Old Surgical Scar at the Ankle Causing Referred Sciatica-Like Pain Treated With a Scar-Fascial Release Technique.

A surgical scar with adhesions to the underlying fascia and periosteum caused radiating pain to different parts of the scar. Abdominal pain is a common complication of surgical scars, but surgical scar associated with extremity pain is rare. A 75-year-old man had a gait disturbance due to right sciatica-like pain from the thigh to the lower leg for >10 years. He also had mild ankle pain due to osteoarthrosis. The medical history was significant for an ankle injury diagnosed as a sprain and intra-articular small fracture, for which he underwent resection of the bone fragment from the anterior a…

Tubercular granuloma arising from floor of orbit : A rare occurrence.

INTRODUCTION: Tuberculosis of the orbit is a unilateral entity commonly involving the bony wall, periosteum or soft tissue in left orbit. It is a rarity. CASE: A fifty seven years old female presented with a localised mass near inferior orbital margin. Owing to its small size and superficial location in orbit, there was no soft tissue displacement or motility disturbance. OBSERVATIONS: Dissection revealed a localised firm mass attached to periorbita covering the orbital floor. The mass was excised and on histopathological analysis it was found to be composed of multiple granulomas containing…

Severe neonatal subgaleal hemorrhage as the first presentation of hemophilia A.

INTRODUCTION: Subgaleal hemorrhage is a rare but potentially fatal birth trauma. It is caused by rupture of the emissary veins (connections between the dural sinuses and scalp veins), followed by the accumulation of blood between the epicranial aponeurosis and the periosteum. Usually, it is associated with instrumental delivery (vacuum extraction, forceps delivery), but it may also occur spontaneously, suggesting the possibility of congenital bleeding disorder. CASE OUTLINE: A full term male neonate was born at 40 weeks gestation by spontaneous vaginal delivery, with birth weight of 3,700 g. …

Vascularization of the long process of the malleus: surgical implications.

Knowledge of the vascularization of the malleus may help to better understand some unwanted effects occurred in those surgical techniques us … rating branches which penetrate the bone from arteries of the tympanic membrane that, following the periosteum/perichondrium, they arrange parallel to the bone. The detachment of the tympanic membrane to fasten a MVP prosthesis causes a damage in the periosteal/perichondral vascular supply of the handle as well as micro-trauma on it, which can promote bone r …

A multiscale mechanobiological model of bone remodelling predicts site-specific bone loss in the femur during osteoporosis and mechanical disuse.

We propose a multiscale mechanobiological model of bone remodelling to investigate the site-specific evolution of bone volume fraction acros … e turnover rate. Mechanical adaptation is found to help preserve intracortical bone matrix near the periosteum. Moreover, it leads to non-uniform cortical wall thickness due to the asymmetry of macroscopic loads introduced by the bending moment. The effect of mechanical adaptation near the endosteum can be greatly affected by whether the mechanical stimulus …

Management of a Peripheral Giant Cell Granuloma in the esthetic area of upper jaw: A case report.

INTRODUCTION: Peripheral Giant Cell Granuloma (PGCG) is the most common giant cell lesion found in the jaws; it originates from the connective tissue of the periosteum or from the periodontal ligament in response to local chronic irritation. Early and accurate diagnosis of these lesions allows for conservative management without compromising adjacent teeth or tissues. PRESENTATION OF CASE: This paper presents a case of PGCG in the esthetic zone of the upper jaw in a 23-year-old female patient. The treatment offered consisted of an excisional biopsy with immediate soft tissue grafting to preve…

PURPOSE: The purpose of our work was to verify the hypothesis that muscle insertions and ligament attachments have an impact on the course o … er the pertrochanteric fracture osteosynthesis. RESULTS AND CONCLUSION: Bone covered only with the periosteum, with no reinforcing elements of the origin or insertions of muscles or attachments of ligaments, represents the locus minoris resistentiae for beginning of fractures. Do diseases cause entheseal changes at fibrous entheses?

Fibrous entheses have been widely used to study activity-related stress to infer behavioural patterns in past populations. Unlike their fibrocartilaginous counterparts, the effect of disease processes has been less widely discussed in the osteoarchaeological literature. This study uses a literature review to ascertain whether specific pathological changes should be taken into account when recording fibrous entheses. Due to the anatomical structure of fibrous entheses it was hypothesised that diseases which affect the structure of bone or the periosteum might be a factor in causing entheseal ch…

Distinguishing stress fractures from pathologic fractures: a multimodality approach.

Whereas stress fractures occur in normal or metabolically weakened bones, pathologic fractures occur at the site of a bone tumor. Unfortunately, stress fractures may share imaging features with pathologic fractures on plain radiography, and therefore other modalities are commonly utilized to distinguish these entities. Additional cross-sectional imaging with CT or MRI as well as scintigraphy and PET scanning is often performed for further evaluation. For the detailed assessment of a fracture site, CT offers a high-resolution view of the bone cortex and periosteum which aids the diagnosis of a …

Osteosarcoma.

Osteosarcoma is the most common bone tumor of children and adolescents. The peak incidence of the disease is in the 15 to 19 year age group. The disease is more commonly seen in males than females. While several factors, including exposure to radiation, genetic disorders such as retinoblastoma, and high rate of bone growth, have been associated with osteosarcoma, in most cases no definite etiology can be established. Osteosarcoma usually originates in the metaphyseal region of long bones and extends through the cortex, causing varying degrees of bone destruction and expansion of periosteum. Th…

Early tissue reactions after circulatory and skeletal damage to the mandible of the rat.

Skeletal and circulatory damage in the form of a fracture of the mandible, proximal to the entry of the inferior alveolar artery and other perforantes, alone or combined with unilateral or bilateral mucoperiosteal extirpation, was inflicted on growing young rats. A morphological study of the tissue changes in the bone, dental tissues and soft tissues was performed. Cutting of the nutrient vessels together with extirpation of the periosteum was followed after 10 days by extensive infarction of the incisor pulp, the odontoblasts and the apical part of the enamel organ. The bone was damaged centr…

Active subperiosteal versus passive subdural 24-hour drainage following single burr hole evacuation of chronic subdural haematoma (the SUPERDURA trial): protocol for a multicentre, randomised non-inferiority trial.

INTRODUCTION: The main treatment of chronic subdural haematoma (CSDH) is neurosurgical evacuation with subsequent drainage. However, consensus on optimal drain modality and placement is lacking. AIM: To examine whether 24-hour active subperiosteal drainage is non-inferior to 24-hour passive subdural drainage after a single burr hole evacuation of a symptomatic CSDH. METHODS AND ANALYSIS: SUPERDURA is a multicentre randomised non-inferiority trial encompassing all neurosurgical units in Denmark. Adult patients with symptomatic CSDH admitted to a Danish neurosurgical unit for single burr hole

Subperiosteal abscess in pediatric acute osteomyelitis: ultrasound-guided percutaneous aspiration in three cases.

Subperiosteal abscess is a recognized complication of pediatric acute osteomyelitis and has traditionally been managed with surgical drainage. Evidence supporting minimally invasive ultrasound-guided therapeutic alternatives in children remains limited. We report three pediatric cases of acute osteomyelitis complicated by subperiosteal abscesses successfully treated with ultrasound-guided percutaneous aspiration. In all patients, ultrasound demonstrated subperiosteal collections measuring 4-7 mm in thickness, with no evidence of substantial intraosseous abscess on ultrasound or magnetic resona
