Periapical radiolucency mimicking an odontogenic cyst.

AIM: To present a clinical case of a giant cell lesion located in an unusual site, initially misdiagnosed and treated as an odontogenic cyst. SUMMARY: Periapical radiolucencies often suggest the presence of odontogenic pathosis, usually inflammatory granulomas or cysts. The high frequency of such lesions tends to lead clinicians to arrive at a diagnosis without completing a comprehensive assessment of the patient or carrying out the full range of available diagnostic tests. A case report of a giant cell lesion, which was misdiagnosed and treated initially as an odontogenic lesion because of i

Efficiency of polylactide/polyglycolide copolymers bone replacements in bone defects healing measured by densitometry.

Healing of bone defects is the most frequent cause of failure in surgical treatments of an odontogenic ostitic processes. The aim of this study was to determine successfulness in healing of bone defects after implantation of alloplastic co-polymer-polyglycol bone implant. A group of 45 cases with periradicular ostitic processes were examined. The densitometric measurements were taken from radiovisiographic dental x-ray images. Patients were observed throughout a period of one year. The results obtained were analyzed and presented graphically. Thirty-eight patients (84%) were treated successful

Glycoproteins expression in apical pathologic tissues: clinical incidences.

Indirect immunofluorescence was used for the localization of the primary collagens, fibronectin and laminin. Specimens were extracted from untreated teeth with periapical lesions from patients 20 to 30 years of age. An histological examination enabled the differentiation of granulomas and cysts, and 5 microns sections were used for the indirect immunofluorescence procedure. Antibodies against Type I, Type III, and Type V collagen and for fibronectin and laminin were obtained from glycoproteins of human cells. The antibody against Type IV collagen was prepared from Type IV collagen of beef reti

[Ocular manifestations of dental disease (apropos of one case)].

Dental infections are able to produce ocular diseases by direct metastatic reflex or allergic pathways. Generally, these diseases quickly heal after a symptomatic local treatment with antibiotics and anti-inflammatory medications associated to an etiologic therapy in order to eliminate the dental infection and the allergic hypersensitivity. The dental surgeon has a key position by promptly establishing a proper diagnosis and an efficient local treatment: the prognosis depends on these factors.

Identification of cellulose fibers in oral biopsies.

Oral biopsies comprising five periapical cysts, three cases of exuberant granulation tissue in extraction sockets and one case of a mandibular radiopaque structure, all displaying material consistent with cellulose fibers on routine histologic examination, were subjected to histochemical, polarization, fluorescence and scanning electron microscopical, energy dispersive X-ray and chemical investigations. Routine screening of periapical lesions with polarization microscopy and identification of possible cellulose fibers with Calcofluor White is advocated. Further, attention is drawn to endodonti

Cells infiltrating human periapical inflammatory lesions.

Cells infiltrating human periapical granulomas and cysts were analyzed using cell suspensions. Over 50% of the total cells studied were lymphocytes whilst neutrophils were the next most frequent cell type observed. Two percent of the cells were mast cells. Only a few monocytes/macrophages and natural killer cells were found. More than half of the lymphocytes infiltrating the lesions were T cells, and more than half of the T cells were of the suppressor/cytotoxic (OKT8) phenotype. The OKT4/OKT8 ratio was 0.5. The findings are compared to previous reports and the differences discussed.

Tumour-simulating squamous cell metaplasia (SCM) in necrotic areas of salivary gland tumours.

Squamous cell metaplasia (SCM) adjacent to necrotic areas of salivary gland tumours must be distinguished from other types of SCM (focal SCM in the excretory ducts of salivary glands; necrotizing sialometaplasia; focal SCM within salivary gland tumours) in respect to the tissue structure. Based on the high cellular proliferation, arcade- or cord-like pseudoneoplastic SCM develops with stellate extension in the surrounding tissue and focal inclusion of goblet cell metaplasia. This proliferative SCM resembles the cellular demarcation of radicular dental cysts. In the Salivary Gland Register 8 ca

Histopathological analysis of 30 specimens of glandular odontogenic cyst.

