Effect of managed care enrollment on primary and repeat cesarean rates among U.S. Department of Defense health care beneficiaries in military and civilian hospitals worldwide, 1999-2002.

BACKGROUND: In response to rising cesarean rates, it is reasonable for health care organizations to look to a managed care model as a means of controlling further rate increases. However, little conclusive evidence exists to support this solution. We undertook a study of the Department of Defense health care beneficiary population to assess the impact of enrollment in TRICARE Prime, the Department's managed care health plan, on cesarean delivery rates. METHODS: Pooled hospital discharge records from 1999-2002 for live, singleton births were analyzed to calculate primary and repeat cesarean ra

Mortality Due to Opioid Overdose in the United States: Trends from a CDC WONDER Analysis (1999-2024).

The opioid epidemic in the United States has led to a sharp rise in overdose deaths over the past two decades. Although some progress was observed prior to the coronavirus disease 2019 pandemic, opioid-related mortality has increased again in recent years, underscoring the need to re-examine national trends. A retrospective analysis of opioid overdose mortality among US adults aged 25 years and older from 1999 to 2024 was performed using data from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiological Research (CDC WONDER) Multiple Cause-of-Death database.

Rethinking Blood Pressure Quality Measurement: Accounting for Seasonal Variation in Value-Based Care.

Blood pressure (BP) control is a core quality metric in value-based care (VBC). The Healthcare Effectiveness Data and Information Set (HEDIS) Controlling High Blood Pressure (CBP) measure uses the final BP reading of the calendar year, a method that overlooks well-documented seasonal variation-BP declines in summer and rises in winter. We evaluated the impact of this pattern on measured performance in a large health system. We conducted a retrospective analysis of monthly BP control rates (July 2019 to December 2025) across the Jefferson Health-Lehigh ambulatory network, totaling 78 months and

Integrating Community Linkages into Care Management Workflows to Address Contextual Drivers of Health: Lessons from a Pilot Intervention.

Adverse contextual drivers (eg, food, housing, transportation insecurity) influence health outcomes, particularly among individuals with chronic conditions. Electronic health record (EHR)-based care management tools can support screening and referrals for contextual drivers, but under-resourced health centers often lack support to integrate these tools into routine workflows. This pilot study assessed the feasibility of a multicomponent implementation support model to improve care managers' use of EHR tools to address contextual drivers. Three health centers within a national network using a s

Psychometric Evaluation of the EVCA in Digital Health Settings Using Rasch Analysis Among Patients with Chronic Diseases.

With the expansion of internet-based health care, value co-creation behaviors of patients with chronic diseases have become increasingly important. However, appropriate instruments for assessing such behaviors among community-dwelling patients in digital health contexts remain limited. This study aimed to psychometrically evaluate the effort in value cocreation activities (EVCA) questionnaire using the Rasch model to establish a valid and reliable measure of patient engagement in value cocreation in digital health. A Chinese version of the EVCA questionnaire, developed following standard proce

Identification and Validation of a Machine Learning Predictive Model for Type 2 Diabetes Mellitus Based on Inflammation-Related Indicators.

Type 2 diabetes mellitus (T2D) is a prevalent metabolic disorder with significant health and economic burdens worldwide. The relationship between inflammation-related indicators and the risk of developing new-onset T2D remains underexplored. This study aims to identify and validate an interpretable predictive model for incident T2D using inflammation-related indicators. We analyzed data from 220,937 participants free of diabetes at baseline in the UK Biobank. Six machine learning algorithms were employed to construct predictive models. Feature selection was performed using Least Absolute Shrin
