[Tuberculous spondylitis of the cervical spine].

WHO estimates that worldwide more than 4 million people are infected with tuberculosis (TB), 95% of them living in third world countries. TB is once again the most frequent infectious disease [1, 2, 7]. Extrapulmonary forms (EPTB) frequently appear with HIV-associated TB [3, 4, 6]. We present the case of a 23-year old patient with abscess forming TB of the right apical lung with infiltration and partial osteolytical destruction of the 7th cervical- and the first thoracal vertebral body. Progressive spinal compression lead to partial sensomotorical deficits. Surgical revision of the abscess, an

Spinal Castleman disease: An imposter-A rare case from Ethiopia.

Castleman disease is a rare lymphoproliferative disorder that presents significant diagnostic challenges, particularly when involving the spine. Its radiologic features often mimic more common pathologies such as lymphoma, meningioma, metastatic disease, and infection like spinal tuberculosis-conditions frequently encountered in Ethiopia. We report a case of a 62-year-old Ethiopian female who presented with progressive paraparesis. MRI revealed a homogeneously enhancing epidural mass with mildly expansile posterior vertebral elements involvement at the T4 level showing diffusion restriction on

Differential diagnosis of dumbbell lesions associated with spinal neural foraminal widening: imaging features.

Computed tomography (CT) and magnetic resonance imaging (MRI) reliably demonstrate typical features of schwannomas or neurofibromas in the vast majority of dumbbell lesions responsible for neural foraminal widening. However, a large variety of unusual lesions which are causes of neural foraminal widening can also be encountered in the spinal neural foramen. Radiologic findings can be helpful in differential diagnosis of lesions of spinal neural foramen including neoplastic lesions such as benign/malign peripheral nerve sheath tumors (PNSTs), solitary bone plasmacytoma (SBP), chondroid chordoma

[Tuberculosis spondylodiscitis in three older patients].

Three patients, two women aged 78 and 70 years, respectively, and a man aged 71 years, had back pain for months. Fever was absent. Blood parameters of infection were slightly or highly elevated. The diagnosis of spondylodiscitis was confirmed by MRI in all three patients. In one patient tuberculosis was confirmed by culture. One patient was from Turkey and the other two patients had been exposed professionally to tuberculosis. On two occasions, spondylodiscitis was complicated by compression of the spinal cord, and surgical intervention was necessary on one occasion because of neurological def

Morphologic and Clinical Aspects of Pott Disease in Ancient Human Remains: A Scoping Review.

STUDY DESIGN: The present study is a scoping review of the literature on Pott disease in ancient human remains. OBJECTIVE: Comprehending the origin and history of Pott disease is relevant to assessing this pathologic condition from an evolutionary perspective. SUMMARY OF BACKGROUND DATA: Tuberculosis (TB) is an ancient pulmonary disease that remains the leading cause of death from a single infectious agent, rating above HIV/AIDS. The disease typically affects the lungs but can also target other anatomic sites. Of those, the most common and characteristic are the skeletal changes involving th

Tuberculosis-triggered cytokine storm with hemophagocytic lymphohistiocytosis and tuberculous spondylitis in an apparently immunocompetent host: a case report and literature review.

Hemophagocytic lymphohistiocytosis (HLH) secondary to disseminated tuberculosis (TB) is a rare, life-threatening hyperinflammatory syndrome. We present a 60-year-old man with recurrent fever and syncope. Workup revealed cytopenias, hyperferritinemia (peak 5,802 ng/mL), elevated C-reactive protein (CRP), and hepatic dysfunction, fulfilling HLH-2004 criteria. Imaging showed miliary lung nodules and tuberculous spondylitis at T9. Bone marrow biopsy confirmed hemophagocytosis, and next-generation sequencing identified Mycobacterium tuberculosis. This case demonstrates that disseminated TB can trig

Idiopathic scoliosis: the relation between the vertebral canal and the vertebral bodies.