Glandular odontogenic cysts (GOC) are relatively rare, and require differentiation from radicular cysts, dentigerous cysts, and central mucoepidermoid carcinoma of jawbone. We conducted clinical, histopathological, and immunohistochemical analysis to elucidate the relationship between the clinical behavior and histopathological features, as well as the origin of tissues and factors related to the development of GOC. Thirty specimens from 29 patients (19 males, 11 females; mean age 51.8, range 18-85 years) diagnosed histopathologically with GOC were studied. By location site, mandibular molar r…

Diagnostic value of dynamic contrast-enhanced MRI for unilocular cystic-type ameloblastomas with homogeneously bright high signal intensity on T2-weighted or STIR MR images.

Typical MR images of ameloblastomas on T2-weighted image (WI) or short inversion time inversion-recovery (STIR) show multiple bright high-signal-intensity loci on a high-signal-intensity background. Unilocular cystic-type ameloblastomas show homogeneously bright high signal intensity on T2WI or STIR as a water-like signal intensity. Therefore, it is difficult to distinguish unilocular cystic-type ameloblastoma from other cystic lesions such as keratocystic odontogenic tumors, radicular cysts (residual cysts) and dentigerous cysts only on the basis of MRI signal intensity. In the present study,…

Cemento-osseous Dysplasia Mimicking Periapical Cyst After the Orthodontic Treatment of the Mandible.

Cemento-osseous dysplasia (COD) is a non-neoplastic condition, characterized by the replacement of normal cancellous bone with fibrous tissues containing woven bone and cementum in the tooth-bearing areas. This report presents the case of a 21-year-old male who developed COD following orthodontic treatment. A panoramic radiograph showed a periapical radiolucent lesion in the right mandibular canine area. Cone beam computed tomography revealed a mixed radiolucent and radiopaque lesion. Histologically, the lesion exhibited cementum-like calcifications and woven bone scattered within the hemorrha

Construction of Preoperative Diagnostic Model of Periapical Cyst by Multiparameter Magnetic Resonance Imaging: A Pilot Study.

AIM: Due to the lack of a predictable and noninvasive method for distinguishing different types of apical periodontitis, it is difficult to estimate the actual prognosis of periapical cysts and the effectiveness of nonsurgical treatment for them. This pilot study aimed to establish a diagnostic model to differentiate periapical cysts and nonperiapical-cyst lesions based on parameters derived from magnetic resonance imaging (MRI). METHODOLOGY: Before endodontic microsurgery, MRI scans were performed for the patients with periapical lesions, including T2-weighted images (T2WI), noncontrast-enha

Bacterial foci in the teeth, oral cavity, and jaw--secondary effects (remote action) of bacterial colonies with respect to bacteriospermia and subfertility in males.

Bacteriospermia requiring medical treatment were diagnosed in more than 70% of the subfertile patients who had since 1988 attended the gynecological clinic at the RWTH hospital in Aachen. In 23% of all cases specific treatment with antibiotics did not reduce the concentrations of bacteria in sperma. Thirty-six patients with bacteriospermia resistant to antibiotic therapy were then subjected to dental examination. A high incidence of potential dental foci was found in all patients. In a test group of 18 patients these sources of potential infection were eliminated. Between dental operations and

[The clinical efficacy of roxithromycin in patients with acute odontogenic infections].

The clinical efficacy of a macrolide antibiotic, roxithromycin, was evaluated in 24 patients affected by acute odontogenic infections. Patients were treated with a first dose of 300 mg p.o. which was followed by 150 mg p.o. 12-hourly for the following two days. Total responses were evaluated by an arbitrary scale. The results showed that an excellent and good response was obtained in 99.96% of treated patients. On the basis of clinically compared data and the drug safety it may be concluded that roxithromycin can be successfully used in the treatment of odontogenic infections.

[Study on the causes of post-endodontic failures: a descriptive analysis of 198 cases of reoperation].

In this article, a statistical analysis describes 198 cases of post-endodontic failures referred from private practitioners for surgical retreatments. Biopsies of these lesions showed that 68.18 per cent of them were identified as apical granulomas, 3.5 per cent as cysts, 16.16 per cent as epitheliated granulomas and 12.12 per cent as "other" cases. The two main groups of causes associated with these failures are: technical errors (55.05 per cent of the cases) and procedural mistakes (17.68 per cent).
