Multi-Body Biomarker Entrapment System: An All-Encompassing Tool for Ultrasensitive Disease Diagnosis and Epidemic Screening.

Ultrasensitive identification of biomarkers in biofluids is essential for the precise diagnosis of diseases. For the gold standard approaches, polymerase chain reaction and enzyme-linked immunosorbent assay, cumbersome operational steps hinder their point-of-care applications. Here, a bionic biomarker entrapment system (BioES) is implemented, which employs a multi-body Y-shaped tetrahedral DNA probe immobilized on carbon nanotube transistors. Clinical identification of endometriosis is successfully realized by detecting an estrogen receptor, ERβ, from the lesion tissue of endometriosis patient

Kidney Injury in Liver Disease.

Acute kidney injury (AKI) occurs frequently in patients with liver disease and increases morbidity and mortality. Hepatorenal syndrome is a common cause of AKI in patients with decompensated cirrhosis and is due to alterations in systemic and renal hemodynamics. Serum creatinine-based estimation of kidney function is a key component of the Model for End-stage Liver Disease score in liver transplant candidates. Continuous renal replacement therapy is used in critically ill patients with liver failure and AKI. Simultaneous liver-kidney transplantation (SLK) may be required in patients with liver

Antibody screening in repeatedly transfused patients.

The purpose of pretransfusion compatibility testing is to prevent immune mediated hemolytic transfusion reactions. Our study aimed to evaluate the gel test for detection of clinically significant antibodies in repeatedly transfused patients. We investigated 200 thalassemic patients in whom, blood group, Rh-D, Rh phenotype determination, antibody screening and identification were done using an ID Microtyping System. Red cell alloantibodies were detected in 21 patients (10.5%). Among these patients, Anti-E was detected in 5 (23.8%), anti-D in 4 (19%), anti-K in 4 (19%), anti-e in 3 (14.3%) and e

Transcriptome Analysis in Patients With Muscle-invasive Bladder Cancer.

BACKGROUND/AIM: Bladder cancer (BC) is the most prevalent malignant tumor in the urinary tract, classified mainly into muscle-invasive BC (MIBC) and non-MIBC (NMIBC). Recent studies highlight the important role of changes in transcriptome activity in carcinogenesis, aiding in the identification of additional differentially regulated candidate genes, improving our understanding of the molecular basis of gene regulation in BC. This study aimed to evaluate the transcriptome of MIBC patients compared with normal subjects. MATERIALS AND METHODS: mRNA sequencing was conducted using the Illumina Nov

Paraneoplastic antibodies targeting intracellular antigens.

Although they are relatively rare, the diagnosis of paraneoplastic neurologic syndromes (PNS) can be aided by the identification of neural autoantibodies in patients' serum and cerebrospinal fluid (CSF). They often clinically manifest as characteristic syndromes, including limbic encephalitis, opsoclonus-myoclonus syndrome, paraneoplastic cerebellar degeneration, and paraneoplastic encephalomyelitis. The antibodies are directed either toward intracellular targets, or epitopes on the cell surface. As compared to cell surface antibodies, intracellular paraneoplastic autoantibodies are more class

Management of Complex Infections in Hemophagocytic Lymphohistiocytosis in Adults.

Hemophagocytic lymphohistiocytosis (HLH) is a syndrome of excessive immune system activation and inflammatory response due to a variety of primary and secondary factors that can cause a range of clinical symptoms and, in severe cases, life-threatening conditions. Patients with HLH are at increased risk of infection due to their abnormal immune function as well as chemotherapy and immunosuppressive therapy at the time of treatment. At the same time, the lack of specific clinical features makes complex infections in HLH challenging to diagnose and treat. The management of complex infections in H

Respiratory Infections in Patients with Primary Immunodeficiency.

