Incision infection with Mycobacterium tuberculosis after total hip arthroplasty without any primary tuberculosis focus.

We describe a case of a tuberculous abscess unassociated with other clinical features of tuberculosis in a 46-year-old woman with a history of 4 hip surgeries plus total hip arthroplasty (THA) due to developmental hip dislocation. Four months after THA, she developed a collection at the incision site for which specimens produced positive culture findings for Mycobacterium tuberculosis. We could not detect any primary focus of tuberculosis anywhere in the patient's body. We performed soft-tissue debridement and drainage completely above the fascia lata to remove pus. The infection recurred twic

[CT scanning and treatment of intracranial tuberculoma].

OBJECTIVE: To evaluate the value of CT scanning and the efficacy of anti-tuberculosis short-course chemotherapy in intracranial tuberculoma patients. METHOD: The CT manifestations and the clinical characteristics of 55 intracranial tuberculoma patients were analysed. RESULTS: 44 out of 55 patients were cured. Among them, 40 patients' lesions were entirely resolved, while other 11 cases, clinical symptoms were subsided and still under treatment. In the contrast enhancing CT scanning, plate shaped or ring-form shadows were shown surrounding the lesions of tuberculoma. The characteristical feat

A service program of antituberculosis chemotherapy with five drugs for four months in the treatment of drug addicts and prisoners with pulmonary tuberculosis in Hong Kong. Hong Kong Chest Service/British Medical Research Council.

The results are reported of a service in Hong Kong of intensive antituberculosis chemotherapy with 5 drugs given daily for 4 months, or until discharge from hospital or release from prison if earlier, in the treatment of male Chinese drug addicts and prisoners who had pulmonary tuberculosis positive for acid-fast bacilli on microscopic examination of the sputum. Of 69 patients who received 4 months of chemotherapy, all those with sputum cultures negative for M. tuberculois initially, and more than 80% of those with positive cultures, 41% of whom had strains resistant to isoniazid, streptomycin

Protective effect of hesperidin, ascorbic acid and their combination on oxidative stress, dyslipidemia, and histological changes in antitubercular drug-induced hepatotoxicity in rats.

BACKGROUND: Hesperidin and ascorbic acid (AA) enhance cellular antioxidant defense systems by neutralizing the free radicals which formed during oxidative stress that could offer protective effects against drug-induced liver injury. Hence, this study aims to investigate the effect of hesperidin, AA and their combination against antitubercular drug (ATDs)- induced hepatotoxicity in Wistar albino rats. MATERIALS AND METHODS: The rats were divided into six groups of 6 animals each. Isoniazid (H), Rifampicin (R), and pyrazinamide (Z) (27, 54, 135 mg/kg.b.wt) were co-administration for 50 days to …

Pharmacokinetics of First-Line Drugs Among Children With Tuberculosis in Rural Tanzania.

BACKGROUND: Dosing recommendations for treating childhood tuberculosis (TB) were revised by the World Health Organization, yet so far, pharmacokinetic studies that have evaluated these changes are relatively limited. We evaluated plasma drug concentrations of rifampicin (RIF), isoniazid (INH), pyrazinamide (PZA), and ethambutol (EMB) among children undergoing TB treatment in Tanzania when these dosing recommendations were being implemented. METHODS: At the end of intensive-phase TB therapy, blood was obtained 2 hours after witnessed medication administration to estimate the peak drug concentr…

Toxicity related to standard TB therapy for pulmonary tuberculosis and treatment outcomes in the REMoxTB study according to HIV status.

BACKGROUND: The phase III REMoxTB study prospectively enrolled HIV-positive (with CD4+ count > 250 cells, not on anti-retroviral therapy) and HIV-negative patients. We investigated the incidence of adverse events and cure rates according to HIV status for patients receiving standard TB therapy in the trial. METHODS: Forty-two HIV-positive cases were matched to 220 HIV-negative controls by age, gender, ethnicity, and trial site using coarsened exact matching. Grade 3 and 4 adverse events (AEs) were summarised by MedDRA System Organ Class. Kaplan-Meier curves for time to first grade 3 or 4 AE w

Tuberculous Meningitis in an Immunocompetent Host: A Case Report.

BACKGROUND Tuberculous meningitis is very rare in the United States in immunocompetent hosts. Risk factors are similar to those of pulmonary tuberculosis, including poverty, malnutrition, overcrowding, a compromised immune system, and coming from an endemic area. Meningeal tuberculosis mortality and other outcomes have changed little over time despite effective therapies due to delay in diagnosis because of its rarity, variable presentation, and often indolent course. CASE REPORT We describe a case of a 57-year-old male immigrant from Senegal with no significant past medical history and no pre

Nasolacrimal tuberculosis: case report highlighting the need for imaging in identifying and managing it effectively.

PURPOSE: To report a rare case of nasolacrimal tuberculosis, conduct a literature review, and to suggest an optimal management plan. METHODS: A 39-year old Zimbabwean female presented with a 3-year history of left epiphora, haemolacria and medial canthal mass. On ophthalmic examination there was no lateral displacement of the left globe. The initial management was external dacryocystorhinostomy. Histology of the biopsy was inconclusive and her symptoms did not improve. This prompted a referral to otolaryngology. Nasal examination revealed a friable mass of the middle turbinate. CT scan showed

Immune thrombocytopenic purpura associated with pulmonary tuberculosis.

Over the past decade, tuberculosis has been recognized worldwide as a public health problem of increasing proportions. We report a patient who presented with epistaxis and generalized petechiae. The diagnosis was immune thrombocytopenia and the patient was treated with intravenous immunoglobulin and pulse steroid. The bleeding continued and thrombocyte level was low despite therapy. Chest X-ray and a computed tomography scan of the thorax showed right upper lobe opacities and bilateral interstitial infiltrates. The patient also had a history of close contact with an individual with active tube

Tuberculous sacro-ileitis: two cases and radiological findings.

Infective sacro-ileitis is due to common bacteria, 25% being tuberculosis and 10% brucellosis. Slow progression characterizes joint tuberculosis, an uncommon variant of this disease. The onset is usually insidious, and early diagnosis requires a high index of clinical suspicion. We report two cases with tuberculous sacro-ileitis which initially mimicked brucellosis infiltration. Diagnosis of tuberculosis of the sacroiliac joint was established by fine-needle aspiration of joint and radiological imaging methods such as computerized tomography, magnetic resonance and three-phase bone scan. The c

Prognosis of tuberculous meningitis in adults in the era of modern antituberculous chemotherapy.

This study reviewed the clinical manifestations and outcome of tuberculous meningitis in the era of modern antituberculous chemotherapy and applied these data in assessing the role of clinical staging evaluated 30 days after treatment in predicting long-term outcome. A total of 41 adult patients with tuberculous meningitis hospitalized at a university hospital in Taiwan from June 1994 through August 1999 were included in this retrospective study. Their age ranged from 16 to 80 years (median, 41 years), and 17 (41.5%) patients had had a variety of underlying immunocompromising diseases. Fever (

Urate excretion: drug interactions.

A derivative of probenecid, 2-nitroprobenecid, was studied in chimpanzees and Cebus monkeys. The uricosuria induced by the drug could be diminished by the infusion of p-aminohippurate (chimpanzee) or hippurate (monkey). Both hippurates inhibited the secretion of the drug and it is likely that the diminished response was the result of decreased access of 2-nitroprobenecid to its site of action. In contrast, pyrazinoate diminished the response to 2-nitroprobenecid without disturbing its renal disposition (both species). This action of pyrazinoate is attributed to its ability to inhibit the secre
