Influence of dose on suxamethonium-induced muscle damage.

We have examined postoperative muscle pain and early increases in serum concentrations of myoglobin after administration of suxamethonium to see if these changes were dependent on the dose of drug. Thirty ASA I and II adult patients undergoing day-case surgery received a standard anaesthetic technique, including one of three doses of suxamethonium: 0.5, 1.5 or 3.0 mg kg-1. The incidence of postoperative myalgia and the severity of fasciculations were greater after suxamethonium 1.5 mg kg-1 than after a dose of 0.5 or 3.0 mg kg-1. Serum concentrations of myoglobin increased in a dose-dependent

Minor complications of general anesthesia in a series of 1220 patients: the influence of mode of ventilation.

Postoperative complications due to anesthetic technique were assessed in 1220 patients undergoing abdominal, neurosurgical, breast, head and neck operations or other surgical procedures. All patients were interviewed directly for vomiting, sore throat, headache, suxamethonium myalgias, ocular, vascular, nerve complications and awareness, 24 and 48 hours postoperatively. The incidence of vomiting was significantly higher in abdominal (27.2%) versus the extra-abdominal operations (16.2%), (p less than 0.005), overall incidence 17.6%. Spontaneous breathing was associated with lower incidence of v

[Screening tests for malignant hyperthermia susceptibility].

The ideal screening test for malignant hyperthermia susceptibility (MHS) has yet to be discovered. It should be simple noninvasive, yet totally specific and sensitive. Until such an ideal test becomes available, allowing simple routine preoperative screening, tests should only be used in certain specific situations. These include: patients in whom a clinical crisis was suspected; the members of the family of a subject labeled MHS because of a fatal, or otherwise, crisis, or in whom tests were positive; patients with other pathological conditions which could be linked to malignant hyperthermia

Adverse reactions to drugs and metabolic problems perceived in Northern Canadian Indians and Eskimos.

Eskimos and to a slightly lesser degree Northern Indians are extremely rapid acetylators as tested with isoniazid. They also tend to clear phenytoin rapidly as proven in Greenland Eskimos and supported by clinical observations in Canadian Eskimos. Most "silent gene" cholinesterase cases found in Canada came from the tiny minorities of Northern Indians and Eskimos and an even higher prevalence was found in an isolate of South Western Alaskan Eskimos. We found alcohol metabolism significantly slower in Northern Indians and Eskimos than Caucasians, which is at variance with findings in other Amer

[Anaphylaxis to muscle relaxants. Predictive value of intradermal tests and study of crossed anaphylaxis].

37 patients were studied, all of whom presented with anaphylaxis to a muscle relaxant. The diagnosis was made after simultaneous intradermal testing (IDT), human basophil degranulation tests (HBDT) and Prausnitz-Küstner tests (PK) of passive cutaneous anaphylaxis. Three tests were positive in 6 patients, both IDT and PK in 9, and both IDT and HBDT in 8. In 14 patients, the IDT, repeated twice, were positive both times. A search for crossed anaphylaxis to the other muscle relaxants was carried out in all the patients during a second series of tests, a few months to years after the first one. Th

Suxamethonium: a new look at pretreatment.

Fifty patients were pretreated with either suxamethonium 10 mg or gallamine 20 mg before an injection of suxamethonium 1 mg kg-1. The effect of the pretreatment upon neuromuscular transmission was monitored by recording the mechanical response of the adductor pollicis muscle to indirect stimulation, using a 2-Hz train-of-four stimulus repeated every 12 s. The pretreatment injection of suxamethonium caused fasciculation in one-quarter of the patients studied. Significant neuromuscular blockade occurred during pretreatment in 16 of the 25 patients investigated. Pretreatment with gallamine 20 mg

Malignant hyperthermia.

The true incidence of malignant hyperthermia is unknown, but the frequency has been estimated as high as 1/14,000 anesthetic events. Review of the literature reports mortality rates up to 70%. Without prompt medical intervention, it is a uniformly fatal disease. Thus, it behooves the physician to have an awareness of the syndrome and its features, so that early recognition and adequate treatment take place. This paper presents a review of the literature on the occurrence, pathology, symptoms and treatment of malignant hyperthermia.

General anesthesia in "inducible" porphyrias.

To evaluate the risk of inducing acute symptoms after general anesthesia in patients with "inducible " porphyrias, the authors analyzed retrospectively the effects of 78 exposure to anesthesia in 47 patients, 33 with acute intermittent porphyria and 14 with variegate porphyria. On 62 occasions, 29 involving the use of a barbiturate, anesthesia was induced in 37 patients who had no porphyric symptoms at the time. None of these patients had an acute attack postoperatively. Anesthesia was induced 16 times in 14 patients during acute episodes; 12 of these patients also received precipitating drugs

Dantrolene and suxamethonium. The effect of pre-operative dantrolene on the action of suxamethonium.

A single oral dose of dantrolene (100--150 mg) given at least 2 hr pre-operatively has been found to reduce significantly the strength of muscular fasciculations, the hyperkalaemia and the incidence of muscle pains following suxamethonium (from 56 to 4%) in a series of forty-eight patients compared with controls. The biceps EMG of the fasciculations was unchanged by dantrolene; the incidence of troublesome side effects was low (9%) and there did not appear to be any alteration in the duration of action of suxamethonium.

Chemical and physical restraint of nonhuman primates.

Numerous publications on primate restraint and anesthesiology have appeared in recent years. This reflects the striking growth of interest in medical primatology and of efforts to improve restraint agents and methods to facilitate humane use of primates in research. For access to earlier literature, readers should consult recent textbooks [12; 29, pp. 469--474], reviews on chemical or physical restraint [3, 4, 16, 27, 30, 37], and other valuable publications on these subjects that have appeared since 1965 [1, 20, 23, 36, 40, 42]. In this brief review we consider publications, principally since

Anaesthesia for electroconvulsive therapy.

The results of a prospective study of 3,500 Anaesthetics for electroconvulsive therapy is presented. Minimal differences were observed between thiopentone and methohexitone. Propanidid and Diazepam were found to be unsuitable induction agents. Other findings included minimal serum potassium elevation and a low incidence of post treatment muscle pains. The place of the single-handed operator and the place of unmodified electroconvulsive therapy is questioned as being unacceptable practice.

Allergic reactions to anaesthetics in surgery: current challenges and perspectives.

INTRODUCTION: This study was conducted to assess the current state of issues related to AR to anaesthetics used in surgical procedures. CONTENTS: A literature review of scientific publications in allergology and anaesthesiology was performed, evaluating the allergenic profile of modern anaesthetics and analysing approaches to preventing AR during surgery. SUMMARY: Currently, the most commonly used local anaesthetics are amides, including lidocaine, bupivacaine, articaine, mepivacaine, ropivacaine, and levobupivacaine. These are considered safer in terms of allergenicity compared to their est

Association of malignant hyperthermia and exertional heat illness in young athletes: An analysis of awareness among clinical and athletic first responders.

BACKGROUND: Malignant hyperthermia (MH), a rare inherited condition seen almost exclusively in the perioperative setting, is triggered by volatile anesthetics or an intravenous paralytic drug, succinylcholine. It can, however, occur without any exposure to anesthetic drugs, being associated with heat illness and rhabdomyolysis, thus presenting a little-known risk to young athletes exercising in hot environments. OBJECTIVE: This study aimed to determine the first responder awareness of MH and its association with heat illness in young athletes within athletic and clinical environments. METHOD
