Trigeminal neuralgia in nasopharyngeal carcinoma.

Tumours at the skull base may show perineural infiltration of the mandibular nerve. Subsequent retrograde spread into the cavernous sinus, Gasserian ganglion, the trigeminal nerve and the pons may be seen. The patient, a known and treated case of nasopharyngeal carcinoma (NPC), complained of trigeminal neuralgia and difficulty in chewing. Magnetic resonance imaging (MRI) revealed a Gasserian ganglion and trigeminal infiltration with resultant atrophy of the muscles innervated by the mandibular nerve. Proximal cranial nerve involvement should be suspected in patients with skull base malignancy …

[The clinical and computed tomographic criteria of the diagnosis of multiple systemic cerebral atrophy with progressive autonomic insufficiency (the Shy-Drager syndrome)].

Based on clinical and CT examination of 3 patients with Shy-Drager syndrome and the reported data, differential diagnosis of the given diseases is under discussion. Shy-Drager syndrome is characterized by CT signs of progressive atrophy of the pons and cerebellum with no essential changes in the supratentorial brain structures. The authors support a proposal to use the term "multiple systemic atrophy" for designation of the disease entity that includes both Shy-Drager syndrome and sporadic patterns of olivopontocerebellar and strionigral degenerations in which the disease debuts by motor disor…

[Imaging of the facial nerve and the petrous apex].

From its origin in the pons to its extracranial exit with a long intrapetrous course, the facial nerve raises various diagnostic problems related to the site and extent of the lesions, which involves the whole field of petrous pathology. The study of the pathology of the neighbouring petrous apex completes this regional analysis. Today, in accordance with the anatomical loco-regional complexity, MRI is the predominant modality of diagnostic imaging (but must be associated with CT to allow the more accurate detection of subtle bone changes); it will be at best directed by the clinical and funct…

Clival chordoma presenting with acute brain stem hemorrhage.

The authors present a case of a 29-year-old man who developed rapidly progressive cranial nerve palsies and a right hemiparesis secondary to a pontine hemorrhage. The rare but correct diagnosis of a clival chordoma which had invaded the brain stem and subsequently hemorrhaged was based on computed tomography and magnetic resonance imaging. The diagnosis was confirmed at surgery when the patient underwent a successful operative decompression of tumor and clot from the pons via a sub-occipital craniotomy. This represents the first case of a clival chordoma to hemorrhage into the brain stem, whic…

Stab wounds to the temporal fossa.

Stab wounds to the temporal fossa appear as a characteristic clinical entity. Patients admitted with stab wounds to the head during the period 1970 to 1986 were reviewed retrospectively. Of these, 10 met the criteria of having suffered a stab wound that penetrated the skull and dura mater of the temporal fossa. Injury to the internal carotid artery-cavernous sinus complex (3 patients) or to the basilar artery-pons region (5 patients) was frequent. Two other patients experienced injury to the trigeminal nerve and the petrous ridge. The mechanical, neurological, radiological, and prognostic feat…

Central pontine myelinolysis: a complication of hyponatremia or of therapeutic intervention?

We report four cases of central pontine myelinolysis (CPM) that illustrate important features of the disorder. The condition is described mainly in the neurological literature and, to our knowledge, is not discussed in the forensic science journals. This disorder must be recognized and understood by the forensic science expert who addresses issues of liability. In cases of multiple motor deficits and death with a history of hyponatremia, CPM must be included in the differential diagnosis. Careful examination of the pons and adjoining structures must be performed. Myelin stains are advisable. T…

Origin of early waves evoked by infraorbital nerve stimulation in man.

The origins of early waves recorded from the scalp of man after stimulation of the infraorbital nerve have been investigated by simultaneous recording from the scalp and from the trigeminal pathway in patients undergoing thermocoagulation rhizotomy. It has been found that the surface-recorded W1, W2 and W3 waves correspond respectively to the activity of the point of entry of the maxillary nerve into the gasserian ganglion, the point of entry of the trigeminal root into the pons and the presynaptic portion of the trigeminal spinal tract. It is remarked that depth recording during surgery may p…

Pseudobulbar palsy after posterior fossa operation in children.

