From hospital to home following a lower limb amputation: A focus group study of healthcare professionals' views and experiences with transitioning.

INTRODUCTION: Patients undergoing major lower limb amputation often have multiple comorbidities, requiring care from numerous healthcare professionals (HCPs). Furthermore, they may experience physical, medical and practical challenges post-amputation. Patients with complex needs are particularly vulnerable during care transitions and may benefit from integrated care models. AIM: This study aimed to explore HCPs' views and experiences regarding the transition process after implementing an integrated care model for patients with lower limb amputation. METHODS: We conducted two focus groups wit

Using the RE-AIM Framework to Understand Pathway Home's Impact, Reach, and Long-Term Sustainability Within the Healthcare System.

Consistent evidence demonstrates that care transition programs can significantly improve mental health outcomes, treatment engagement, satisfaction, and overall quality of life. However, the extent and fidelity of implementation across behavioral health systems remain underexamined. This article applies the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) framework to evaluate the Pathway Home model, a community-based care transition program in New York that supports individuals returning to the community after psychiatric hospitalization or other institutional settings

Chronic postsurgical pain and transitional pain services: a narrative review highlighting European perspectives.

BACKGROUND/IMPORTANCE: Chronic postsurgical pain (CPSP) is a significant, often debilitating outcome of surgery, impacting patients' quality of life and placing a substantial burden on healthcare systems worldwide. CPSP (pain persisting for more than 3 months postsurgery) leads to both physical and psychological distress. Recognized as a distinct chronic pain entity in International Classification of Diseases, 11th Revision, CPSP enables better reporting and improved management strategies. Despite advancements in surgical care, CPSP remains prevalent, affecting 5%-85% of patients, with higher

Gemcitabine with cisplatin and nivolumab: Redefining standard of care for first-line metastatic urothelial carcinoma?

Nivolumab with gemcitabine/cisplatin in the CheckMate 901 trial improved overall and progression-free survival in front-line locally advanced/metastatic urothelial cancer. The EV-302 trial establishes enfortumab-vedotin plus pembrolizumab as the preferred standard in this setting. Personalized decisions are crucial given patient eligibility and economics. Ongoing trials and predictive biomarkers will further refine treatment strategies.

Transition and management of patients with Duchenne Muscular Dystrophy: a narrative review based on Italian experts' opinion and real-world experience.

OBJECTIVES: Duchenne Muscular Dystrophy (DMD) is a severe, progressive, X-linked disorder resulting in muscle wasting, progressive functional loss and cardiomyopathy. Therapeutic strategies feature glucocorticoid corticosteroids plus gene therapy/stop codon read-through, plus standards of care. Prolonged survival, delayed loss of ambulation (LoA), and innovative treatment prescriptions pose new clinical challenges, including identification of new outcome measures/targets and implementation of continuity of care. METHODS: We report on the results of an Italian experts' meeting held in Rome, It

Successful transition for adolescent survivors of CHD: the Cambridge cardiac transition pathway.

PURPOSE: National standards to ensure effective transition and smooth transfer of adolescents from paediatric to adult services are available but data on successful transition in CHD are limited. The aim of this study is to assess the effectiveness of our transition pathway. METHODS: Adolescents with CHD, aged 15-19 years, who attended the joint cardiac transition clinic between 2009 and 2018 were identified from the Patient Administration Systems. Patient attendance at their first adult CHD service appointment at Royal Papworth Hospital was recorded. RESULTS: 179 adolescents were seen in th

Irreversible Electroporation Balloon Therapy for Palliative Treatment of Obstructive Urethral Transitional Cell Carcinoma in Dogs.

Progression of transitional cell carcinoma (TCC) in dogs often leads to urinary obstruction. This observational pilot study aimed to evaluate the safety and efficacy of irreversible electroporation (IRE) balloon therapy for the palliative treatment of TCC with partial urethral obstruction. Three client-owned dogs diagnosed with TCC causing partial urethral obstruction were enrolled. After ultrasonographic and cystoscopic examination, IRE pulse protocols were delivered through a balloon catheter device inflated within the urethral lumen. After the procedure, the patients were kept overnight for

[Patient transitioning is only successful if paediatric urology remains a part of urology].

Thanks to the excellent care of complex urogenital malformations during the neonatal period and childhood available today, an increasing number of patients require competent long-term care in adulthood. Ideally, urology takes responsibility for the transition of these individuals, who have a lifelong need for urological follow-up care. Only if adequate knowledge is available and maintained will these patients receive appropriate care throughout adult life. This care is demanding and must be reimbursed appropriately. Solid basic training in urology, as well as specialized additional training in

The young adult with Juvenile Idiopathic Arthritis (JIA): A clinical review of transition, complications, and psychosocial management.