Recurrent and life-threatening respiratory infections are nearly universal in patients with primary immunodeficiency diseases (PIDD). Early recognition, aggressive treatment, and prophylaxis with antimicrobials and immunoglobulin replacement have been the mainstays of management and will be reviewed here with an emphasis on respiratory infections. Genetic discoveries have allowed direct translation of research to clinical practice, improving our understanding of clinical patterns of pathogen susceptibilities and guiding prophylaxis. The recent identification of inborn errors in type I interfer

Molecular insight into medulloblastoma and central nervous system primitive neuroectodermal tumor biology from hereditary syndromes: a review.

Through the study of uncommon familial syndromes, physicians and scientists have been able to illuminate the underlying mechanisms of some of the more common sporadic diseases; this is illustrated best by studies of familial retinoblastoma. A number of rare familial syndromes have been described in which affected individuals are at increased risk of developing medulloblastoma and/or supratentorial primitive neuroectodermal tumors. The descriptions of many of these syndromes are based on patients observed by clinicians in their clinical practice. Determination of the underlying genetic defects

Beyond Lassa Fever: Systemic and structural barriers to disease detection and response in Sierra Leone.

BACKGROUND: Lassa fever (LF) often presents clinically as undifferentiated febrile illness. Lassa Fever cases in Sierra Leone have been falling since the 2014-2016 Ebola epidemic. Data from other LF endemic countries suggest that this is not a true reflection of local epidemiological decline, but rather a function of either health seeking behaviour or the health/referral system. In Sierra Leone, many other diseases present with a similar early clinical picture, including COVID-19 and Marburg Disease (which has recently emerged in neighbouring Guinea). This empirical study explores the implemen

Chimeric Antigen Receptor T-Cell Therapy and Imaging Applications for Large B-Cell Lymphoma.

Chimeric antigen receptor (CAR) T-cell therapy has revolutionized the treatment of large B-cell lymphoma (LBCL) and other hematologic malignancies. Its mechanism of action relies on recent biotechnological advances that allow clinicians to harness and enhance a patient's immune system to fight cancerous cells. The indications for CAR T-cell therapy continue to expand, with ongoing trials evaluating their use in other hematologic and solid organ malignancies. This review explores the vital role of diagnostic imaging in patient selection and treatment response in CAR T-cell therapy for LBCL and

Prevalence of actinic keratosis in patients attending the dermatology outpatient clinic.

Actinic keratoses (AKs) are precancerous epidermal lesions that develop on sensitive, frequently sun-exposed skin surfaces. There are very little data regarding AK prevalance. The aim of this study was to investigate the prevalence of AK and related conditions among patients in a dermatology outpatient clinic.Patients attending our dermatology outpatient clinic between January 1, 2015 and December 31, 2017 were evaluated retrospectively usingan automated system. A total of 54,786 patients aged ≥30 years attending the dermatology outpatient clinic were included in the study. We identified 1375

[Establishment of a diagnostic system in disaster medicine].

Disaster medicine is a special field of medicine which is required at unexpected times under poor medical circumstances, such as the transport of several patients at once, complex information and lack of medical staff. In order to provide accurate diagnostic information under such poor medical conditions, it is necessary to establish a well-considered and functional system to prevent malpractice in a serial process from the identification of each patient to blood sampling, its analysis and reporting, and in the process of blood transfusion, as a typical example. We have established a diagnosti

Paradoxical carbon dioxide embolism through an iatrogenic atrial septal defect after catheter ablation for atrial fibrillation during laparoscopic hepatectomy: two case reports and a brief review of the literature (with video).

Carbon dioxide (CO2) embolism is an intraoperative complication during laparoscopic hepatectomy (Hx). Paradoxical CO2 embolism with right-to-left systemic shunting can cause cerebral infarction. Iatrogenic atrial septal defect (IASD) can occur after catheter ablation (CA) for atrial fibrillation (AF), and patients with IASD are at risk of paradoxical CO2 embolism. The present case report described two CO2 embolism cases during laparoscopic Hx after CA for Af (one is a paradoxical CO2 embolism). Intraoperative transesophageal echocardiography (TEE) is required for quick identification of CO2 em

Identification of STAT3 signaling as a shared pathogenic signature in systemic lupus erythematosus, chronic obstructive pulmonary disease, and asthma.

