Degradable and environmentally responsive polymers have been actively developed for drug delivery and regenerative medicine applications, yet inadequate consideration of their compatibility with terminal sterilization presents notable barriers to clinical translation. This Review discusses industry-established terminal sterilization methods and aseptic processing and contrasts them with innovative approaches aimed at preserving the integrity of polymeric implants. Regulatory guidelines, fiscal considerations, and potential pitfalls are discussed to encourage early integration of sterility regu

An aesthetically pleasing face is centered on strength, symmetry, and balance. A defined, well-projected chin and jawline are crucial to this harmony and recontouring the lower third of the face plays a significant role in plastic surgery. Chin augmentation can be performed with injectables, implantation, or osseous genioplasty. The selection of the best procedure is based on the type and extent of deformity. This article reviews chin augmentation with emphasis on alloplast implantation. The relevant anatomy, patient evaluation, implant indications and limitations, alloplast selection, and sur

Chin deformities are commonly encountered defects in patients seeking rhinoplasty. Careful preoperative evaluation may identify patients who could benefit from concurrent augmentation or reduction mentoplasty. Alloplastic chin implants and sliding genioplasty represent the main accepted methods of chin augmentation. Although both procedures may be used for retrognathia or microgenia, the sliding genioplasty may also be used in chin asymmetry, prognathia, and vertical height discrepancies. This article outlines the methods to analyze the chin, and discusses the treatment options available for c

Congenital iris defects may usually present either as subtotal aniridia or colobomatous iris defects. Acquired iris defects are secondary to penetrating iris injury, iatrogenic after surgical excision of iris tumours, collateral trauma after anterior segment surgery, or can be postinflammatory in nature. These iris defects can cause severe visual disability in the form of glare, loss of contrast sensitivity, and loss of best corrected visual acuity. The structural loss of iris can be reconstructed with iris suturing, use of prosthetic iris implants, or by a combination of these, depending on t

Cutaneous reactions to metal implants, orthopedic or otherwise, are well documented in the literature. The first case of a dermatitis reaction over a stainless steel fracture plate was described in 1966. Most skin reactions are eczematous and allergic in nature, although urticarial, bullous, and vasculitic eruptions may occur. Also, more complex immune reactions may develop around the implants, resulting in pain, inflammation, and loosening. Nickel, cobalt, and chromium are the three most common metals that elicit both cutaneous and extracutaneous allergic reactions from chronic internal expos

Internally hexed and threaded root-form implants provide the clinician with many new treatment options for single tooth restorations. Coupled with an interlocking hex on the abutment, they provide stability for single tooth restorations while maintaining the options of screw or cement retention. Five techniques for fabricating single tooth restorations are presented. Clinical and laboratory evaluations of implant angulation, quantity and quality of the gingival tissues, and limitations in vertical height are discussed in relation to design, abutment selection, and laboratory procedures for eac

In a review of 50 primary total condylar knee arthroplasties in 25 female and in 14 male patients the factors affecting the necessity of lateral patellar release and bone grafting of the medial tibial plateau were established. Lateral patellar release was performed in 18 of 33 arthroplasties in female patients, in only two of 17 operations in male patients, and on all but one of the knees with preoperative valgus deformity. Bone grafting of the medial tibial plateau was necessary mainly in small knees, i.e., for six of 18 small prostheses in contrast to one of 15 standard-sized implants in fem

Reconstruction of the microtic ear remains one of the most challenging procedures encountered by the reconstructive surgeon. The use of autogenous rib cartilage continues to be the gold standard for microtia repair. Numerous refinements and modifications in the original technique described by Tanzer have paved the way for exceptional results in experienced hands. However, ideal results are not always achieved, and there continue to be drawbacks with the standard approach to reconstruction with autogenous rib cartilage. In an attempt to circumvent these shortcomings, surgeons have developed alt

Complex implant rehabilitations can include procedures requiring multiple phases of treatment, commonly referred to as staged approaches. The reasons for staged approaches are varied but usually involve serial extraction of hopeless teeth. These treatment plans both enable the patient to avoid removable prostheses by keeping natural teeth during healing phases, and circumvent the immediate loading of some implants placed in grafted bone. One major disadvantage to serial extraction in a staged approach is the potential for gingival changes. These changes include gingival recession around abutme

Amputation has a profound impact on an individual's quality of life (QoL) and functional ability. While socket prostheses are the current first-line treatment, they often cause socket-related issues. Bone-anchored prostheses (BAP) have been introduced to address these problems and improve the amputee experience. This systematic review and meta-analysis aim to compare the QoL between bone-anchored and socket prostheses in transfemoral amputees. A systematic review and meta-analysis were conducted from November 2023 to July 2024, following PRISMA guidelines. Databases including PUBMED, EMBASE, S

BACKGROUND: Rotating-hinge total knee prostheses may be used for the treatment of global instability about the knee. The designs of previous generations were associated with suboptimal outcomes.
QUESTIONS/PURPOSES: We evaluated the clinical and radiographic outcomes of salvage knee reconstructions using modern-generation, modular, kinematic rotating-hinge total knee prostheses.
METHODS: We retrospectively reviewed 26 rotating-hinge arthroplasty devices to examine whether acceptable results were obtainable using a single arthroplasty design. The average age of the patients was 77 years (SD, 9

PURPOSE: To evaluate the survival and success rates of short (> 6 mm and ≤ 8.5 mm) implants after at least 2 years of functional loading. Implants were assessed using clinical parameters such as marginal bone loss (MBL), pocket depth (PD), keratinized mucosa width (KMW), bleeding on probing (BoP), and the peri-implant condition (mucositis or peri-implantitis). Any correlations between clinical parameters were analyzed.
MATERIALS AND METHODS: This observational and retrospective study included 114 posterior maxillary and mandibular implants placed in 27 women and 38 men with a mean age of 68.0

BACKGROUND: Augmentation mammaplasty is the second most commonly performed aesthetic operation. Since the moratorium on gel-filled devices in 1992, saline-filled devices have become the prosthesis of choice in the United States. Although inflatable breast implants have been available since 1965, high rates of complete implant deflation have been reported. In this retrospective study, the authors highlight the effect of filling volume on the incidence of implant deflation, other implant-related trade-offs, and implant survival.
METHODS: Forty-nine women who had undergone aesthetic augmentation

Hypoplastic asymmetry due to hemifacial microsomia (HFM) often represents the most difficult reconstruction in the craniomaxillofacial clinic. Although autogenous grafts are generally used for temporomandibular joint reconstruction (TMJR), the use of TMJR prostheses is not well established. The aim of this review was to identify, collect and analyse the use of extended TMJR (eTMJR) prostheses in patients with HFM, describing clinical features, surgical procedures and postoperative complications. Online searches of all major databases were performed according to PRISMA guidelines. All studies w
