Bone healing after tooth extraction with or without an intervention: a systematic review of randomized controlled trials.

OBJECTIVES: To assess whether the use of a graft and/or membrane post-tooth extraction improves healing of the site dimensionally, radiographically, and/or histologically. MATERIALS AND METHODS: MEDLINE and EMBASE and the Cochrane Central register of controlled trials (CENTRAL) were searched up until August 2011. Randomized controlled trials that included and compared healing post-tooth extraction between a control (no intervention) and a graft and/or membrane (test) were selected. RESULTS: Titles and abstracts of 2,861 papers were screened. A total of 42 papers were selected for full text r

The Effect of Autologous Alveolar Bone Grafting on Nasolabial Asymmetry in Unilateral Cleft Lip and Palate.

The objective of this study was to examine whether an autologous alveolar bone graft has an effect on the nasolabial asymmetry in unilateral cleft lip, alveolus, and palate. Fifteen children (mean age 7.5 ± 2.4 years) with non-syndromic unilateral cleft lip and palate (CLP) were included. Non-ionizing three-dimensional images were acquired prior to and three months after the alveolar bone grafting procedure. A 2D and a landmark-independent 3D asymmetry assessment were used to detect changes of asymmetry in the nasolabial area. For the 2D assessment, a cleft and non-cleft side ratio for 4 linea

A prospective longitudinal study of postnatal dentoalveolar and palatal growth: The anatomical basis for CAD/CAM-assisted production of cleft-lip-palate feeding plates.

This study describes the dentoalveolar and palatal growth during the first months of life. Knowledge concerning this development is essential to avoid unwanted events such as mucosal ulcerations or restriction of growth when cleft-lip and palate (CLP) patients are treated. The results involve the generation of CAD/CAM CLP-feeding plates. Intraoral impressions from 32 healthy newborns were taken monthly for 5 months, supplemented by measurements of body weight, length, and occipital-frontal head circumference. The casts were digitalized, and two observers manually selected defined anatomical la

Brown tumour of the mandible in primary hyperparathyroidism; a case report.

We present an uncommon diagnosis in a 34-year-old female with a non-healing extraction socket. Incisional biopsy revealed multi-nucleated giant cells suggestive of central giant cell granuloma (CGCG). The computed tomography (CT) report made incidental note of a parathyroid mass. The parathyroid hormone (PTH) level was checked and found to be abnormally high. A diagnosis of brown tumour of the mandible was made. The patient was referred to a head and neck surgeon and the parathyroid mass was removed. The mandibular lesion was managed conservatively and continues to regress post-normalisation o

Bone grafting with platelet-rich plasma in alveolar cleft. Case report.

Bone grafting of the alveolus has become an essential part of the contemporary surgical management of oral clefts. The benefits of this procedure are the stabilization of the maxillary arch, elimination of oronasal fistulae, the reconstruction of the soft tissue nasal base support, creation of bony support for subsequent tooth eruption or, when they are not present or not preserved, for implants application. The authors show a case of bone grafting with the aid of platelet-rich plasma (PRP). Because of the difficulties due to the oral cleft and to its surgical reparation (big size of bone defe

Preservation of Bone and Soft Tissue Components of the Alveolar Ridge during Immediate Implantation in the Aesthetic Zone of Jaws with Bone Deficiency.

UNLABELLED: The use of immediate implantation (directly into the extracted tooth socket) in the aesthetic zone is a rather difficult task due to the specific structural features of the bone tissue of the alveolar processes and parts of the jaw. The aim of the study was to develop and evaluate the efficacy of the new method of immediate implantation during restoration of teeth in the anterior part of the jaw with bone deficiency, which allows obtaining a high aesthetic result. MATERIALS AND METHODS: The clinical study involved patients with partially edentulous anterior maxillae, chronic apica…

Mesenchymal dental pulp cells attenuate dentin resorption in homeostasis.

Dentin in permanent teeth rarely undergoes resorption in development, homeostasis, or aging, in contrast to bone that undergoes periodic resorption/remodeling. The authors hypothesized that cells in the mesenchymal compartment of dental pulp attenuate osteoclastogenesis. Mononucleated and adherent cells from donor-matched rat dental pulp (dental pulp cells [DPCs]) and alveolar bone (alveolar bone cells [ABCs]) were isolated and separately cocultured with primary rat splenocytes. Primary splenocytes readily aggregated and formed osteoclast-like cells in chemically defined osteoclastogenesis med

Optimizing mandibular second molar mesialization: A comparative analysis of stress distribution and displacement using tie-back and temporary skeletal anchorage device-assisted mechanisms with a nonlinear finite element model.

INTRODUCTION: This study aimed to determine the optimal approach for mandibular second molar (M2M) mesialization in mandibular first molar extraction patients under 3 distinct scenarios-corticotomy-assisted or nonassisted-by evaluating 2 mesialization techniques (temporary skeletal anchorage device [TSAD]) supported coil spring and tie-back). The finite element method was used to compare stress distribution and displacement patterns. METHODS: Six models were designed to simulate M2M mesialization. In the first 3 models (hook models), the force was applied using posted arches with tie-backs to

Prosthetic concerns about atrophic alveolar ridges.

Recently, research has provided much insight into the problem of residual ridge resorption. It has become clear that ridge maintenance and preservation dramatically improves the quality of restorative dentistry provided, in addition to lessening the complexity of the treatment plan for the dentist. This article discusses the concerns, difficulties, and prosthetic challenges that must be addressed when restoring a patient with residual ridge resorption to acceptable function and esthetics. While patients can expect to lose a significant portion of their residual ridge as a result of normal phys

Decontamination of autogenous bone grafts collected from dental implant sites via osteotomy: a review.

Autogenous bone particles can be obtained with bone collectors during implant osteotomy; however, collected bone particles contain oral microorganisms that may cause infectious complications. The purpose of this review was to summarize the clinical studies that have evaluated the degree of this contamination and the effectiveness of different decontamination methods. Stringent aspiration protocol, preoperative oral chlorhexidine rinse, and antibiotic prophylaxis are important precautions to be implemented when collected bone particles are to be implanted. Despite reducing contamination of coll

Bone augmentation techniques.

BACKGROUND: The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of partial and completely edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. For prosthetic-driven dental implant therapy, reconstruction of the alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be n

New Guidelines for the Treatment of the Alveolar Septum in the Immediate Dentoalveolar Restoration Technique Associated with Osseodensification: A Case Series.

Achieving initial implant stability at a molar extraction site can be challenging due to the bone width and quality, as well as anatomical limitations like the maxillary sinus and inferior alveolar nerve. The implant placement should achieve precise centralization with the interradicular septum to facilitate implant stabilization and postextraction alveolar ridge preservation/regeneration with bone grafting. Immediate nonocclusal crown placement aids peri-implant tissue maturation for the desired outcome. This retrospective series introduces guidelines for treating sockets based on alveolar se