While patients with systemic lupus erythematosus (SLE) have a higher risk of developing chronic obstructive pulmonary disease (COPD) and asthma, the underlying mechanisms have yet to be elucidated. To this end, by integrating transcriptomic data obtained from blood specimens for each disorder, this study made an effort to investigate shared pathogenic signatures in these three disorders. After identification of shared differentially expressed genes (DEGs), their protein interactions (PPIs) were mapped using a high confidence threshold (0.7). Construction of the networks led to the identificati

Navigating diagnostic options for inborn errors of immunity in children: a case-based illustration.

PURPOSE OF REVIEW: In recent years, there has been a dramatic increase in the number of recognized inborn errors of immunity (IEI), many of which present in childhood. This review discusses diagnostic approaches for some of the more common presentations of IEI in childhood. RECENT FINDINGS: Implementation of newborn screening (NBS) using the T cell receptor excision circle (TREC) assay has led to the timely identification of patients with severe combined immunodeficiency (SCID) as well as both syndromic and nonsyndromic forms of T cell lymphopenia, including DiGeorge syndrome. Improvements in

Pythiosis: Case report leading to new features in clinical and diagnostic management of this fungal-like infection.

A 21-year-old man developed infectious keratitis after swimming in Spain whilst wearing contact lenses. Mycelial growth from a corneal sample suggested keratomycosis, but a drastic worsening of the patient's condition was observed on antifungal drugs. On day 38, panfungal PCR identified Pythium insidiosum, which is an aquatic organism belonging to the oomycete family. Based on the recent literature, this patient was promptly prescribed a systemic and local antibiotic regimen and cure was ultimately achieved. In order to facilitate P. insidiosum identification in future cases, we have generated

Neuropathological and histochemical changes in a multiple mitochondrial DNA deletion disorder.

The identification of cytochrome c oxidase (COX)-deficient/succinate dehydrogenase (SDH)- positive cells using sequential histochemistry has proved important in the identification of cells with high mitochondrial DNA (mtDNA) mutant load. We demonstrate large numbers of COX-deficient/SDH-positive neurons in a mosaic pattern throughout the CNS of a patient with a multiple mtDNA deletion disorder. This patient had prominent central and peripheral nervous system involvement with marked cerebellar ataxia, a parkinsonian extra-pyramidal movement disorder, external ophthalmoplegia, dysphagia, and a s

Risk of Progression to Autoimmune Disease in Severe Drug Eruption: Risk Factors and the Factor-Guided Stratification.

The identification of risk factors is key not only to uncover the pathogenesis of autoimmune disease but also to predict progression to autoimmune disease. Drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms is likely the best prototypic example for analyzing the sequential events. We conducted a retrospective study of 55 patients with drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms followed up for the possibility of later development of autoimmune disease ∼18 years after resolution. Nine patients progressed

[Monoclonal gammopathies of undetermined significance - update for referring physicians].

Monoclonal gammopathies of undetermined significance (MGUS) are frequently diagnosed in the global population. Because of its possible transformation into a hematological malignancy, the identification of a MGUS requires a regular and generally long follow-up. However, this risk of transformation differs between the individuals and different laboratory criteria have been identified as predictive factors for progression and were combined in scoring systems that allow correct classification of individuals. The management of these patients needs to be adapted according to the calculated risk prof

Diagnosis and management of autoimmune diseases in the ICU.

Autoimmune diseases encompass a broad spectrum of disorders characterized by disturbed immunoregulation leading to the development of specific autoantibodies, resulting in inflammation and multiple organ involvement. A distinction should be made between connective tissue diseases (mainly systemic lupus erythematosus, systemic scleroderma, inflammatory muscle diseases, and rheumatoid arthritis) and vasculitides (mainly small-vessel vasculitis such as antineutrophil cytoplasmic antibody-associated vasculitis and immune-complex mediated vasculitis). Admission of patients with autoimmune diseases
