Transtemporal management of temporal bone encephaloceles and CSF leaks: review of 56 consecutive patients.

CONCLUSIONS: This report details our experience in the management of 56 consecutive patients with mastoid encephaloceles and cerebrospinal fluid (CSF) leaks. The majority were managed through the transmastoid route with temporalis fascia and calvarial bone graft. Among the patients operated on at least 18 months before analysis, there was a median follow-up of 54 months with no intracranial complications. The average body mass index (BMI) of patients with spontaneous CSF leaks was found to be statistically significantly greater than that of patients with CSF leaks with traumatic, infectious or

[Fibrous dysplasia of the scull discovered accidently on CT from different indication].

UNLABELLED: Fibrous dysplasia (dysplasia fibrosa -DF) is a local disorder of spongy bone structure caused by local pathological metabolism. Transformation of trabecular bony structure into connective tissue takes place. Inside the transformed bone scattered connective tissue elements, cartilaginous elements and pathological density are found. The disease proliferates either in the medullary bone or in subperiosteal region, and the development of fibrous tissue causes bone expansion with thinning of overlying cortex. The radiological appearance depends on the stage of the disease. The margins o

Tissue Nonspecific Alkaline Phosphatase (TNAP) Regulates Cranial Base Growth and Synchondrosis Maturation.

Hypophosphatasia is a rare heritable disorder caused by inactivating mutations in the gene (Alpl) that encodes tissue nonspecific alkaline phosphatase (TNAP). Hypophosphatasia with onset in infants and children can manifest as rickets. How TNAP deficiency leads to bone hypomineralization is well explained by TNAP's primary function of pyrophosphate hydrolysis when expressed in differentiated bone forming cells. How TNAP deficiency leads to abnormalities within endochondral growth plates is not yet known. Previous studies in hypophosphatemic mice showed that phosphate promotes chondrocyte matur

Computed Tomography Findings in Pathology-Confirmed Rhino-Orbital Mucormycosis During the COVID-19 Era: A Retrospective Cross-Sectional Study.

BACKGROUND AND AIMS: Rhino-orbital mucormycosis (ROM) emerged as a severe complication during the COVID-19 era, particularly in immunocompromised patients. Computed tomography (CT) plays a critical role in early detection and assessment of disease extent. This study aimed to describe the CT imaging features of pathology-confirmed ROM and to evaluate their associations with orbital involvement, particularly proptosis. METHODS: This retrospective, imaging-based study included patients with histopathologically confirmed ROM who underwent noncontrast paranasal sinus CT. CT scans were systematical

P14.05 How far can they grow... - Two clinical examples

Abstract Introduction: Cerebral metastases are the most common form of central nervous system (CNS) tumours in adults. However, malignant neoplasm may also involve structures external to the brain, such as tissue surrounding the base of the skull, and then metastasize to the brain either by direct invasion or by spreading by the cranial nerves. CASE1: A 74 year-old man, with a past history of chronic kidney disease due to renal artery thrombosis and hypertension, was admitted in the emergency room (ER) complaining of persisting pain in the superior half of the right hemiface and frontal region

Simple assessment of the localization of posterior communicating artery aneurysms to the anterior petroclinoid ligament.

The microsurgical management of posterior communicating artery (PCoA) aneurysms can be modified depending on their localization, with respect to the tentorial edge consisting of the anterior petroclinoid ligament (APCL). However, the imaging of APCL has been impractical to date. In this study, we evaluated a simple method for assessing the relationship between PCoA aneurysms and APCL, using three-dimensional computed tomographic angiography (3D-CTA). We retrospectively reviewed all surgically treated cases of PCoA aneurysms via the pterional approach in our institute from May 2013 to August 20

Spheno-occipital synchondrosis: Examining the degree of fusion in a South African Black skeletal sample.

Estimating age-at-death is one of the many biological demographics that a forensic anthropologist needs to determine for a set of unknown skeletal remains. A useful skeletal developmental marker, which can aid in estimating age in sub-adult remains, is the state of fusion of the spheno-occipital synchondrosis. This study aimed to determine the repeatability of a three-stage scoring method and the age at which the spheno-occipital synchondrosis begins and completes fusion in a Black South African sample. A total of 147 male and female skeletal individuals aged between 12-30 years from the Raymo

Bony Regeneration of the Sella after Transsphenoidal Pituitary Surgery.

OBJECTIVE: The purpose of this study is to demonstrate the possible bony regrowth of the sella after transsphenoidal surgery without any intraoperative sellar reconstruction. METHODS: Radiologic findings of the sella were reviewed in patients with pituitary tumors treated by transsphenoidal surgery. In 17 patients who had postoperative cranial computed tomography scans, bony regeneration of the sellar floor was evaluated by comparing immediate and late postoperative scans. The bony opening reduction was measured in transverse and sagittal planes. RESULTS: The median bony opening diameter in

Hypertelorism and micro-orbit simultaneous correction in a patient with paramedian bilateral facial clefts.

