BACKGROUND: Trabeculectomy is performed as a treatment for many types of glaucoma in an attempt to lower the intra-ocular pressure. The surgery involves creating a channel through the sclera, through which intra-ocular fluid can leave the eye. If scar tissue blocks the exit of the surgically created channel intra-ocular pressure rises and the operation fails. Antimetabolites are used to inhibit wound healing to prevent the conjunctiva scarring down on to the sclera. The principal antimetabolites used are 5-Fluorouracil and Mitomycin C. Both may be applied on a sponge between the conjunctiva an…

Direct visualization of the superior angle during ab interno bleb revision can be achieved by rotating the globe with traction sutures, allowing surgeons to use a standard direct goniolens rather than an indirect gonioprism. In this series of 13 eyes with failed or failing trabeculectomy blebs, globe rotation permitted consistent identification of the trabeculectomy ostium, and facilitated effective needling with the Grover-Fellman spatula without intraoperative perforations or bleb leaks. This simple modification may broaden the adoption of ab interno bleb revision and improve access to the s…

Despite being documented in medical history from over 2400 years ago, primary congenital glaucoma (PCG), being a disease with low incidence rate, remains a challenge to ophthalmologists. The article provides a broad overview on the pathophysiology and diagnostic approach to PCG with major emphasis on the treatment options of PCG. While reviewing on the well-established treatment options, namely goniotomy, trabeculo-tomy and combined trabeculotomy-trabeculectomy, emphasis has also been made to recent updates on secondary treatments: trabeculectomy, antimetabolites, glaucoma-drainage devices and…

The medical treatment of primary open angle glaucoma has progressively become more and more efficient and safe, and surgery is therefore mos … have come from the new technique of non-penetrating deep sclerectomy, which is actually an external trabeculectomy involving removal of the area of maximal resistance to aqueous outflow. This procedure has a very low risk of complications, much lower than that of standard trabeculectomy, but its efficacy is still controversial. Whatever the technique chosen for …

BACKGROUND: Glaucoma is the term for a group of eye disorders that causes progressive damage to the optic nerve, which can lead to visual impairment and, potentially, irreversible blindness. Minimally invasive bleb surgery (MIBS) reduces eye pressure through the implantation of a device that creates a new subconjunctival outflow pathway for eye fluid drainage. MIBS is a less invasive alternative to conventional/incisional glaucoma surgery (e.g., trabeculectomy). We conducted a health technology assessment of MIBS for people with glaucoma, which included an evaluation of effectiveness, safety, …

PRCIS: This systematic review summarizes surgical options after failed glaucoma drainage devices, showing that second implants and cyclophotocoagulation provide meaningful intraocular pressure reduction, while trabeculectomy and newer surgical approaches may represent promising alternatives.
PURPOSE: Glaucoma drainage devices (GDDs) are essential in the management of refractory glaucoma, yet up to 10% fail annually. Optimal surgical management after GDD failure remains unclear. This systematic review summarizes current evidence on surgical options after failed GDDs.
METHODS: A systematic sea…
