Endoscopic retrograde cholangiopancreatography in ruptured liver hydatid cyst.

One of the most common and serious complications of hepatic hydatid cyst disease is communication between the cyst and the biliary tree. Sur … ory, imagery, and ERCP findings for all patients. The therapeutic methods performed were endoscopic sphincterotomy, extraction by balloon or Dormia basket, stenting, or nasobiliary drainage. Sixteen patients with ruptured hepatic hydatid cyst into bile ducts were seen in 9 years. Nine of 16 patients had a surgical history of hepatic hydatid cyst and three patien …

Timing of cholecystectomy for biliary pancreatitis: do the data support current guidelines?

BACKGROUND: Current guidelines suggest that cholecystectomy be performed within 2 weeks after discharge following an episode of biliary panc … creatitis) after discharge. Recurrences (31.3%) occurred within 2 weeks after discharge. Endoscopic sphincterotomy protected against preoperative recurrent pancreatitis but was associated with a higher incidence of other gallstone-related events. Median total length of hospital stay was greater for group B than for group A [7 (range, 2-37) days vs. 5 (1-45) days …

Post-cholecystectomy syndrome and sphincter of Oddi dysfunction: past, present and future.

Post-cholecystectomy syndrome and the concept of a causal relationship to sphincter of Oddi dysfunction, despite the controversy, has presen … ed as follows. Patients with sphincter of Oddi stenosis will most often benefit from treatment with sphincterotomy. Patients with classic biliary pain and some objective evidence of biliary obstruction may have a sphincter of Oddi disorder and should be considered for endoscopic evaluation and therapy. Patients with atypical post-cholecystectomy pain, without an …

Performance of different approaches to difficult biliary cannulation in endoscopic retrograde cholangiopancreatography: A systematic review and network meta-analysis.

BACKGROUND: It is unclear which is the best strategy for difficult biliary cannulation in endoscopic retrograde cholangiopancreatography (ERCP). AIMS: We compared the different techniques, through a network meta-analysis combining direct and indirect comparisons. METHODS: We identified 12 randomized controlled trials (1605 patients) comparing different techniques for difficult biliary cannulation (early and late needle knife techniques, pancreatic guidewire- and stent-assisted techniques, transpancreatic sphincterotomy, and endoscopic ultrasound rendez-vous [EUS-RV]) either with each other o…

Chronic pancreatitis.

PURPOSE OF REVIEW: We review selected important clinical observations reported in 2012. RECENT FINDINGS: Celiac disease is a risk factor for pancreatitis. Patients with recurrent acute pancreatitis likely have chronic pancreatitis, do not benefit from pancreatic sphincterotomy, and may not benefit from biliary sphincterotomy. Analysis of endoscopic ultrasonography (EUS) images with an artificial neural network (ANN) program may improve chronic pancreatitis diagnosis compared with clinical interpretation of images. In a multicenter, randomized controlled trial of chronic pancreatitis patients,…

Should we care about the internal anal sphincter?

The internal anal sphincter is currently regarded as a significant contributor to continence function. Four physiological and morphological aspects of the internal anal sphincter are presented as part of the current evidence base for its preservation in anal surgery. 1) The incidence of continence disturbance following deliberate internal anal sphincterotomy is underestimated, although there is presently no prospective imaging or physiologic data supporting the selective use of sphincter-sparing surgical alternatives. 2) Given that the resting pressure is a measure of internal anal sphincter f…

Endoscopic hemostasis with fibrin glue for refractory postsphincterotomy and postpapillectomy bleeding.

BACKGROUND: Bleeding is a feared complication of endoscopic sphincterotomy and papillectomy. Fibrin glue has been proposed as an effective adjunct in securing hemostasis. However, its use has been limited by the risk of early occlusion of the injecting needle, and its role has not been defined in the setting of refractory post-ERCP bleeding. We present a modified technique of endoscopic hemostasis with diluted fibrin glue in the setting of postsphincterotomy and postpapillectomy bleeds. OBJECTIVE: We aimed to verify that diluted fibrin glue can be easily and successfully injected and is effec…

Pressor response during cystomanometry in spinal injury patients complicated with detrusor-sphincter dyssynergia.

Herein attention is focused on the documentation of high blood pressure response with detrusor-sphincter dyssynergia in spinal cord injury patients and with its amelioration after extended sphincterotomy. During cystomanometric evaluation of the bladder the monitored blood pressure response in 53 spinal injury patients, 27 non-dyssynergic spinal injury patients and 18 non-spinal injury patients was compared. A high correlation was observed among the magnitudes of blood pressure increase, level of injury and the severity of detrusor-sphincter dyssynergia in spinal injury patients. During cystom…

Acute recurrent pancreatitis secondary to the rare association of a duodenal duplication cyst and a pancreas divisum.

Duodenal duplication cysts are rare congenital anomalies that can cause acute pancreatitis. Pancreas divisum is also a congenital anomaly, often discovered incidentally, but is considered a possible cause of acute pancreatitis. We report the case of the combination of both anomalies causing recurrent episodes of acute pancreatitis in a young man. Endoscopic treatment by partial excision of the cyst with a polypectomy snare and sphincterotomy of the minor papilla was successful.

[Results of endoscopic treatment of choledochal calculosis and cicatricial stenosis of the papilla].

The results obtained with endoscopic papillo-sphincterotomy in the treatment of gallstones and cicatricial stenosis of the papilla are reported. The treatment proved effective in 91.3% of patients with papillary stenosis and the incidence of side-effects (cholangitis, pancreatitis, bleeding, perforation) was 7.5%. Endoscopic PST hitherto restricted to cases judge inoperable due to serious concomitant pathologies may be a valid alternative to surgery wherever total sphincter dysfunction can be ascertained.

[Transurethral resection of bladder neck in female patients with neurogenic bladder].

Transurethral resection of the bladder neck was performed in 6 female patients with neurogenic bladders who presented with a large volume of residual urine and bladder neck obstruction on voiding cystourethrography without detrusor hyperreflexia on cystometry. In 4 of the patients, transurethral sphincterotomy was performed concomitantly. Five of the patients demonstrated significant improvement in bladder emptying and 4 did not need self-catheterization. No complications of the operation were observed.

[Infundibulotomy of the Vater's ampulla at impacted stone in the papilla].

Infundibullotomy or the method of endoscopic choledochoduodenostomy is an alternative to surgical bilio-digestive anastomosis for common bile duct obstructions due to impacted stone in the ampulla de Vater when occasionally standard endoscopic sphincterotomy was unsuccessful. In addition the endoscopic choledochoduodenostomy is a method described as final palliative treatment in elderly patients and/or those as high operative risk with malignant obstructions in the distal part of the common bile duct.