The aim of this paper is to report a case of a patient with paramedian bilateral facial clefts and hypertelorism associated with a severe encephalocele and micro-orbit. The patient required a facial bipartition to correct a transsphenoidal encephalocele, and a modified medialization surgery of the orbits to simultaneously expand the micro-orbit and correct the hypertelorism. These procedures achieved hypertelorbitism correction, orbital expansion which allowed symmetrical facial growth, and a functional orbit that permitted the use of an ocular prosthesis. We present this unique case to highli

Endoscopic study for the pterygopalatine fossa anatomy: via the middle nasal meatus-sphenopalatine foramen approach.

OBJECTIVE: The purposes of this study were to locate the constant anatomic landmarks, which are very important and helpful for endoscopic surgery and not well described for the pterygopalatine fossa (PPF) surgery via the middle nasal meatus-sphenopalatine foramen approach to establish a safe surgical mode. METHODS: Eight cases of adult skull specimens were selected for the simulated surgery. The Messerklinger surgical approach was used under the endoscope. The uncinate process was removed successively, and the anterior ethmoid sinus and posterior ethmoid sinus were opened. The opening of the

Endoscopic transsphenoidal surgery for isolated fibrous dysplasia of the clivus.

INTRODUCTION: Fibrous dysplasia is a non-neoplastic disorder of bone, related to abnormal proliferation of fibro-blasts. CASE REPORT: We report a 41-year-old female who presented with hyperprolactinaemia and an incidental clival lesion on MR scan. This was sub-totally resected via an endoscopic transsphenoidal approach and fibrous dysplasia was confirmed histologically. Her postoperative re-covery was un-eventful and at 8 months follow-up, she remains well, other than for the intermittent headaches and with no pituitary hormone deficiencies. CONCLUSION: The present case highlights the need t

Mystery of the Muenke midface: spheno-occipital synchondrosis fusion and craniofacial skeletal patterns.

PURPOSE: The spheno-occipital synchondrosis (SOS) is an important site of endochondral ossification in the cranial base that closes prematurely in Apert, Crouzon, and Pfeiffer syndromes, which contributes to varying degrees of midface hypoplasia. The facial dysmorphology of Muenke syndrome, in contrast, is less severe with low rates of midface hypoplasia. We thus evaluated the timing of SOS fusion and cephalometric landmarks in patients with Muenke syndrome compared to normal controls. METHODS: Patients with Muenke syndrome who had at least one fine-cut head computed tomography scan performed

Clinical and ophthalmological outcome of endoscopic transorbital surgery for cranioorbital tumors.

OBJECTIVE: Cranioorbital tumors are complex lesions that involve the deep orbit, floor of the frontal bone, and lesser and greater wing of the sphenoid bone. The purpose of this study was to describe the clinical and ophthalmological outcomes with an endoscopic transorbital approach (TOA) in the management of cranioorbital tumors involving the deep orbit and intracranial compartment. METHODS: The authors performed endoscopic TOAs via the superior eyelid crease incision in 18 patients (16 TOA alone and 2 TOA combined with a simultaneous endonasal endoscopic resection) with cranioorbital tumors…

Rare presentation of Ewing sarcoma metastasis to the sella and suprasellar cistern.

We present an exceedingly rare case of a Ewing sarcoma metastasis manifesting as a sellar mass mimicking a pituitary adenoma. The differential diagnosis of the young adult with a sellar mass is presented and correlated with a review of available literature, demonstrating this case's unique potential for clinical teaching. More specifically, this case illustrates that in a patient with a clinical history of Ewing sarcoma, a metastasis may involve the sella and suprasellar cistern without apparent osseous involvement.

[Isolated traumatic oculomotor nerve palsy caused by minor head trauma].

Isolated traumatic oculomotor nerve palsy caused by minor head trauma is extremely rare. We report a case of this condition. A 65-year-old woman was transferred to our hospital after minor head injury. Initial examination of her right eye revealed a dilated pupil. The result of computed tomography, magnetic resonance imaging, and magnetic resonance angiography of the brain were normal. An examination conducted 6 hours after the accident, examination demonstrated right ptosis, and another conducted 20 hours after the accident, she demonstrated partial limitation of adduction, supraduction, and

Clivus chordoma in a 9-year-old child: case report and review of the literature.

Chordomas are bone tumors of the axial skeleton. They arise from notochordal remnants. In children these tumors are extremely rare and are predominantly located in the skull base. The authors report on a clivus chordoma in a 9 7/12-year-old girl. It presented as a nasopharyngeal mass with destruction of the clivus and paralyses of the ninth, tenth, and eleventh cranial nerves on the right side. After incomplete resection by a transoral transclival route, high-dose radiotherapy was added. This treatment was effective as demonstrated by follow-up CAT scans. A short review of the current literatu