Juvenile idiopathic arthritis (JIA) is the most common chronic rheumatic disease of childhood, and up to half of patients continue to experience active disease or complications into adulthood. Despite this, adult rheumatology care remains largely oriented toward adult-onset disease, creating a mismatch in expertise for patients with childhood-onset arthritis. As a result, adult providers must address clinical challenges unique to childhood-onset disease, including persistent inflammation, temporomandibular joint (TMJ) involvement, long-term medication exposure, chronic pain, psychosocial morbi

[Upper tract urothelial carcinoma - regional differences observed in several quality indicators].

Upper tract urothelial carcinoma is a rare disease entity with complex diagnostic and treatment options. In the Swedish National Quality Register for Bladder and Urinary Tract Cancer (SNRUBC), prospective registration of tumour and treatment characteristics is performed with high coverage, where data including quality indicators are presented online (RODRET). Between 2023 and 2024 regional differences in the majority of quality indicators are observed in the SNRUBC, with target levels only met in a few regions. For some quality indicators, such as proportion of patients referred to multidiscip

[A consensus report by the Working Group on Eating Disorders of Sociedad Española de Nutrición Clínica y Metabolismo (GTTCA-SENPE). Evaluation, medical and nutritional management of anorexia nervosa. Update 2023].

Anorexia nervosa (AN) is a multifactorial disorder. A possible role of the social network and the gut microbiota in pathogenesis has been added. Exogenous shocks such as the COVID19 pandemic have had a negative impact on patients with AN. The potential medical and nutritional impact of malnutrition and/or compensatory behaviors gives rise to a complex disease with a wide range of severity, the management of which requires a multidisciplinary team with a high level of subject matter expertise. Coordination between levels of care is necessary as well as understanding how to transition the patien

The importance of genetic counselling for turner syndrome transition.

The healthcare transition (HCT) is the process of planning, monitoring, and adjusting the clinical management from children's care to adult specialists. Although this practice is common for all children, it is especially crucial (and challenging) for those with chronic disorders and genetic conditions that also involve mental health issues, requiring a multidisciplinary approach. In this review, we aim to assess the current status of transition for girls and young women with Turner syndrome (TS) as a model as it is one of the most common sexual chromosomal aneuploidies. We first describe the s

[Tuberous sclerosis complex: diagnosis and current treatment].

Tuberous sclerosis complex is an autosomal dominant genetic multisystemic disorder caused primarily by mutations in one of the two tumor suppressor genes TSC1 or TSC2, resulting in increased activation of the mTOR pathway. Regarding clinical manifestations, a wide range of phenotypic variability exists, with symptoms constellations that may differ in affected organs (brain, skin, heart, eyes, kidneys, lungs), age of presentation and severity, but usually with great impact in biopsychosocial aspects of health and in quality of life. Main clinical neurological features are epilepsy (frequently,

A narrative review of mental health support for people during transition from incarceration to community: the grass can be greener on the other side of the fence.

BACKGROUND: Despite significant need for mental health services targeting the requirements of inmates transitioning into the community there is little research about successful recovery-oriented or person-centred transition programs. AIMS: This systematic narrative review brings together existing evidence to inform policymakers and practitioners about current practice in transition support, and barriers and facilitators of effective practice. METHOD: We carried out a systematic narrative review of recovery-oriented or person-centred mental health support programs supporting transition from i

Growing up with juvenile idiopathic arthritis.

Juvenile idiopathic arthritis (JIA) is the most common chronic pediatric illness in the United States. The disease encompasses a group of heterogeneous chronic arthritis conditions that begin before age 16 years and persist for more than 6 weeks. Formerly termed juvenile rheumatoid arthritis (JRA), JIA now includes polyarticular, oligoarticular, psoriatic, enthesitis-related arthritis, systematic arthritis, and undifferentiated arthritis. Diagnosis is based on clinical and laboratory features. Treatment includes immunosuppressant therapy, non-steroidal anti-inflammatory drug (NSAIDS), and biol

Perioperative chemotherapy for upper tract urothelial cancer.

Upper tract urothelial cancer (UTUC) is an aggressive disease associated with significant morbidity and mortality. Radical nephroureterectomy (RNU) with bladder cuff removal is considered the standard of care for most invasive UTUCs but distant relapses after surgery for locally advanced, high-grade disease are common. Although multimodality treatment with perioperative chemotherapy has been investigated thoroughly in recent years, adjuvant chemotherapy has primarily been analyzed in small retrospective uncontrolled studies and a clear benefit for this treatment modality is yet to be establish
