One-year open-label trial of thalidomide in ankylosing spondylitis.

OBJECTIVES: To test in a large open study whether thalidomide is potentially useful in treating ankylosing spondylitis, and to see if thalidomide induces any change in expression of genes in peripheral blood mononuclear cells (PBMC). METHODS: Thirty male patients with treatment-refractory ankylosing spondylitis were recruited into a 12-month open study using thalidomide at a dosage of 200 mg/day. Seven indices were measured as primary outcome measures, and 6 other indices as secondary outcome measures. Transcripts in the PBMC of some of these patients were first screened with microarray, and …

[Spontaneous vertebral pseudarthrosis in ankylosing spondylitis. Apropos of 3 cases].

PURPOSE OF THE STUDY: The authors describe the surgical treatment of three cases of vertebral pseudarthrosis in two patients suffering from rheumatoid spondylitis for an average of 20 years. MATERIAL: Two pseudarthrosis involved the thoracic level (T9-T10 and T10-T11), after an insignificant trauma. In the other case, the lesion was a secondary localization (L5-S1) arising spontaneously two years after the treatment of a T10-T11 lesion. Initial conservative treatment was unsuccessful in all three cases. Anterior surgery was performed in all three cases. In two out of the three, posterior surg…

Thoracolumbar rotation in ankylosing spondylitis. A new noninvasive measurement method.

Thoracolumbar rotation was measured by a noninvasive method in 135 patients with ankylosing spondylitis. The mean of the total rotation from the level of the xiphisternum to S1 was 45.2 degrees, which is approximately half of that in healthy subjects measured by comparable methods. Rotation had a significant negative correlation with the duration of the disease and with the radiologic degree of lumbar spine involvement, but not with the degree of radiologic sacroiliitis. Reproducibility of the method was good. Measurement of thoracolumbar rotation is seldom used in the assessment of ankylosing…

[Tuberculous spondylitis with syndesmophytes and paraspinal ossification. Two case reports].

The authors described 2 patients with spinal tuberculosis. The first one was presented with multiple anterior marginal involvement of vertebral bodies, centrosomatic spondylitis of L3, associated with a syndesmophytic showing spinal ossification. In the second case, spinal tuberculosis involved the vertebral arc of L2 and L4. A paravertebral ossification on both sides of L4 was seen. The nature of these vertebral and paravertebral ossifications was discussed.

HLA-D locus typing in ankylosing spondylitis and Reiter's syndrome.

HLA-D typing of 44 patients with ankylosing spondylitis (AS) and 31 patients with Reiter's syndrome (RS) did not show increased frequency of any particular Dw allele in either population of patients as compared to controls. Such studies also allowed each patient's general response to be compared with other general responses within each experiment. Contrary to reports of diminished lymphocyte responses in AS patients, hyperresponsiveness in both AS and RS patients was found.

Neutrophil chemotaxis in ankylosing spondylitis, Reiter's disease, and polymyalgia rheumatica.

Neutrophil chemotaxis was found to be normal in 14 patients with ankylosing spondylitis, in 10 patients with Reiter's disease, and in 8 patients with polymyalgia rheumatica.

Psoriatic spondylitis. Association with advanced nongranulomatous upper lobe pulmonary fibrosis.

We describe the case of a patient with psoriasis, peripheral and axial arthropathy, and upper lobe fibrosis. This association of findings has not, to the best of our knowledge, been described in the past. Pulmonary fibrosis is not found to be more common in people with psoriasis than would be expected in a control Veterans Administration population and, while this association may be coincidental, pulmonary fibrosis in psoriatic spondylitis should be searched for in other patients.

Reliability of the Bath Ankylosing Spondylitis Disease Activity Index and the Bath Ankylosing Spondylitis Functional Index administered via tele-assessment in patients with axial spondyloarthritis.

INTRODUCTION: The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Bath Ankylosing Spondylitis Functional Index (BASFI) are widely used to assess disease activity and function in patients with axial spondyloarthritis (axSpA). However, their reliability when administered via tele-assessment has not been investigated. This study aimed to evaluate the reliability of BASDAI and BASFI administered via tele-assessment in axSpA patients. METHODS: This study included patients with axSpA. BASDAI and BASFI were administered by phone at two separate times, 24 hours apart (test-retest). In…

Harmonization, data management, and statistical issues related to prospective multicenter studies in Ankylosing spondylitis (AS): Experience from the Prospective Study Of Ankylosing Spondylitis (PSOAS) cohort.

Ankylosing spondylitis (AS) is characterized by inflammation of the spine and sacroiliac joints causing pain and stiffness and, in some patients, ultimately new bone formation, and progressive joint ankyloses. The classical definition of AS is based on the modified New York (mNY) criteria. Limited data have been reported regarding data quality assurance procedure for multicenter or multisite prospective cohort of patients with AS. Since 2002, 1272 qualified AS patients have been enrolled from five sites (4 US sites and 1 Australian site) in the Prospective Study Of Ankylosing Spondylitis (PSOA…

Juvenile onset ankylosing spondylitis (JAS) has less severe spinal disease course than adult onset ankylosing spondylitis (AAS): clinical comparison between JAS and AAS in Korea.

OBJECTIVE: To assess the frequency of juvenile onset ankylosing spondylitis (JAS) in Korean patients with AS and to differentiate the clinical characteristics of JAS from adult onset ankylosing spondylitis (AAS). METHODS: We studied 98 consecutive patients with AS who visited the rheumatology clinic of a tertiary referral center and compared clinical and radiographic features of JAS (n = 41) with those of AAS (n = 57). RESULTS: Median age at onset in JAS was 14 years (range 7-16) and in AAS 22 years (range 17-38) (p < 0.01). Patients with JAS at presentation showed fewer spinal symptoms and …

2019 Update of the American College of Rheumatology/Spondylitis Association of America/Spondyloarthritis Research and Treatment Network Recommendations for the Treatment of Ankylosing Spondylitis and Nonradiographic Axial Spondyloarthritis.

OBJECTIVE: To update evidence-based recommendations for the treatment of patients with ankylosing spondylitis (AS) and nonradiographic axial spondyloarthritis (SpA). METHODS: We conducted updated systematic literature reviews for 20 clinical questions on pharmacologic treatment addressed in the 2015 guidelines, and for 26 new questions on pharmacologic treatment, treat-to-target strategy, and use of imaging. New questions addressed the use of secukinumab, ixekizumab, tofacitinib, tumor necrosis factor inhibitor (TNFi) biosimilars, and biologic tapering/discontinuation, among others. We used t…

Incorporating assessment of the cervical facet joints in the modified Stoke ankylosing spondylitis spine score is of additional value in the evaluation of spinal radiographic outcome in ankylosing spondylitis.

BACKGROUND: To aim was to investigate the additional value of incorporating the de Vlam cervical facet joint score in the modified ankylosing spondylitis (AS) spine score (mSASSS) for the evaluation of spinal radiographic outcome in AS. METHOD: Baseline and 4-year radiographs from 98 consecutive patients from the Groningen Leeuwarden AS (GLAS) cohort, who had AS treated with TNF-α inhibitors, were scored by two readers; the vertebral bodies were assessed according to the mSASSS (0-72) and cervical facet joints (C2-C7) were assessed according to the method of de Vlam (0-15). The combined AS sp…
