BACKGROUND: Cardiac surgery for congenital heart disease covers a wide spectrum from simple to complex cardiac and extracardiac malformations. Innovations in pediatric cardiac surgery and perioperative care over the past decades have allowed surgical correction or at least palliation in almost all complex congenital heart defects in the first years of life. Diaphragmatic paralysis (DP) due to phrenic nerve injury after congenital cardiac surgery is an important respiratory complication resulting with respiratory insufficiency, lung infections, prolonged hospital stay time and even death.

The trachea of a 50 year-old woman was intubated for management of respiratory failure following neurotoxic snake bite. She developed bilateral vocal cord palsy following intubation and was managed with tracheostomy and other supportive measures. Vocal cord palsy is a rare complication of intubation, which, in most situations, is preventable.

This article discusses the diaphragm from a surgical perspective. Although it is a relatively simple organ compared with other structures, the diaphragm serves important anatomic and functional roles necessary for proper respiratory function. It is an organ of little irregularity or disease, and easily manipulated in the operating room by those who have a basic understanding of its anatomic details.

OBJECTIVES: (1) To present a rare case of stridor secondary to prolonged laryngospasm in a patient with Parkinson's disease, and (2) to review the literature on stridor in Parkinson's disease.
METHODS: We report a 73-year-old Parkinson's disease patient who developed acute stridor due to prolonged laryngospasm triggered by overspill of excessive secretions. The literature was reviewed, following a Medline search using the keywords 'Parkinson's disease' and 'stridor' or 'airway obstruction' or 'laryngospasm' or 'laryngeal dystonia' or 'bilateral vocal cord palsy'.
RESULT: Only 12 previously r

Paradoxical vocal fold dysfunction (PVFD) with high effort exercise can result in disruptions to ventilation, dyspnea, inspiratory stridor, elevated heart rate, and syncope. This single subject study experimentally tested an inspiratory muscle strength training (IMST) program with behavioral therapy on a 15-year-old male crew member. Outcome variables were maximum inspiratory pressure (MIP), and dyspnea ratings. Following 5 weeks of IMST, MIP increased by 93% from baseline function while dyspnea ratings substantially decreased. Outcome included successful competition with his high-school crew

Platypnea-orthodeoxia is a rare syndrome that is often associated with interatrial shunting through a patent foramen ovale or atrial septal defect. We describe the case of a 65-year-old woman with progressive dyspnea and hypoxia when standing and walking, which was relieved by assuming the recumbent position. The diagnosis was confirmed by tilt-table transesophageal echocardiography demonstrating a large right-to-left shunt through a patent foramen ovale while the patient was in a semiupright position and no significant shunt while in a recumbent position. This case demonstrates that platypnea

It is presented the clinical record of a female newborn of 39 weeks of gestational age, with birth weight of 4600 g, with diaphragmatic palsy eight days after birth. Were realized diaphragmatic plicature and the clinical course was without any complication. Were reviewed the causes of diaphragmatic palsy and was described the relationship, with vein cannulation for parenteral nutrition.

A case of rabies was treated with intensive medical support. This led to a prolongation of life to 25 days. The neurologic progression of the disease was monitored clinically and with serial EEG. This prolonged course enabled us to witness and manage a wide variety of unusual complications including the adult respiratory distress syndrome, hypothermia, myocarditis, and diabetes insipidus. This report documents the clinical features, diagnostic problems, complications, and management.

Rare upper airway lesions may be mistaken for asthma. A 16-year-old Hispanic male athlete presented to our allergy clinic with a 4-month history of wheezing and snoring with hoarseness and progressive fatigue on exertion or during sleep. His mother taped periods of harsh stridor and sleep apnea. There was no family history of vocal cord abnormalities. A year before the onset of symptoms, he suffered injury to his oral cavity with a loss of consciousness during a wrestling match. He denied dysphagia or dysphonia. He failed to respond to bronchodilators, cromolyn, or prednisone therapy during 4

Takotsubo syndrome (TTS) is a transient stress-induced cardiomyopathy that typically presents with apical ballooning; focal variants are rare and may closely mimic acute coronary syndromes. We present the case of a 74-year-old woman who developed acute hypoxemic respiratory failure secondary to vocal cord and phrenic nerve paralysis following an interscalene block. During hospitalization, she exhibited isolated troponin elevation without chest pain or electrocardiographic changes. Coronary angiography demonstrated nonobstructive coronary arteries, while ventriculography revealed focal lateral

This report details a case of acute idiopathic velopharyngeal insufficiency in a previously healthy eight-year-old girl, presenting with sudden voice alteration and nasal regurgitation following mild respiratory symptoms. Physical examination identified unilateral velar paralysis with open rhinolalia, without additional neurological deficits. Extensive diagnostic evaluation, including nasopharyngoscopy, cerebral and cervical imaging, and infectious serologies, yielded unremarkable findings. In the absence of spontaneous improvement after one week, the patient received a brief course of prednis

In amyotrophic lateral sclerosis (ALS) and animal models of ALS, including SOD1-G93A mice, disassembly of the neuromuscular synapse precedes motor neuron loss and is sufficient to cause a decline in motor function that culminates in lethal respiratory paralysis. We treated SOD1-G93A mice with an agonist antibody to MuSK, a receptor tyrosine kinase essential for maintaining neuromuscular synapses, to determine whether increasing muscle retrograde signaling would slow nerve terminal detachment from muscle. The agonist antibody, delivered after disease onset, slowed muscle denervation, promoting …

Ten patients with intermediate syndrome (IMS) after acute organophosphorus (OP) pesticides poisoning were observed. The occurrence rate of IMS was 5.4% among total inpatients of acute OP poisoning in the same period. The majority of IMS cases were of severe oral OP poisoning. Seven to 68 hours after poisoning there were initial signs including motor weakness of muscles innervated by the 9th and 10th cranial nerves (e.g. dysphagia, hoarse voice). The neck flexor muscles and proximal limb muscles were also involved. Patients with respiratory muscle paralysis usually required urgent tracheal intu

Five cases of acute lead poisoning resulted from the self-injection of lead and opium pills which were crushed, heated and suspended in water. Two of the five patients died of illnesses in which hepatic failure and reversible acute tubular necrosis were prominent features. One of these two had a severe neuropathy, with flaccid quadriplegia and respiratory paralysis. The other three patients had relatively minor symptoms but unequivocal biochemical evidence of lead toxicity. Autopsy changes included hepatic degeneration with inclusion bodies, regenerating renal tubular epithelium and wasting of…
