Identifying clinico-radiological determinants of post-stroke fatigue 3 months post-stroke in a French hospital-based cohort of non-severe stroke patients without psychiatric comorbidities.

Post-stroke fatigue (PSF) is an overlooked and debilitating condition. As a multidimensional construct, fatigue encompasses physical, cognitive, and emotional components, complicating efforts to understand PSF pathophysiological mechanisms and identify key predictors. We aimed to investigate the impact of lesion characteristics on different facets of subacute PSF while accounting for socio-demographic, psychological, and neurological factors. We assessed 231 patients with first-ever mild ischemic stroke without recent anxiety or depressive disorders using the Multidimensional Fatigue Inventory…

Evidence-based stroke r-ehabilitation: an expanded guidance document from the european stroke organisation (ESO) guidelines for management of ischaemic stroke and transient ischaemic attack 2008.

OBJECTIVE: Despite recent high-profile advances in our understanding of rehabilitation post-stroke, the evidence base remains weaker than in other areas of stroke management. Under the aegis of the European Stroke Organisation a select committee was assembled to collate and appraise the evidence base for rehabilitation interventions. METHODS: Following systematic literature searching, relevant abstracts were screened for data quality and relevance. These data were summarized and presented to the members of the expert panel, who, both individually and across group discussions, modified the con…

[Artificial intelligence in the management of stroke].

The rise of artificial intelligence (AI) has generated considerable expectations for improving stroke management. In this work, we present current AI applications for stroke prevention, triage, and treatment selection during the acute phase. We then discuss the potential value of AI in predicting neurological recovery. Currently, only the detection of atrial fibrillation and the identification of large-vessel occlusions on brain imaging have been validated. Nevertheless, recent advances are opening up new perspectives for using AI algorithms in stroke care.

Peroxiredoxins in Stroke: Friends and Foes.

Stroke, a leading cause of mortality and long-term disability, induces complex cascades of oxidative stress and neuroinflammation that exacerbate brain injury. The peroxiredoxin (Prx; Prdx) family, composed of six thiol-dependent antioxidant enzymes (Prx1-6), plays a pivotal role in regulating redox homeostasis and immune responses in the brain. This review synthesizes current knowledge on the isoform-specific roles of Prxs in both ischemic and hemorrhagic stroke, highlighting their dual and context-dependent functions. Intracellular Prxs generally protect neurons and maintain blood-brain barr…

The dilemma of thromboprophylaxis in stroke patients.

Acute stroke is one of the leading causes of mortality worldwide, with venous thromboembolism (VTE) representing a significant yet under-recognised complication. The risk of VTE is highest in the early post-stroke period, driven by factors encapsulated by Virchow's triad. However, the risk of haemorrhagic transformation following ischaemic stroke, and haematoma development and expansion after haemorrhagic stroke complicate the routine use of pharmacological VTE prophylaxis. In this review, we discuss the epidemiology and aetiology of VTE post-stroke, haemorrhagic transformation after acute isc…

Corin in cardiovascular diseases and stroke.

Corin is a type II transmembrane serine protease highly expressed in the heart. It plays a critical role in regulating fluid balance and improving cardiac function by converting pro-atrial natriuretic peptide into mature atrial natriuretic peptide. CORIN variants have been identified in patients with hypertension, heart failure, atrial fibrillation, and stroke. In vivo and in vitro, corin deficiency increases blood pressure and impairs cardiac function. Circulating soluble corin appears to have potential as a stable and specific biomarker for the risk prediction and prognostic assessment of ca…

Resistance training for people with stroke.

RATIONALE: Levels of physical activity and physical fitness are low after stroke. Low muscle strength is common, particularly on the affected side, and is associated with post-stroke disability. Resistance training exercise interventions could increase muscle strength, improve physical function and reduce disability, and may benefit secondary prevention. OBJECTIVES: The primary objective of this review is to determine whether resistance training after stroke has any effects on death, disability, adverse events, risk factors, fitness, walking, and indices of physical function when compared to …

The role of myeloperoxidase in the pathogenesis of stroke.

