Reevaluating the brain disease model of addiction.

The brain disease model of addiction has dominated public and scientific discourse on addiction (termed substance use disorder [SUD] in the DSM-5) over the past 3 decades. The model framed addiction as a chronic and relapsing brain disease caused by structural and functional brain alterations. The purpose of this model was purportedly dual, as both an aetiological theory and a tool to reduce stigma. Weak empirical support and concerns about the model downplaying fundamental psychosocial causes of SUDs have led to stark disagreement as to whether addiction should be conceptualised as a brain di

Interoceptive Processing in Substance Use Disorders (SUDs): From the Neuroanatomy to Insights from Computational Models and Predictive Coding Frameworks.

Substance use disorders (SUDs) represent complex public health challenges characterized by a blend of genetic, cognitive, environmental, and psychosocial factors. This chapter explores the critical role of interoceptive processing - the internal sensing of physiological states - in the neurobiology and treatment of SUDs. Interoceptive dysfunctions are highlighted as central to the craving, emotional regulation, and decision-making processes that underpin addictive behaviors. The importance of the insula in sustaining drug use, particularly nicotine, underscores a broader involvement of interoc

Datura stramonium - A Dangerous Weed and Alternative Drug of Abuse: An Overview of Poisoning Cases in 21st Century.

Datura stramonium is a well-known cosmopolitan weed known by several common names: thorn apple (due to the appearance of its fruits), Angel's trumpet (linked to its flowers), loco seed (referring to the hallucinogenic properties of its seeds), jimson weed (originating from the first recorded poisoning involving this plant), and apple of Peru (indicating the plant's origin). All parts of the plant contain the tropane alkaloids hyoscyamine, scopolamine, and atropine together with several minor alkaloids, which cause poisoning such as typical anticholinergic syndrome with neuropsychiatric effects

Neuroglia in substance use disorders.

Substance use disorders (SUD) remain a major public health concern in which individuals are unable to control their use of substances despite significant harm and negative consequences. Drugs of abuse dysregulate major brain and behavioral functions. Glial cells, primarily microglia and astrocytes, play a crucial role in these drug-induced molecular and behavioral changes. This review explores preclinical and clinical studies of how neuroglia and their associated neuroinflammatory responses contribute to SUD and reward-related properties. We evaluate preclinical and clinical evidence for targe

Illicit Drug Induced Hyperthermia (IDIH).

Illicit drug induced hyperthermia (IDIH) is a life-threatening emergency requiring timely recognition and management. The pathophysiology of hyperthermia secondary to illicit drug toxicity can be associated with and complicated by additional precipitants such as exertion and high ambient temperature (> 31°C). We present cases highlighting the clinical features, morbidity and mortality associated with IDIH. The priorities of management include interventions to decrease ongoing heat production, active cooling and targeted supportive care. Temperature control should ideally be achieved within 15

Current status of pheniramine misuse among substance use disorder patients: A narrative review of extent, patterns, and clinical implications.

BackgroundThere is growing concern about the misuse of Over-the-counter (OTC) medications around the world. Pheniramine is a first-generation antihistamine, which is widely available and is commonly used for allergic conditions. Initially considered to be a safe anti-allergic agent, growing evidence suggests its potential for misuse and dependence. Notably, it is reported that pheniramine is being used to mix other substances such as opioids during injection drug use. However, pheniramine misuse is not adequately studied in existing literature.ObjectiveThis review aims to synthesize the litera

Association between nucleus accumbens volume and substance use disorder: a narrative review.

INTRODUCTION: The nucleus accumbens (NAc) is central to the brain's reward circuitry, mediating motivation and emotional processes. Emerging evidence suggests that structural and functional changes in the NAc, including volume alterations, may contribute to the neuropathology of substance use disorder (SUD). This review evaluates current findings on the association between NAc volumetric changes depicted by magnetic resonance imaging (MRI) and SUD. METHODS: A literature search was conducted in PubMed, EMBASE, BVS, Web of Science, Scopus, Cochrane Library, and PsycInfo. Terms used in searches

Assessment of Abuse Potential of Three Indazole-Carboxamide Synthetic Cannabinoids 5F-ADB, MDMB-4en-PINACA and ADB-4en-PINACA.

5F-ADB, MDMB-4en-PINACA and ADB-4en-PINACA are three potent indazole-carboxamide synthetic cannabinoids (SCs) that have been widely abused in recent years. However, the pharmacological research on these compounds remains limited, especially in vivo research data. The purpose of the present study was to investigate the pharmacological effects of 5F-ADB, MDMB-4en-PINACA and ADB-4en-PINACA in mice, comparing their in vivo effects with those caused by Δ9-tetrahydrocannabinol (Δ9-THC), the main psychoactive substance in cannabis. We evaluated the cannabinoid-specific pharmacological effects of 5F-A

Deaths related to the use of diarylethylamines, with a focus on the United Kingdom: A systematic review and case series report.

BACKGROUND: Diarylethylamine drugs possess dissociative properties. These emerged as drugs of misuse, with reports of strong addictive potential, high tolerance and compulsive intake. AIMS: Since one of these drugs, diphenidine, was added to the 1971 Convention on Psychotropic Substances, the United Kingdom (UK) had to consider its control. The Advisory Council on the Misuse of Drugs 2023 advice included toxicity and mortality involving this and related molecules. Relevant mortality data were collated to understand the international and UK situations. METHODS: A systematic review was employe

Problematic diagnosis of substance-induced disorders in ICD-11.

The ICD-11 was introduced in January 2022. In chapter 6, "Mental, behavioral and neurodevelopmental disorders" we find the section "Disorders due to substance use and addictive behaviors" (section 6C4). Changes from the ICD-10 in this section include broadening the categories of harmful use and dependence, including more types of substances, and the addition of more behavioral addictions (gaming disorder). These changes have been discussed and debated [1].

A Clinical Guide to Support the Implementation of Addiction Consult Services and the Value of Teaching and Technical Assistance.

People with substance use disorders (SUDs) are increasingly admitted to general hospitals; however, many hospital systems lack both formal structures and skilled staff to provide high-quality care for inpatients with SUDs. Inpatient addiction consult services (ACSs), which are increasingly being implemented around the country, are an evidence-based strategy to add focused care for people with SUDs into the general medical setting. In 2018, New York City Health + Hospitals (H + H) launched an ACS program called Consult for Addiction Care and Treatment in Hospitals in six hospitals, supported by
