Standardization of the Child Physical Abuse Evaluation in a Pediatric Emergency Department.

BACKGROUND: Child abuse is the third leading cause of death in children aged 4 years and younger. In children aged 24 months or younger with suspected nonaccidental trauma (NAT), a skeletal survey (SS) to evaluate for occult fracture is recommended. Laboratory testing and cranial imaging are recommended in specific ages and scenarios. Before this initiative, there was no standardized guideline for NAT evaluation in our pediatric emergency department (ED). METHODS: A team of ED nurses and physicians and child abuse pediatricians reviewed data and identified barriers to obtaining laboratory tes

Psychological Distress and Cognitive Function in Women: Exploring Potential Mediation by Use of Opiates, Sleep Aids, or Minor Tranquilizers.

Objectives: Distress, including posttraumatic stress disorder (PTSD) and depression, is associated with lower cognitive function and higher use of medications, including sleep aids, opiate pain relievers, and minor tranquilizers. Whether use of these medications is linked to lower cognitive function, and whether such medication use might partially explain the relationship between distress and cognition remains unclear. Using data from 10,653 women in the Nurses' Health Study II, we assessed associations between distress and past-month medication use; medication use and cognitive function; and

Mental health burden among females living with HIV and AIDS in sub-Saharan Africa: A systematic review.

Mental health problems, particularly depression and anxiety, are common in women and young girls living with HIV/ AIDS particularly in low- and middle-income (LMICs) countries where women's vulnerability to psychiatric symptoms is heightened due to the prevalent intersectional stressors such as stigma and intimate partner violence. However, no synthesized evidence exists on the mental health burden of females living with HIV/AIDS (FLWHA) in Africa. This systematic review aimed to synthesize the current evidence on the mental health burden among FLWHA in sub-Saharan Africa. A systematic literat

Team Cognition in Handoffs: Relating System Factors, Team Cognition Functions and Outcomes in Two Handoff Processes.

OBJECTIVE: This study investigates how team cognition occurs in care transitions from operating room (OR) to intensive care unit (ICU). We then seek to understand how the sociotechnical system and team cognition are related. BACKGROUND: Effective handoffs are critical to ensuring patient safety and have been the subject of many improvement efforts. However, the types of team-level cognitive processing during handoffs have not been explored, nor is it clear how the sociotechnical system shapes team cognition. METHOD: We conducted this study in an academic, Level 1 trauma center in the Midwest

Psychological responses of hospital-based nurses working during the COVID-19 pandemic in the United States: A cross-sectional study.

Studies show decreased well-being during the COVID-19 pandemic, especially for healthcare providers from Asia. Less is known about the psychological responses of working during the pandemic on hospital-based registered nurses (RNs) in the United States (US). Therefore, the purpose of this paper is to report the well-being of U.S.-based hospital RNs working during the initial acute phase of COVID-19 and compare it with well-being among healthcare workers described in two global meta-analyses. We conducted a cross-sectional survey in May-June 2020 (N = 467). Well-being was measured using the fol

Myofascial-manual lymphatic drainage for burn trauma: a service evaluation.

Improving microcirculation in the area of a wound is an important aspect of wound healing. It can be improved by a new manual methodology: myofascial-manual lymphatic drainage (M-MLD). This hybrid technique uses elements of musculoskeletal medicine and manual lymphatic drainage. Through palpation, M-MLD therapeutically affects the functional pathology of soft tissues, with reflection in trigger points, and also the functional pathology of lymphatic system, namely by lymphostasis in particular regional lymph nodes. Most cases of post-traumatic and postoperative conditions lead to dynamic insuff

Off-Hour Admission and Mortality Risk for 28 Specific Diseases: A Systematic Review and Meta-Analysis of 251 Cohorts.

BACKGROUND: A considerable amount of studies have examined the relationship between off-hours (weekends and nights) admission and mortality risk for various diseases, but the results remain equivocal. METHODS AND RESULTS: Through a search of EMBASE, PUBMED, Web of Science, and Cochrane Database of Systematic Reviews, we identified cohort studies that evaluated the association between off-hour admission and mortality risk for disease. In a random effects meta-analysis of 140 identified articles (251 cohorts), off-hour admission was strongly associated with increased mortality for aortic aneury

[Measuring residual urine by portable ultrasound scanner].

Urethral catheterization, the standard method of measuring residual urine, is uncomfortable and associated with risk of infection and trauma to the urethra. It has also been reported as inaccurate to a certain extent. We compared catheterization with ultrasound scanning in a prospective study of 52 men and 3 women. 100 measurements of postvoiding residual urine by portable ultrasound scanner, were each followed immediately by urethral catheterization (both procedures performed by an experienced nurse in our outpatient clinic). A difference of > 25 ml between measurements by scanner and by cath

Prevention of mental handicaps in children in primary health care.

Some 5-15% of children aged 3 to 15 years in both developing and developed countries suffer from mental handicaps. There may be as many as 10-30 million severely and about 60-80 million mildly or moderately mentally retarded children in the world. The conditions causing mental handicaps are largely preventable through primary health care measures in developing countries. Birth asphyxia and birth trauma are the leading causes of mental handicaps in developing countries where over 1.2 million newborns die each year from moderate or severe asphyxia and an equal number survive with severe morbidit

Team-Based Improvements in Pediatric Massive Transfusion Practice.

INTRODUCTION: Massive transfusion protocols (MTP) reduce morbidity and mortality in patients with life-threatening hemorrhage. However, in pediatric settings, MTP activations are rare, leading to inconsistent transfusion practices and underuse of adjunct therapies. We hypothesized that implementing a multidisciplinary MTP response team at our institution would be associated with improved adherence to best practice blood product ratios during pediatric MTP. METHODS: We conducted a retrospective cohort study of pediatric patients who underwent MTP activation at our tertiary pediatric hospital b

Five-year outcomes in a randomised controlled trial of prolonged exposure therapy and supportive counselling for post-traumatic stress disorder in adolescents: a task-shifted intervention.

BACKGROUND: Cognitive-behavioural therapies with a trauma focus are effective in reducing posttraumatic stress disorder and other psychological distress in adolescents. Long-term follow-up data on adolescents treated for PTSD remain scarce, with few studies extending beyond 12 months after treatment completion. OBJECTIVE: To evaluate the maintenance of treatment gains in a comparative study of effectiveness of PE-A and SC up to 60 months post-treatment. METHOD: Sixty-three adolescents diagnosed with PTSD were randomly assigned to either treatment, provided by newly trained and supervised non
