Laryngeal cleft.

Laryngeal clefts are rare congenital anomalies, first described in 1792 by Richter, that allow communication between the tracheal and esophageal axis. The incidence is 1 in 10,000 to 20,000 births, which represents approximately 1.5% of the laryngeal pathology in children. Laryngeal clefts result from a failure of fusion of the posterior cricoid lamina and development of the tracheoesophageal septum. Recent work has further refined our understanding of this complex development.

Cricotracheal resection.

A surgeon facing the problem of a child with subglottic stenosis currently has a wide range of surgical options. Cricotracheal resection is the preferred option for grade IV and severe grade III stenoses that are clear of the vocal cords. Laryngotracheal reconstruction as a less extensive procedure is preferred for some grade II and less severe grade III stenoses. Stenosis close to the vocal cords remains a challenge and can be treated by extended partial cricotracheal resection. Pediatric surgeons and otolaryngologists should learn carefully the technique of partial cricotracheal resection fo

The role of plastic surgery in the management of airway obstruction.

A patient with the rare genetic disease of mitochondrial oxidative phosphorylation is presented. The phenotypic presentation included localized, idiosyncratic lipodystrophy that caused life-threatening respiratory obstruction. Plastic surgical excision and suction-assisted lipoplasty of huge deposits of fat and skin led to marked improvement in patient posture and ventilation. This rare disorder, stages of treatment, and salient references are discussed.

[Deglutition and partial supracricoid laryngectomies].

The indications of partial supracricoid laryngectomy have become more numerous since the first descriptions made by Majer, Labayle and Piquet. The study of the published functional results emphasizes that false passages during deglutition are the main postoperative problem. This technical note is aimed at drawing attention to the various points to be respected intra- and postoperatively to allow swallowing in as physiological a manner as possible.

Large retropharyngeal cold abscess in an adult with respiratory distress.

A rare case of an adult with a large retropharyngeal cold abscess completely obstructing the airway and causing acute respiratory distress is presented. An urgent tracheostomy to safeguard the airway was required. The clinical features, surgical anatomy, aetiopathogenesis and management of a retropharyngeal abscess are discussed.

Aspiration of a tracheostomy plug.

A plastic truncated conical tracheostomy plug passed down the outer cannula of a Jackson tracheostomy tube and into the lungs of a respirator-dependent man during the monthly change of the inner cannula. Despite repeated flexible and rigid bronchoscopic visualization, attempts at dislodgement were unsuccessful. An exploratory thoracotomy was also unsuccessful in recovering the plug. Seventeen days postaspiration, with percussion and postural drainage, the patient expectorated the plug, even in the absence of the cough reflex. Such plugs have now been modified by passing a ring through the hole

Division of the recurrent laryngeal nerve for idiopathic laryngeal spasm.

A rare case of idiopathic laryngeal spasm presented itself as sleep apnea in a middle-aged man. A tracheostomy followed by the division of the recurrent laryngeal nerve relieved all of the symptoms.

The rotary door myocutaneous flap. A reliable technique for laryngotracheal reconstruction.

We examined the possible uses of the "rotary door" sternohyoid myocutaneous flap (RDF) in laryngotracheal reconstruction. This well-vascularized myocutaneous flap, when rotated axially, can replace or widen the anterolateral walls of the airway. It provides a large epithelial surface, together with bulky structural support. The flap is readily available within the immediate surgical field and can replace large defects in the airway, from the level of the glottis to the cervical trachea. After extensive structural and soft-tissue loss of the larynx and trachea were produced to simulate commonly

An unusual complication of naso-enteral feeding with small-diameter feeding tubes.

The occurrence of nasotracheal intubation with feeding tubes of various types is well known but poorly documented. The small-diameter feeding tubes currently available for enteral hyperalimentation may be more prone to this complication because of their small size and the rigid guide wire which is required for placement. A high index of suspicion when placing these tubes in patients at risk, use of the wire guide to pass the nasopharynx only, and x-ray verification of tube location prior to usage should avert this potentially life-threatening mistake.

[Aerodynamic effect of tracheal voice tubes].

A usual rhinomanometer was used to construct an apparatus to measure the aerodynamic resistance of a tracheostomy speaking tube lying in the trachea. This resistance, evoked by the sieve or the hole in the tube, is considerable. A model of a trachea was made from a silicone tube to assess the resistance of several types of tracheostomy speaking tubes.

Bilateral arytenoid cartilage lateralization for laryngeal paralysis in the dog.

Bilateral arytenoid cartilage lateralization was used successfully to treat 3 dogs with bilateral laryngeal paralysis. In our hands, this techniques was more effective than unilateral arytenoid cartilage lateralization in resolving the signs of laryngeal paralysis and in avoiding the complications that may follow partial laryngectomy.

[Association of posterior laryngeal cleft and atresia of the oesophagus. Two cases treated successfully (author's transl)].

Two rare cases of oesophageal atresia associated with a posterior laryngeal cleft were treated successfully: a so-called "O" interarytenoid cleft recognised secondarily to the type II atresia, not treated surgically; a so-called II cricoid cleft recognised before the treatment of type I atresia was closed in a first stage by an anterior transl-laryngeal approach. A bibliographical review is followed by a discussion of the diagnosis aspects and therapeutic possibilities of laryngeal cleft, the approach, the timing of surgery on the basis of its type and the type of oesophageal atresia.

Long custom-made plastic tracheostomy tube in severe tracheomalacia.

Tracheomalacia occasionally occurs after repair of esophageal atresia and distal tracheoesophageal fistula. It is usually mild, and tracheostomy is rarely necessary. In severe cases of tracheomalacia after repair of esophageal atresia and distal tracheoesophageal fistula, the tracheomalacia may extend below the tip of a regular plastic tracheostomy tube. A patient with just such a problem is presented. A custom-made long plastic tracheostomy tube was designed. A method of gluing the upper portion of a standard plastic tracheostomy tube to an uncuffed thin-walled polyvinyl chloride endotracheal

Sleep apnea syndrome and supraglottic edema.

Obstructive sleep apnea is becoming more widely recognized. Otolaryngologists will be called upon to evaluate affected patients and offer consultation as to the appropriateness of surgical intervention. The extent to which this syndrome is attributable to edema of the larynx and hypopharynx is unknown. It may, on occasion, explain chronic fatigue, weight loss, and daytime somnolence seen in some patients following irradiation of the head and neck. Sleep apnea associated with upper airway obstruction can be confirmed by polysomnography during sleep. Monitoring the EEG, electro-oculogram, electr

Upper airway obstruction in infectious mononucleosis.

Life-threatening upper respiratory obstruction is an unusual complication of infectious mononucleosis. Although the majority of fatalities r … ectomy using a halothane and oxygen induction technique is recommended for severe airway occlusion. Tracheotomy is currently reserved for those patients with progressive alveolar hypoventilation, hypercarbia, atelectasis, and bulbar paralysis. In general, tonsillectomy is well-tolerated, eliminating airway obstruction, improving swallowing function, and rapid …
