A Novel Method for Treating Distal Radius Diaphyseal Metaphyseal Junction Fracture in Children.

BACKGROUND The treatment of distal radius diaphyseal metaphyseal junction (DMJ) fracture in children is a clinical problem; several treatments are available, but none are very effective. Therefore, this study aimed to report a novel method for treating this fracture using limited open reduction and transepiphyseal intramedullary fixation with Kirschner wire. MATERIAL AND METHODS From January 2018 to December 2019, a total of 15 children (13 boys and 2 girls) with distal radius DMJ fractures with a mean age of 10 years (range: 6-14 years) were included in the study. The operation time, incision

Association between perceived and objective hand-wrist function in distal radius fracture.

The relationship between patient-reported outcome measures and objective measures is important for understanding patient expectations. The aim of this study was to investigate the relationship of each section of the Michigan Hand Outcomes Questionnaire (MHQ) to objective measurements in patients with distal radius fracture (DRF). Eighty-four patients who were treated operatively or non-operatively for DRF were included. Injury severity was assessed by the Modified Hand Injury Severity Score. Joint motion was assessed at 6 and 12 weeks and grip strength at 12 weeks after non-operative treatment

Outcomes of Arthroscopic-Assisted Distal Radius Fracture Volar Plating: A Meta-Analysis.

PURPOSE: The use of wrist arthroscopy to assist fixation of distal radius fractures with volar locking plates (VLPs) has been gaining popularity in recent years; however, there is no consensus on its benefits. This meta-analysis compares outcomes of arthroscopic-assisted VLP to fluoroscopic-assisted VLP in distal radius fractures through a systematic review of the published literature. METHODS: A systematic search of publications from databases (Medline, EMBASE, Scopus, and Cochrane) was obtained from inception to May 2020. A random-effects meta-analysis was used to calculate effect sizes. Ou…

Complex Regional Pain Syndrome and Distal Radius Fracture: Etiology, Diagnosis, and Treatment.

Complex regional pain syndrome (CRPS) is a chronic pain condition characterized by a constellation of signs and symptoms including pain out of proportion to the injury or insult, autonomic dysfunction, trophic changes, and impaired function. CRPS may occur following either conservative or surgical management of distal radius fractures and can significantly complicate the trajectory of a patient's recovery. Although the incidence, diagnosis, prevention, and treatment of this condition have been extensively studied, optimal methods to identify, prevent, and treat this condition remain controvers…

Are Junior Residents Competent at Closed Reduction and Casting of Distal Radius Fractures in Children?

BACKGROUND: At many institutions, junior orthopaedic surgery residents perform the closed reduction and casting of pediatric distal radius fractures (DRFs). The purpose of this study was to evaluate the competency of junior residents compared to senior residents in the initial management of pediatric DRFs. METHODS: This investigation was a case-control study analyzing the outcomes of children with displaced DRFs treated by junior versus senior residents. The cohorts were matched with respect to fracture type. Radiographs were measured to assess fracture angulation, displacement, and cast inde…

The Predictive Factors of Displacement of Adult Distal End Radius Fracture Treated with Casting.

Background: In the conservative management of distal radial fractures (DRFs), the optimal dorsi-volar angulation of the wrist at cast immobilization and proper cast molding to minimize the risk of redisplacement are unclear. This study identified the predictors of fracture redisplacement during cast immobilization for adult DRFs. Methods: We examined for dorsi-volar angulation, gap index, volar tilt (VT), radial inclination (RI), and radial length (RL) in lateral or posteroanterior radiographs of 90 DRFs. We investigated possible predisposition factors for redisplacement including patient age,

Early Retrieval of Spanning Plates Used for Fixation of Complex Fractures of the Distal Radius.

Background  Spanning plates are being increasingly used for the treatment of complex fractures of the distal radius. The traditional recommendation is to leave the hardware in place for at least 12 weeks. Questions/Purpose  This study assesses the comparative outcomes of spanning plates removed at or before 10 weeks. We hypothesized that acceptable healing and functional outcomes can be achieved with earlier hardware removal to allow for earlier range of motion, rehabilitation, and return to function. Patients and Methods  All patients treated for a comminuted, intra-articular distal radius fr

Transitioning from hurting to healing: self-management after distal radius fracture.

