Breathing exercises for dysfunctional breathing/hyperventilation syndrome in children.

BACKGROUND: Dysfunctional breathing is described as chronic or recurrent changes in breathing pattern causing respiratory and non-respiratory symptoms. It is an umbrella term that encompasses hyperventilation syndrome and vocal cord dysfunction. Dysfunctional breathing affects 10% of the general population. Symptoms include dyspnoea, chest tightness, sighing and chest pain which arise secondary to alterations in respiratory pattern and rate. Little is known about dysfunctional breathing in children. Preliminary data suggest 5.3% or more of children with asthma have dysfunctional breathing and

Pneumococcal vaccines for children and adults with bronchiectasis.

BACKGROUND: Bronchiectasis is increasingly recognized as a major cause of respiratory morbidity especially in developing countries. Even in affluent countries, bronchiectasis is increasingly seen in some community subsections (e.g. Aboriginal communities) and occurs as a comorbidity and disease modifier in respiratory diseases such as chronic obstructive pulmonary disease (COPD). Respiratory exacerbations in people with bronchiectasis are associated with reduced quality of life, accelerated pulmonary decline, hospitalisation and even death. Conjugate pneumococcal vaccine is part of the routine

[Exposure to environmental tobacco smoke and respiratory symptoms in school children of Nis].

INTRODUCTION: There is relatively strong evidence among children showing that passive smoking is associated with respiratory symptoms and respiratory diseases, but few studies have been carried out in Serbian population. OBJECTIVE: Establishment of association between exposure to tobacco smoke within family and prevalence of the upper and lower respiratory symptoms in school children. METHOD: In this descriptive epidemiological study, studied population consisted of 533 school children, 12.96 +/- 1.54 years old, residents of Nis town, from the area with identical open air pollution. Environm

Blood, breath, fears redux, and panic attacks: comment on Roth, Wilhelm, and Pettit (2005).

This article discusses the hyperventilation theory (HVT) of panic attacks (PAs) proposed by R. Ley in the context of criticisms raised by W. T. Roth, F. H. Wilhelm, and D. Pettit. Their interpretation of HVT is flawed by misunderstanding, misinterpretation, and oversimplification of the complex psychophysiological principles on which current HVT is based. Consequently, the "propositions" they derive are inappropriate. Further, Roth et al. disregarded/overlooked: (a) differences among types of PAs, (b) abundant empirical evidence that PA patients are distinguished by presence or absence of prom

Cardiorespiratory findings in sudden unexplained/unexpected death in epilepsy (SUDEP).

Sudden unexplained/unexpected death in epilepsy (SUDEP), with an incidence of 0.35-9.3/1000 patient-years depending on the severity of epilepsy, remains a diagnostic and therapeutic challenge. Potential pathomechanisms comprise cardiac arrhythmia, due to myocardial ischemia, electrolyte disturbances, arrhythmogenic drugs, or transmission of the epileptic activity via the autonomic nervous system to the heart, and central or obstructive apnea. In most studies on SUDEP, data are lacking about the family and patient's own clinical history, cardiovascular symptoms, concomitant diseases and prior f

[Respiratory emergencies].

Respiratory emergencies in a patient with cancer can have their origin in pathologies of the airway, of the pulmonary parenchyma or the large vessels. The cause can be the tumour itself or concomitant complications. Obstruction of the airway should be initially evaluated with endoscopic procedures. Surgery is rarely possible in serious situations. The endobronchial placement of stents or radioactive isotopes (brachytherapy), tumoural ablation by laser or photodynamic therapy can quickly alleviate the symptoms and re-establish the air flow. Treatment of haemoptysis depends on the cause that is

["Silent" gastroesophageal reflux and upper airway pathologies in childhood].

In children, gastroesophageal reflux (GER) plays an important role in both acute and chronic upper airway disorders including stridor, chronic cough, recurrent upper respiratory infections, obstructive apnea, laryngospasm, and wheezing. Diagnosis may prove difficult unless there is reason to suspect GER and one is aware of the concept of "silent" GER. This paper presents our experience with chronic and/or recurrent respiratory disorders of uncertain origin and without gastrointestinal symptoms in children. Thirty-two pediatric patients with upper respiratory symptoms were evaluated. Out-patien

A review of the respiratory disorder in the Rett syndrome.

The characteristics and physiological accompaniments of the respiratory disorder in the Rett syndrome are reviewed and placed in the context of other clinical studies. Events are indicated which may play a part in the occurrence of vacant spells. A scheme is proposed to explain the various manifestations of the Rett syndrome disorder throughout life. The Rett syndrome is viewed as a developmental disorder in which the evolving clinical signs are due to the effects of maturation and aging on an abnormal brain rather than to primary degenerative disease. Thus development elicits preexisting defe

[Respiratory problems during sleep].

Continuous recording of numerous physiological variables during the night has led to the discovery of various respiratory disorders occurring during sleep. The most important are: 1. central apneas, defined by excessive frequency of respiratory arrests of excessive duration, 2. obstructive apneas, characterized by marked obstruction at the level of the oropharynx with persistence of respiratory movements, 3. episodes of marked nocturnal hypoxemia, without apnea, in certain patients with chronic obstructive lung disease. The pathophysiology, clinical picture and possible therapeutic resources a

Presumed homozygous achondroplasia. A review and report of a further case.

Presumptive homozygous achondroplasia (PHA) is a rare disorder. The clinical and detailed histological findings of the fifth case of this chondrodystrophy, studied in detail, are presented here. There is a severe disorder of the normal growth of the long bones, whose nature, however, is not well understood; it shows some resemblance to the pathological changes described in thanatophoric dysplasia which also presents many clinical similarities. The aspect of greatest interest, however, is the relation of PHA to the "classical" form of achondroplasia. Some investigators have maintained that in t
