BACKGROUND: Treatment of scaphoid nonunion with a vascularized bone graft based on the 1,2‑intercompartmental supraretinacular artery (1,2‑ICSRA) involves complex wrist anatomy. Traditional teaching methods rely mainly on anatomical atlases and two‑dimensional (2D) computed tomography (CT) images, making it difficult for students to mentally reconstruct three‑dimensional (3D) spatial relationships. Three‑dimensional digital anatomy technology can provide visual 3D structural information that clearly illustrates the spatial relationships among anatomical structures, thereby potentially improvin

INTRODUCTION: This is single centre retrospective review of a consecutive series of patients with scaphoid nonunion (SNU) treated using the Ilizarov technique without bone graft. Fifteen of the original 18 patients were available for clinical and radiological examination at a minimum follow-up of 5 years (range 5-10 years). An evaluation was made of the late functional results, satisfaction scores, residual symptoms, grip strength and the presence of radiocarpal and scaphoid degenerative changes.
METHODS: The series consisted of 15 patients; 14 males; 1 female, with a mean SNU duration of 15.

This article documents the outcome of treatment of intraosseous ganglia and simple bone cysts of the carpal bones by curettage and injectable calcium phosphate bone cement (CPC) grafting. The patients consisted of five men and three women. One had a cystic lesion in the scaphoid, one in the hamate, and five in the lunate. Curettage of the lesions was performed, and CPC was injected into the cavity. Five patients were diagnosed with a ganglion and three with a simple bone cyst. Among the five patients with wrist pain, the pain disappeared completely in four. Radiographs showed apparent partial

Preiser's disease, also known as avascular necrosis of the scaphoid, is a rare condition that is incompletely understood in regard to pathophysiology, diagnosis, and management. There have been numerous case reports and case series evaluating a variety of conservative and operative interventions, but optimal treatment has not been well established. We describe the case of a 20-year-old female with stage II Preiser's disease that was managed with a vascularized bone graft from the 1,2 intercompartmental supraretinacular artery, in addition to temporary dorsal wrist-spanning bridge plate fixatio

We compared the long-term outcome in 61 patients (62 fractures) treated operatively or conservatively for an acute fracture of the carpal scaphoid. A total of 30 fractures was randomised to conservative treatment using a cast and 32 to operative treatment using a Herbert bone screw. The duration of sick leave was shorter for patients treated by operation, but this was only significant in patients with blue-collar occupations. There were no differences between the groups in respect of function, radiological healing of the fracture, or carpal arthritis after follow-up at 12 years. Those managed

We report a case of avascular necrosis of the scaphoid, lunate and triquetrum in a young 21-year-old patient, after a purely ligamentous peri-lunate dislocation of the wrist. He presented with a Mayfield III peri-lunate dislocation after a road traffic accident and underwent an open reduction and internal fixation. Post-operatively, the scapho-lunate gap widened after removal of the temporary K-wires, and he gradually developed avascular necrosis of the scaphoid, lunate and triquetrum, and osteoarthritis of his wrist. We present this unusual case of simultaneous avascular necrosis of multiple

Proximal row carpectomy (PRC) is an effective treatment option for degenerative or posttraumatic osteoarthritis of the wrist in conditions such as scapholunate advanced collapse, scaphoid nonunion advanced collapse, Kienbock disease, and chronic fracture dislocations of the carpus. PRC involves excision of the scaphoid, lunate, and triquetrum, and relies on the articulation of the remaining capitate from the distal row to articulate with the lunate fossa. PRC offers the potential advantage of greater range of motion, technical ease, and decreased immobilization, and eliminates specific complic

Hamate fractures, which are classified as fractures of the body or the hook, are rare, representing only 2% to 4% of carpal fractures. Hamate body fractures can occur as isolated injuries, but in a high-energy trauma, associated carpal bone fractures or carpal disruption can be seen. Hamate fractures can be easily underdiagnosed. Pain in the ulnar side of the palm and swelling of the wrist after a wrist injury should alert the physician. Anteroposterior and lateral radiographs of the wrist may not always be adequate for the diagnosis. Computed tomography should be used to explain the pain in t

