Lisfranc fracture," "metatarsal fracture," "ORIF," "open reduction internal fixation," "arthrodesis," and "fusion." These criteria left 183 articles for re

"Age Is Just a Number": Distal Radius Fractures in Patients Over 75.

Background: Management of distal radius fractures in patients over 65 is a topic of significant study, but there are variations within this group in terms of independence and activity level. This study compares the outcomes of operative distal radius fracture treatment in patients over 75 with those aged 65 to 74, to evaluate the effects of patient demand and advanced age on outcome. Methods: A retrospective review of a single-institution distal radius fracture database was performed. All patients over age 65 were evaluated for inclusion. Patient factors including activity, independence level,

A trans-eyebrow zig-zag approach for frontal bone fractures✰.

Frontal bone fractures are amongst the most common facial fractures and surgical management, which has traditionally involved access via a coronal flap, can result in unsightly scarring, alopecia, paraesthesia, facial nerve weakness and temporal hollowing. Alternative approaches include use of endoscopically-assisted surgery, often through the eyebrow, which minimises the risk of unsightly scarring but may also sacrifice access in the process. In this technical note, we discuss a surgical technique for open reduction and internal fixation of fractures of the bone overlying the frontal sinus an

Treatment of Femoral Shaft Fractures in Patients with Klippel-Trénaunay Syndrome: A Report of 2 Cases.

CASES: Long-bone fractures in patients with Klippel-Trénaunay syndrome (KTS), a rare disorder of the venous, lymphatic, and capillary system, are difficult to treat with many complications. Two patients diagnosed with KTS presented with closed femoral shaft fractures after low-energy falls. Conservative treatment, open reduction internal fixation, and intramedullary nailing resulted in painful nonunions. Ultimately, both patients achieved pain relief and the ability to ambulate after en bloc resection and reconstruction. CONCLUSIONS: These cases demonstrate the challenges in achieving bony un

Fracture of the Entire Posterior Talar Process in an Adolescent Boy: A Case Report.

CASE REPORT: Fractures of the talus in adolescents are uncommon and usually involve the talar body or neck. We report a rare fracture of the entire posterior talar process in a 14-year-old boy who fell during skateboarding. He was managed with open reduction and fixation with a headless screw. He gradually regained sports activities after 4 months and had no further complaints at his last follow-up evaluation, 18 months postoperatively. CONCLUSION: In posterior talar process fractures, operative treatment can provide a good functional outcome. Patients must be followed with serial radiographs

Adding deltoid ligament repair in ankle fracture treatment: Is it necessary? A systematic review.

BACKGROUND: Deltoid ligament injuries are typically caused by supination-external rotation or pronation injury. Numerous ligament reconstruction techniques have been proposed; however, clear indications for operative repair have not yet been well established in the literature. METHODS: We reviewed primary research articles comparing ORIF treatment for ankle fracture with versus without deltoid ligament repair. RESULTS: Five studies were identified with a total of 281 patients. 137 patients underwent ORIF with deltoid repair, while 144 patients underwent ORIF without deltoid ligament repair.

Unburied versus buried wires for fixation of pediatric lateral condyle distal humeral fractures: A meta-analysis.

Open reduction and internal fixation with Kirschner (K) wires has been reported as an efficient and convenient technique for pediatric lateral condyle distal humeral fractures. However, no single study has been large enough to definitively determine whether the K-wires should be buried or unburied. Therefore, we performed a meta-analysis pooling the results from several clinical trials to compare the outcome of using buried versus unburied K-wires. Potential academic articles were identified from the Cochrane Library, Medline (1966-2017.3), PubMed (1966-2017.3), Embase (1980-2017.3), ScienceDi

[Surgical exposures for distal radius fractures].

This is a review of the various approaches that can be used for open reduction and internal fixation (ORIF) of distal radius fractures. The main dissection steps are exposed and the specific indications for each approach are described. The anterior approach is discussed extensively as it is now the gold standard for ORIF of distal radius fractures. The lateral and posterior approaches are also described as they are sometimes needed for complex fractures.

Acetabular fractures in skeletally immature rugby players.

Since the introduction of professionalism to the sport of rugby union, an increasing rate of injury has been noted in the playing population. In addition, overall participation numbers have increased globally, with more amateurs and young people involved in the sport. Acetabular fractures are generally seen after violent trauma and can be associated with poor long-term outcomes. We have previously described two acetabular fractures in young patients (16 and 24 years of age) sustained during rugby matches. We present a case series of four closed, isolated acetabular fractures in three skeletall

Acetabular Fractures in the Elderly: A Critical Analysis Review.

In the physiologically compromised elderly patient with an acetabular fracture, nonoperative treatment is associated with functional outcomes (as indicated by the Western Ontario and McMaster Universities Osteoarthritis Index [WOMAC] scores) similar to those seen in healthier patients who undergo open reduction and internal fixation, with similar mortality and lesser need for late conversion to total hip arthroplasty. Open reduction and internal fixation of displaced acetabular fractures in patients older than 60 years of age is an excellent option provided that the patient does not have risk