PURPOSE: Chronic pain and disability after musculoskeletal trauma support the need for non-biomedical perspectives to frame experiences of recovery. Self-management is a specific holistic lens with implications for improving health outcomes. This qualitative study explored how adults made meaning of self-management after distal radius fracture (DRF). METHODS: Thirty-one individuals aged 45-72 with a unilateral DRF participated in a semi-structured interview 2-4 weeks after discontinuing full-time wrist immobilization. Interviews were audio-recorded and transcribed, and constructivist grounded

Classification and Management of Failed Fixation of the Volar Marginal Fragment in Distal Radius Fractures.

Greater understanding of specific fracture patterns following distal radius fractures has arisen with the advent of volar plating. The volar marginal fragment (VMF) is a small peripheral piece of bone which is critical to carpal stability. Failure to achieve good fixation of the VMF can result in volar subluxation of the carpus and distal radioulnar joint instability. Due to its small, distal nature, this fragment can be easily missed and difficult to fix. Loss of reduction of the VMF following operative fixation presents specific challenges and surgical considerations dictated by patient char…

Distal radius sections offer accurate and precise estimates of forearm fracture load.

BACKGROUND: Forearm fracture risk can be estimated via factor-of-risk: the ratio of applied impact force to forearm fracture load. Simple techniques are available for estimating impact force associated with a fall; estimating forearm fracture load is more challenging. Our aim was to assess whether failure load estimates of sections of the distal radius (acquired using High-Resolution peripheral Quantitative Computed Tomography and finite element modeling) offer accurate and precise estimates of forearm fracture load. METHODS: We scanned a section of the distal radius of 19 cadaveric forearms

Changes in the Rate of Postoperative Flexor Tendon Rupture in Patients with Distal Radius Fractures.

Background: Flexor tendon rupture is a major complication after volar locking plating for distal radius fracture (DRF). Few studies have investigated changes in the rate of postoperative flexor tendon rupture in patients with DRFs. The present study aimed to investigate the changes in the rate of postoperative flexor tendon rupture and to assess plate placement and reduction positions. Methods: We retrospectively reviewed patients in whom more than 24 months had passed since DRF surgery. The patients were interviewed by telephone. Forty-nine patients (50 fractures; 2007-2009) from institution

Ultrasonographic Classification of Scapholunate Interosseous Ligament Injury Associated With Distal Radius Fracture.

PURPOSE: Although scapholunate interosseous ligament (SLIL) injury is often associated with distal radius fracture, it is frequently missed in x-rays. The purpose of this study was to develop a diagnostic classification based on ultrasonography for SLIL injury and evaluate its diagnostic accuracy. METHODS: Patients who required surgery for distal radius fracture were included in this study. Ultrasonography and arthroscopy were performed in all patients. The original ultrasonography-based classification of the dorsal part of SLIL was developed as follows: for type A, the SLIL surface was outli

The Use of a Vascularized Distal Ulna Autograft for Complex Distal Radius Fracture Nonunions.

In patients with segmental nonunion of the distal radius, the distal ulna and distal radioulnar joint may not be salvageable. As an alternative to a vascularized free fibula, a pedicled distal ulna vascularized graft is a useful salvage technique. The procedure relies on the vascular bundle of the pronator quadratus and the dorsal oblique arterial anastomosis between the anterior and posterior interosseous arteries running along the interosseous membrane as the pedicle. We present 3 patients who received a distal ulna vascularized graft with concomitant wrist arthrodesis for distal radius segm

Use of a Large Tenaculum Clamp as a Reduction Technique for Treatment of Distal Radius Fractures.

Distal radius fractures are common upper extremity injuries requiring surgical treatment. In the context of management with a volar locking plate (VLP), a number of described techniques assist with restoration of individual anatomical parameters such as radial length, volar tilt, and articular congruity. We present a surgical technique that utilizes a large tenaculum bone clamp to provide an efficacious reduction in several planes. With anteroposterior compression, the clamp enables volar translation of the distal fracture fragment. This compression also decreases the interval between the dist…

Reduction Loss After Distal Radius Fracture Fixation With Locked Volar Plates.

Distal radius fractures are among the most common injuries in the upper extremity. While many studies have looked at the maintenance of reduction with volar locking plates, there is a paucity of literature comparing the ability of different plates to maintain reduction over time. This study reviews the ability of various plates to maintain radiographic reduction at union after distal radius fracture treatment. Loss of some aspect of fracture reduction was routinely observed following locked volar plating regardless of implant. However, choice of implant did have a significant impact on final r…