Stroke is the leading cause of mortality and morbidity worldwide, significantly impacting human welfare and overall health. Myeloperoxidase (MPO), a heme peroxidase secreted by neutrophils, plays a crucial role in the body's defense mechanisms, exhibiting pro-inflammatory and pro-oxidative properties. Additionally, MPO compromises the structural integrity and functional capacity of blood vessels, potentially leading to the formation and dislodgement of atherosclerotic plaques, vascular stenosis, thrombosis, and ultimately contributing to stroke occurrence. Following a stroke, a significant inf…

Stroke remains a leading cause of disability worldwide, impacting millions of survivors with long-term neurological and functional impairments. Advances in acute stroke treatment have improved survival rates, but post-stroke disability (PSD) continues to pose significant challenges. This review provides a comprehensive overview of the common types of PSD, including motor dysfunction and spasticity, cognitive impairment, dysphagia, depression, epilepsy, aphasia, pain, and fatigue. Each of these conditions affects patients' independence, quality of life, and long-term prognosis. Current manageme…

[Stroke: current state of the problem].

We review the current and emerging topics in ischemic stroke (IS) and intracerebral hemorrhage (ICH). We discuss the association of genetic predisposition and healthy lifestyle, ambient particulate air pollution, weather parameters, variations in the anatomy of cerebral blood vessels, psychological stress, depression, insonmia with the development of IS or ICH. Also, the role of oral anticoagulants (AC) as a new risk factor for ICH is presented. The issues of pathophysiology of IS and ICH are considered, in particular, the discrepancy between blood flow and metabolism and penumbra stability in…

A stroke patient with an unusual electrocardiogram.

We present a case of a 70 year old man with a history of paroxysmal atrial flutter who was admitted to the hospital with symptoms and imaging consistent with an acute stroke. Physical exam was notable for a pulse rate of 50 beats per minute and right sided facial droop with mild dysarthria. Admission ECG shows a junctional bradycardia with evidence of dual AV node physiology, rarely manifested in a retrograde fashion on a standard ECG. The patient likely experienced parasympathetic sinus node slowing in the setting of acute stroke. During post-stroke monitoring, the patient demonstrated return…

Diabetes and Stroke: What Are the Connections?

Stroke is a major cause of death and long-term disability worldwide. Diabetes is associated with an increased risk of cardiovascular complications, including stroke. People with diabetes have a 1.5-2 times higher risk of stroke compared with people without diabetes, with risk increasing with diabetes duration. These risks may also differ according to sex, with a greater risk observed among women versus men. Several mechanisms associated with diabetes lead to stroke, including large artery atherosclerosis, cerebral small vessel disease, and cardiac embolism. Hyperglycemia confers increased risk…

Antithrombotic Therapy in the Prevention of Stroke.

OVERVIEW: Ischemic stroke is a leading cause of death and disability throughout the world. Antithrombotic therapy, which includes both antiplatelet and anticoagulant agents, is a primary medication of choice for the secondary prevention of stroke. However, the choices vary with the need to incorporate evolving, newer information into the clinical scenario. There is also the need to factor in co-morbid medical conditions as well as the cost ramifications for a particular patient as well as compliance with the regimen. Pertinent Updates: In the acute setting, dual antiplatelet therapy from three…

Development and Testing of Thrombolytics in Stroke.

Despite recent advances in recanalization therapy, mechanical thrombectomy will never be a treatment for every ischemic stroke because access to mechanical thrombectomy is still limited in many countries. Moreover, many ischemic strokes are caused by occlusion of cerebral arteries that cannot be reached by intra-arterial catheters. Reperfusion using thrombolytic agents will therefore remain an important therapy for hyperacute ischemic stroke. However, thrombolytic drugs have shown limited efficacy and notable hemorrhagic complication rates, leaving room for improvement. A comprehensive underst…

Narrative review of sleep and stroke.

Sleep disorders, such as sleep-disordered breathing (SDB), insomnia or restless legs syndrome (RLS), are common in the general population and after stroke. In some cases, sleep disturbances are pre-existing, but can also appear de novo as a direct consequence of brain damage or due to stroke-related complications. Furthermore, some sleep conditions may act as a risk factor of stroke. This review explores the available evidence of the two-way relationship between sleep and stroke. Cardiovascular physiological changes during sleep are described, as well as the evidence on the relationship betwee…
