Traumatic brain injury and stroke often result in contralateral sensorimotor deficits, including asymmetric posture. While these lateralized effects are classically attributed to neural tract disruption, recent evidence supports the existence of a complementary humoral pathway. Our previous work demonstrated that the neurohormone Arg-vasopressin (AVP) mediates right hindlimb responses to left-sided brain injury, whereas the dynorphin - κ-opioid receptor system is involved in regulation of left hindlimb reflexes following right-sided lesions. We hypothesized that AVP-expressing hypothalamic neu

Three adult patients with unilateral optic neuropathy, seropositive for Jamestown Canyon virus (JCV) IgM and acute infection, experienced acute unilateral vision loss. Two cases had swollen optic nerves with vision loss in a pattern suggestive of papillitis, whereas the third had retrobulbar optic neuritis. All presented with vision loss rather than typical meningoencephalitis symptoms (e.g., nuchal rigidity, photophobia, and headache). Case 1 had 20/20 vision (right/left), relative afferent pupillary defect (left), and marked optic disc edema. Perimetry revealed an increased blind spot. Final

BACKGROUND: Microtia is a congenital anomaly of the external ear that often leads to hearing loss due to associated auditory canal and middle ear malformations. While right-ear predominance and conductive hearing loss are well-documented, few studies have compared anatomical and audiological differences between sporadic and familial cases, especially in underrepresented populations.
OBJECTIVE: To compare the clinical, anatomical, and auditory characteristics of patients with sporadic versus familial microtia in an Ecuadorian cohort.
METHODS: A cross-sectional study was conducted on 146 patie

We report a 46-year-old man with bacterial endocarditis and cardiac failure, who developed status epileptics. The patient was apparently well until July of 1991 when there was a gradual onset of fever and general fatigue. He was hospitalized to the cardiology service of our hospital where diagnosis of bacterial endocarditis and aortic insufficiency was made. On October 9, 1991, he suddenly developed cardiogenic shock, and emergency replacement of the aortic valve was made; at the operation, the main trunk of the left coronary artery showed embolic occlusion, and the myocardial movement was mar

Moyamoya disease (MMD) is a rare progressive cerebrovascular disorder characterized by stenosis of the terminal internal carotid arteries with fragile collateral vessel formation. It is common in East Asia but underreported in Nepal. Pediatric cases typically present with ischemic symptoms such as transient ischemic attacks, stroke, or seizures. We report a 7-year-old Nepali boy with 11 days of fever followed by recurrent tonic-clonic seizures, progressive right-sided weakness, and impaired fine motor function. Examination showed right hemiparesis with an upgoing plantar reflex. MRI and MR ang

BACKGROUND: Vestibulo-ocular reflex (VOR) function is expected to be normal in patients with presbycusis during sudden head rotations.
AIM: This study aimed to determine whether presbycusis was accompanied by vestibular system pathologies. In addition, it was examined whether there was a difference existed between the patients with and without presbycusis in terms of normative data.
MATERIALS AND METHODS: A total of 40 individuals were included in the study: 20 in the presbycusis group and 20 in the control group. The vestibular systems of both groups were evaluated using the video head impu

Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barre syndrome (GBS) which usually presents with descending paralysis. Common symptoms are ophthalmoplegia, ataxia, and areflexia. Our case presented with an atypical presentation. A 52-year old lady presented to the neurology outpatient department with frequent falls, blurring and doubling of vision and difficulty swallowing. These symptoms followed mild non-bloody diarrhea for two weeks ago. She had bilateral ptosis, lateral gaze palsy in both eyes, absent gag and cough reflex; she was unable to walk in a straight line and had right-

We report the case of a 35 year old patient presenting with acute onset of right periocular pain with palpebral edema due to nose blowing with absence of trauma. Early assessment showed periorbital emphysema without ophthalmological involvement, normal pupillary light reflex and visual acuity. Computerized tomography (CT) scan of facial and orbital bones objectified right pneumo-orbita with orbital fat herniation via a partial agenesis of the lamina papyracea, without muscle incarceration. Emphysema gradually reduced after two weeks by applying pressure dressing associated with broad spectrum

