Mangling injuries of the upper extremity are severe, high-energy traumas that damage multiple functional systems including skin, nerves, vascular, tendon, and bone. Management requires familiarity with various techniques in orthopedic, vascular, and plastic surgery. A specific technique cannot be described due to the various combinations of injuries that can occur with mangled upper extremities, but we present principles and recommendations for treatment and judgment.

PURPOSE: The rectus femoris (RF) forms the anterior portion of the quadriceps muscle group. It has a proximal tendinous complex (PTC) which is constituted by a direct tendon (DT), an indirect tendon (IT), and a variable third head. Direct and indirect tendons finally converge into a common tendon (CT). All the PTC shows a medially sloping in its proximal insertion. We investigated several anatomical specimens and discovered a new component: a membrane connecting the CT with the anterior superior iliac spine. Such membrane constitutes a new origin of the PTC. The aim of this study was to clarify

Simultaneous bilateral rupture of the patella tendon is a very rare condition, especially in young and healthy individuals. In this case report, a 44-year-old male sustained bilateral rupture while jumping during an amateur handball game. He was examined with X-ray and MRI and received surgery in both knees with suture of the tendon and reinforcement with metal cerclages. MRI of suspected injuries to the knee extensor mechanism is suggested to be of diagnostic value.

Tibialis posterior tendon dislocation is a rarely described entity that is easily missed, resulting in delayed diagnosis and treatment. A review of the English published data on the topic showed inconsistency in the reporting of injuries and surgical management techniques, leading us to describe a novel classification system to guide treatment and future reporting. We also describe a case of tibialis posterior tendon dislocation in a professional volleyball player and our surgical technique for correction, including retromalleolar groove deepening.

OBJECTIVE: Hamstring injuries are common in sports. Although management and outcomes are sport specific, clinical evaluation alone is a poor guide for treatment planning and prognostication. Cross-sectional imaging has added value in these cases.
CONCLUSION: Specifically, the location (tendon attachment, classic or intramuscular myotendinous junction, or extramuscular portion of the tendon), specific muscles involved, and anatomic extent are factors that can influence the immediate treatment, expected convalescent period, and risk of recurrence in these athletes.

INTRODUCTION: We sought to highlight the significance of ultrasound in assessing injuries to the Achilles tendon.
METHODS: Using a case study approach, we present two case reviews involving insertional enthesopathy and tendonitis of the Achilles.
RESULTS: Our study shows the lesions in the Achilles tendon and the peritendinous structures noted on clinical examination of the patients reviewed were better characterized with the use of ultrasound.
CONCLUSION: This outcome is in line with information from available literature and current practice.

Achilles tendon rupture is a relatively uncommon occurrence in a general ED population. The history can be subtle, and physical findings may not be clear-cut. Prompt diagnosis and treatment of these injuries, however, is important to improved clinical outcome. The emergency physician needs to remain vigilant for this diagnosis to avoid this orthopedic pitfall. This review article examines the clinical presentation, diagnostic technique, and management options applicable to the emergency physician in the treatment of Achilles tendon rupture.

Operative repair of the Achilles tendon is associated with complications such as wound issues, infection, and rerupture, presenting significant challenges for orthopedic surgeons. With a range of salvage techniques available-but no clear consensus or algorithm on the optimal approach-treatment must be tailored to each patient and case. This article explores various salvage options, including tendon transfers, V-Y advancements, gastrocnemius turn-down procedures, and allograft techniques, illustrated through case examples to provide practical insights.

Tendon injuries are common and heal through the formation of a fibrotic scar rather than regeneration. This poor natural healing results in impaired function and increases the risk of reinjury. In 2019, we reviewed the current state of the field's understanding of the cellular basis of fibrotic tendon healing, focusing on the resident and infiltrating cell populations that appear at key stages and what was known about their role in tendon healing at the time. Since then, advances in transcriptomic technologies and widespread adoption of lineage tracing have refined our understanding of these p…

PURPOSE: Flexor tendon adhesion to tissues is one of the most frequent complications reported after flexor tendon repair. The human amniotic membrane (HAM) was used to wrap the tendon repair site to decrease fibrotic response and tendon adhesion.
METHODS: A total of 19 patients with flexor tendon injuries were subjected to surgical repair. The repair site was wrapped with human amniotic membrane (HAM) in nine cases. The remaining ten cases served as controls as no HAM wrap was used. The clinical outcome was assessed by pain, range of motion, and pinch strength. The healing of repair was evalu…

PURPOSE: Distal pectoralis major pathology is complex. Recent anatomical findings have identified the deltopectoral tendon, a structure distinct from the pectoralis major tendon, originating from the clavicular bundle and merging with the distal deltoid tendon. This study aims to characterize the deltopectoral tendon, assess its consistency, and define its relationship to the pectoralis major tendon to improve the understanding of distal pectoralis major pathology.

With an incidence of 3% of all biceps tendon injuries, rupture of the distal biceps tendon is a rare injury but can be associated with significant functional impairment of the elbow. In case of a complete rupture, the diagnosis can be made clinically with a pronounced power deficit, in particular for supination of the forearm. In cases of unclear symptoms magnetic resonance imaging should be included. Regarding the therapeutic approach, there is general consensus in the current literature that surgical treatment with anatomical reconstruction of the tendon footprint is superior to the conserva…

Injuries to the pectoralis major muscle are relatively infrequent. The mechanism of injury is usually an eccentric shortening of the pectoralis major under heavy load, such as when performing a bench press exercise. We report a case that presented to us with a history of sudden pain in the left pectoral region while doing heavy bench press exercise. The patient sustained a type III D pectoralis muscle -tendon avulsion. Surgical repair was done through a bi-cortical tendon sliding technique using two cortical buttons. In this article we describe our modifications to the previously described sur