The authors describe their experience with an unusual postoperative syndrome of pseudobulbar palsy occurring a few days after the extirpation of large midline vermian/4th ventricular tumors in children. The patients had a delayed onset of supranuclear cranial nerve palsies associated with emotional incontinence and lability that resolved over several weeks to months. It is postulated that retraction pressure on the medial cerebellum and split vermis is the operative insult responsible for edema that subsequently tracked along fiber pathways in the middle and superior cerebellar peduncles into …

Early auditory evoked potentials: developmental aspects and validity in neuropaediatric and audiologic disorders.

Auditory evoked potentials and in this context especially five waves in the first 10 ms (early auditory evoked potentials = EAEP) are a diagnostic aid in topodiagnosis of posterior fossa diseases. This is due to waves I to V which arise along the acoustic nerve and in brain stem structures such as medulla, pons and mid-brain. Besides an indication about the site of a lesion in the posterior fossa, wave V allows an objective threshold determination. The present results were gained in normal children aged 1-3 years and in children with neuropaediatric and audiologic disorders.

Newer applications of the histological stain prepared from Pterocarpus santalinus.

A histological stain prepared from the heartwood of Pterocarpus santalinus Linn. has been found to be an excellent nuclear stain for various cells of animal and plant origin. As an elastic tissue stain, the results are comparable to standard elastic tissue stains. The striations of voluntary muscle fibers are well shown. The Nissl granules and fibers of cranial nerves in the pons are visualized. When counterstained with light green, it differentially stains muscle and fibrous tissue. The stain can be used as counterstain with certain histochemical procedures with satisfactory results. The prep…

Bilateral occlusion of basilar artery branches.

In a case in which the patient became totally paralysed except for blinking and vertical eye movements, microscopic serial sections of the pons showed bilateral infarcts which were due to occlusion of two small basilar branch arteries, one on each side. One basilar branch was occluded by an atheroma lying at its junction with the basilar artery and the other by an intramural dissection within the wall of the basilar artery. This case provides the clinico-pathological correlation for two further basilar branch infarcts. An unusual finding was that one of the branch arteries supplied the basis p…

HDL protein composition differs between young white European and South Asian men before and after weight gain.

South Asians (SAs) in the UK are at an increased risk of cardiovascular disease (CVD), develop type 2 diabetes mellitus at a lower age and body mass index, and have a lower high-density lipoprotein cholesterol (HDL-C) concentration than their white European (EU) counterparts. The failure of HDL-C raising therapies for CVD risk reduction has turned attention to its composition and function. A previous study comparing the effect of moderate weight gain on SA and EU men found baseline and weight gain-induced ethnic differences in body composition, adipocyte function and insulin resistance (Clinic

Whole-brain connections of glutamatergic neurons in the mouse lateral habenula in both sexes.

BACKGROUND: The lateral habenula (LHb) is an epithalamus nucleus that is evolutionarily conserved and involved in various physiological functions, such as encoding value signals, integrating emotional information, and regulating related behaviors. The cells in the LHb are predominantly glutamatergic and have heterogeneous functions in response to different stimuli. The circuitry connections of the LHb glutamatergic neurons play a crucial role in integrating a wide range of events. However, the circuitry connections of LHb glutamatergic neurons in both sexes have not been thoroughly investigate…

Ginger (Zingiber officinale Roscoe) preparations for prophylaxis of postoperative nausea and vomiting: A Bayesian network meta-analysis.

ETHNOPHARMACOLOGICAL RELEVANCE: Ginger has been proposed for prevention of postoperative nausea and vomiting (PONV), however it remains equivocal whether ginger can be an alternative option and which certain preparation is optimal for PONV prophylaxis. AIM OF THE STUDY: We conducted a network meta-analysis (NMA) to compare and rank relative efficacy for PONV control among all available ginger preparations collected in the databases. METHODS: Eligible records were identified by retrieving Medline (via Pubmed), Embase, Web of Science, CENTRAL, CNKI, WHO ICTRP and ClinicalTrials.gov for randomi