BACKGROUND: Extraosseous Ewing sarcoma (EES) arising from peripheral nerves is extremely rare with only 11 cases mentioned in the literature. It poses a diagnostic challenge to the clinicians due to its nonspecific presentation and rare occurrence.
CASE DESCRIPTION: A 56-year-old woman came with a history of low back pain and intractable right leg pain. Physical examination revealed the normal motor power of both lower limbs with decreased sensation along the lateral aspect of the foot with decreased ankle reflex. There was no improvement with conservative management. It was a diagnostic chal

We report about a patient, who in 2005, as a 37-year-old, experienced a sudden, mild to moderate, mid-frequency sensorineural hearing loss in the right ear, along with tinnitus and mild dizziness. MRI of the temporal bone revealed a very small (1 mm) enhancing lesion in the second turn of the right cochlea after injection of contrast medium. Hearing gradually deteriorated, with complete hearing loss in 2015. At this time, an MRI scan showed a lesion completely filling the cochlea. The tumor was removed through a subtotal cochleoectomy. The cochlea was partially reconstructed with cartilage and

In the antisaccade task, subjects are requested to suppress a reflexive saccade towards a visual target and to perform a saccade towards the opposite side. In addition, in order to reproduce an accurate saccadic amplitude, the visual saccade vector (i.e., the distance between a central fixation point and the peripheral target) must be exactly inverted from one visual hemifield to the other. Results from recent studies using a correlational approach (i.e., fMRI, MEG) suggest that not only the posterior parietal cortex (PPC) but also the frontal eye field (FEF) might play an important role in su

UNLABELLED: A 69-yr-old woman was admitted to the intensive care unit after cardiac surgery. Immediately after the discontinuation of cardiopulmonary bypass, she had a circulatory arrest. A 13-min open-chest cardiac massage was followed by 70 min of cardiopulmonary bypass. Right hemiplegia and right extensor plantar reflex were noted after the patient awakened. She had been included in a prospective study protocol measuring, before and after surgery, cerebral blood flow with transcranial Doppler (TCD). The data were retrospectively analyzed, and it was established that the TCD had recorded cer

The authors report the initial results of stenting in four patients of Takayasu arteritis for 11 occlusive carotid and subclavian arteries between January 1999 and December 2000. The lesions included stenoses of two right subclavian, three right common carotid, two left common carotid, and two left subclavian arteries, and total occlusion of two subclavian arteries. A total of 14 stents were implanted in 10 arterial lesions, resulting in a 91% procedural success rate. One failure was due to inability to cross the total occlusion of the subclavian artery. Procedural complications and problems w

A 58-year-old man developed dysarthria followed by a personality change. Subsequently, he developed muscle weakness and atrophy of the left upper and lower limbs, leading to repeated falls when he tried to walk. Neurological examination showed mild dementia, dysarthria, dysphagia, atrophy and fasciculation of the tongue, and muscle weakness and atrophy of all four extremities, particularly on the left side. Deep tendon reflexes were slightly diminished in the upper limbs and slightly exaggerated in the lower limbs without Babinski's sign. Cranial MRI revealed marked atrophy of the medial porti

The vertical and torsional vestibulo-ocular reflexes (VOR) were investigated in 3 patients with surgical occlusion of the posterior semicircular canal and 1 patient with singular neurectomy, for treatment of refractory paroxysmal positional vertigo. Stimuli comprised sinusoidal oscillation in the coronal ("roll") and sagittal ("pitch") plane as well as in two oblique planes intermediate between pitch in order to stimulate left anterior + right posterior (LARP) and right anterior + left posterior (RALP) canal pairs separately. One case with left side BPPV was investigated pre and post-operative

Sarcoidosis is a multisystemic, noncaseating granulomatous disease of unknown etiology. Neurosarcoidosis (NS) is the involvement of the central nervous system (CNS) in sarcoidosis, and it occurs in approximately 5%-10% of cases. NS can present with a variety of clinical features, making diagnosis challenging. A comprehensive diagnostic approach is required to obtain a definitive diagnosis. In this case we present a 13-year-old boy with diabetes mellitus presented with acute right-sided weakness, paresthesia, headaches, and episodes of loss of consciousness, followed by confusion and aggressive