We present the case of a twenty year old man who developed static scapholunate instability on the right side following a road traffic accident seven months previously. A dynamic surgical technique was devised to treat this problem using the tendon of extensor brevi carpi radialis as an active transfer. The tendon, divided at its insertion on the base of the third metacarpal, was reinserted into the distal part of the scaphoid using two anchors after shortening it by removing the distal centimetre. The tendon was rerouted via a pulley created 'de novo' at Lister's tubercle in order to centre th

The purpose of this study was to determine if exercise alters wrist joint laxity, as measured by the mechanical behavior of the scaphoid bone. The load-displacement behavior of the scaphoid was studied in the palmar-dorsal direction in both wrists of 7 healthy volunteers (n = 14) before and after 2 exercise protocols (grip and push-up). When compared to the rested values, both exercise protocols significantly increased the displacement at 40 N by 47% (grip) and by 34% (push-up). Accordingly, the stiffness decreased significantly by 36% (grip) and by 32% (push-up). Partial recovery was document…

BACKGROUND: The standard diagnostic work-up for hand and wrist fractures consists of history taking, physical examination and imaging if needed, but the supporting evidence for this work-up is limited. The purpose of this study was to systematically examine the diagnostic accuracy of tests for hand and wrist fractures.
METHODS: A systematic search for relevant studies was performed. Methodological quality was assessed and sensitivity (Se), specificity (Sp), accuracy, positive predictive value (PPV) and negative predictive value (NPV) were extracted from the eligible studies.

INTRODUCTION: Non-union is a prevalent complication of scaphoid fractures. Late diagnosis is common and has a clinical impact due to functional limitations for the patient. Multiple treatments have been proposed to manage this complication, ranging from conservative (i.e., orthopedic) to surgical treatment. The vascularized medial femoral condyle technique has shown satisfactory clinical and paraclinical results, mainly in presence of avascular necrosis of the proximal pole but data regarding functional outcomes and patient satisfaction is scarce. This case series aims to describe the clinical

Background Pediatric carpal injuries are a clinical challenge due to their non-specific clinical features and occult nature on plain radiography. We hypothesized that early magnetic resonance imaging (MRI) will allow prompt diagnosis and treatment stratification, and that distal pole fracture of the scaphoid requires a shorter duration of immobilization. This study aims to assess the injury pattern and clinical outcomes of under-16-year-olds treated with acute post-traumatic wrist injuries in accordance with the unit's protocol. Methods All patients under the age of 16 years treated for susp

INTRODUCTION: To compare the cost-effectiveness and patient impact between acute magnetic resonance imaging (MRI) management and conventional management in the diagnosis of occult scaphoid fractures in a rural setting.
METHODS: Consecutive patients presenting to a rural emergency department (ED) with a suspected scaphoid fracture were randomly assigned to either conventional management (6) or acute MRI management (10) (3 patients were excluded from the study analysis). All healthcare costs were compared between the two management groups and potential impacts on the patients' pain, mobility an

BACKGROUND: The aim of this study was to analyze the long-term results of a modified Matti-Russe technique for the treatment of scaphoid non-union and pseudarthrosis. The modification was based on the use of bone graft taken from the ipsilateral distal radius, rather than from the iliac crest, as originally described.
MATERIAL/METHODS: Between 1987 and 2000, 23 consequent male patients with scaphoid waist non-union or pseudarthrosis underwent surgery by a modified Matti-Russe technique. During the 5-year follow-up, patient evaluation was based on radiological findings and the Green and O'Brie

BACKGROUND: Chronic dynamic scapholunate (SL) instability remains difficult to treat. Several treatment options exist.
PURPOSE: We propose an arthroscopic dorsal round-block capsulo-ligamentoplasty technique to treat dynamic scapholunate instability: a dorsal purse-string-like suture around the dorsal aspect of the proximal row, following the course of the dorsal intercarpal ligament (DICL). A Fiberwire suture travels between the radiocarpal and midcarpal space, piercing the attachments of the DICL on the triquetrum and of the scaphotrapezial ligament on the distal scaphoid. It reinforces the

BACKGROUND: Scaphoid fracture is the most commonly fractured carpal bone in the athletic patient, accounting for over 85% of all sport-related carpal bone fractures, and is particularly common in sports involving high impact injuries to the wrist. The management of such injuries comprises both conservative and surgical techniques, as guided by fracture location and type. Athletes demonstrate a unique challenge with regards to the management of scaphoid fractures due to their requirement to return to sport, as soon as able.
AIM: To review systemically all studies recording return to sport foll
