Uncertainty and relative risks of radiation exposure.

Exposure to ionizing radiation in a nonmilitary emergency is likely to alarm those persons exposed and their families. Not only is the outcome uncertain, but the risks are difficult to convey. This communication discusses uncertainties in short-term outcome and long-term effects, as well as means of conveying the magnitude of risks to patients and their families. It emphasizes that the perception of radiation hazards is contextual. It offers three ways of presenting the prognostic long-term risks of accidental radiation exposure by comparing them with the incidence of cancer and genetic abnorm…

[Radiation-induced damage to the large arteries].

The extracranial arteries of 28 irradiated patients with malignant tumors of the head and neck were examined by continous wave (CW) Dopplersonography and duplex-scan. The results were compared with those obtained from 556 healthy people of the same average age. The mean period between irradiation and examination was 7.3 years. Dopplersonography revealed stenoses or occlusions of the extracranial arteries in 9.1% of normal persons. In the group of irradiated patients 21% had stenoses or occlusions of the extracranial arteries. The results show, that the incidence of stenosis or occlusion of ext…

Clinical and nutritional implications of radiation enteritis.

The clinical and nutritional significance of radiation enteritis was assessed in eight patients with chronic diarrhea which followed curative doses of radiotherapy for pelvic malignancies. Steatorrhea, found in seven malnourished patients, was ascribed to ileal disease or previous surgery, or to bacterial contamination of the small intestine. Lactose intolerance, assessed by breath hydrogen excretion after oral lactose and by jejunal lactase levels, was found in six patients. In a subgroup of five patients, the administration of two different defined formula liquid diets by nasoduodenal infusi…

Modulation of immune function by UV radiation.

In addition to its carcinogenic activity, ultraviolet (UV) radiation is capable of modifying certain immunologic reactions. Immunologic alterations induced in mice by UV radiation include both local and distant effects. Local alterations result from a direct effect of UV radiation on an immune reaction that takes place at the site of irradiation. Distant alterations are those in which exposure of skin to UV radiation at one site modifies an immune reaction occurring at a distant, unexposed site. Based on recent studies, we propose that there may be two types of distant alterations. One is nons…

Surgery for radiation-induced carcinoma of the hypopharynx.

Eighteen radiation-induced tumors of the hypopharynx examined during a 20-year period are described. These tumors represent slightly less than 5 per cent of all tumors of the hypopharynx examined during the same period. Most were due to irradiation for thyrotoxicosis, and the mean latent period was 35 years. Five patients were not treated, and 13 patients were treated, mainly by pharyngolaryngectomy followed by skin flap repair (deltopectoral or pectoralis major). Despite a fairly high complication rate, an adjusted five-year survival of 35 per cent was achieved.

Pterygium and ultraviolet radiation: a positive correlation.

Pterygium has long been believed to be an environmental disease, Ultraviolet (UV) radiation playing a major role in its development. In examination of more than 100 000 Aborigines and non-Aborigines in rural Australia a strong positive correlation between climatic UV radiation and pterygium prevalence was found, providing further evidence of a causal relationship. In addition interesting differences were found in prevalence in the different racial groups and between the sexes. These differences may be explained by differences in lifestyle.

Radiation therapy of carcinoma of the tonsillar region.

From 1960 through 1976, 353 consecutive patients with carcinoma of the tonsillar region were primarily treated with radiation therapy. The mean age of patients was 58 years with males predominating (78%). Patients were classified according to a TNM and stage classification. Most of the tumors were large, and the specific site of origin could not be determined in 33% of the patients. The predominant identifiable sites were the soft palate and uvula, 18%, the anterior tonsillar pillar, 18%, and the tonsillar fossa, 13%. Patients were treated with radiation to doses of 6700 rad given in 48 days a…

Discounting in assessment of future radiation effects.

There is a question as to whether cancer fatalities to be experienced in the distant future as a result of radioactivity produced today shou … se for this purpose is much more cost-effective than spending money now to reduce their exposure to radiation. The history of interest rates over the past 5000 yr indicates that at least 3% real annual interest can be expected. It may not be necessary to actually set up a trust fund as its purpose is largely accomplished by the decrease in the public debt w …

The influence of radiation on the skin and the basis of protection.

Synopsis This paper reviews current ideas concerning the reactions involved in sunburn, suntanning, premature ageing and carcinogenesis due to acute or chronic exposure to sunlight. The physiological mechanisms of protection involving melanin pigmentation, thickening of the epidermal horny layer and urocanic acid are briefly discussed. The importance of wavelengths from 290-315 nm (UV-B) for these reactions is emphasized as is the need to investigate further the involvement of longer wavelength (UV-A, 315-400 nm).

Results of spinal fusion for radiation scoliosis.

There were 150 children with neuroblastoma and 183 children with Wilms' tumor who received radiation therapy, orthovoltage up to 1967, and cobalt therapy thereafter. In all except two cases, radiation therapy crossed the midline. Of all of the children treated, 13 required spinal fusion. There were six with Wilms' tumor; three had kyphosis; and three had scoliosis. Nine fusions were done with three pseudoarthroses, one broken rod, and two hook pullouts. There were seven with neuroblastoma, of whom five had kyphosis and two had scoliosis. Nineteen spinal fusions were done on these seven patient…

The role of radiation therapy for carcinoma of the lung.

A large proportion of patients with carcinoma of the lung may benefit from the use of radiation therapy. Operable patients have not been shown to benefit from preoperative irradiation, but postoperative irradiation has improved survival in those found to have involvement of hilar or mediastinal lymph nodes. Radiation therapy is the only potentially curative treatment for patients who are inoperable, but do not have distant metastasis. Control of the local tumor is very dependent upon dose-fractionation-time relationships. Patients who are relatively asymptomatic, i.e., they have a high perform…