STUDY DESIGN: The axial length of the vertebral canal and the anterior aspect of the vertebrae were measured in 36 skeletons, 15 with probable idiopathic scoliosis. OBJECTIVES: To compare the discrepancy in length of the vertebral canal and the anterior spinal column in skeletons having probable idiopathic scoliosis with the degree of deformity. SUMMARY AND BACKGROUND DATA: In idiopathic scoliosis, the vertebral bodies rotate toward the convexity of the curve, whereas the vertebral canal tends to retain a midline position. The vertebral canal therefore will be relatively short. The degree of

Kyphosis of the thoracic and thoracolumbar spine in the pediatric patient: normal sagittal parameters and scope of the problem.

As measured by the Cobb angle, normal sagittal kyphosis is 20 degrees to 40 degrees, which encompasses most of the angulated consecutive vertebrae in the thoracic region of the spine. With pathologic kyphosis, however, the segmental analysis of different regions of the thoracic spine plays an important role. Methods of determining sagittal measurements as well as the causes of kyphosis of the thoracic and thoracolumbar spine, including Scheuermann's disease, spinal cord injury, laminectomy, neurofibromatosis, genetic origins, Marfan syndrome, and tuberculosis, are also important in determining

FURTHER STUDIES OF EXPERIMENTAL ALLERGIC ENCEPHALOMYELITIS IN THE FOWL. II. THE INFLUENCE OF DOSE VARIATIONS ON THE HISTOLOGICAL LESIONS.

The effects of varying the dose of the constituents of an inoculum known to produce EAE in a reciprocal cross of Brown Leghorn fowls have been studied. Large amounts of the emulsifying agent suppressed the severity of the histological lesions. Fifty mg. of homologous spinal cord and 2 mg. of Mycobacterium tuberculosis per bird were found to be the optimum doses necessary to produce consistently severe histological lesions. A standard procedure for inducing EAE for quantitative studies in `B×I' Brown Leghorn fowls was adopted.

Tuberculosis revisted: classic imaging findings in childhood.

Tuberculosis (TB) remains one of the major public health threats worldwide, despite improved diagnostic and therapeutic methods. Tuberculosis is one of the main causes of infectious disease in the chest and is associated with substantial morbidity and mortality in paediatric populations, particularly in low- and middle-income countries. Due to the difficulty in obtaining microbiological confirmation of pulmonary TB in children, diagnosis often relies on a combination of clinical and radiological findings. The early diagnosis of central nervous system TB is challenging with presumptive diagnosi

Musculoskeletal tuberculosis.

The diagnosis of musculoskeletal tuberculous infection remains a challenge to clinicians and requires a high index of suspicion. The combination of indolent onset of symptoms, positive tuberculin skin test, and compatible radiographic findings strongly suggest the diagnosis. TB, however, must be confirmed by positive culture or histologic proof from the aspiration of synovial fluid or biopsy of the bone or synovium. Prompt diagnosis and treatment of skeletal TB are important to prevent serious bone and joint destruction and severe neurologic sequelae in the case of spinal involvement.

The first rare and fatal case of invasive aspergillosis of spinal cord due to Aspergillus nidulans in an Iranian child with chronic granulomatosis disease: review of literature.

BACKGROUND AND PURPOSE: Invasive aspergillosis (IA) of the central nervous system (CNS) is a devastating complication which is rarely reported in immunocompromised children. In this case presentation, we reported a rare and fatal IA with spinal cord involvement in a 10-year-old child with X-linked chronic granulomatosis disease (CGD). CASE REPORT: The child had a previous history of pulmonary tuberculosis. A cervical spine X-ray revealed the involvement of cervical vertebrae (T4/T5) and ribs causing spinal cord compression and epidural abscess. The patient underwent a decompressive laminectom

Streptococcus pneumoniae meningitis complicated by an intramedullary abscess: a case report and review of the literature.

BACKGROUND: Intramedullary abscess is a rare neurosurgical condition that usually arises in the setting of penetrating trauma to the spinal cord, infected congenital dural sinuses, or tuberculosis. CASE PRESENTATION: We describe a case of a 35-year-old African American male who presented with sepsis and a clinical picture of meningitis. The patient continued to have declining neurological status with decreasing sensation and worsening motor strength in all four extremities. He was found to have an intramedullary abscess in the cervical spinal cord that was treated with a decompressive posteri

Tuberculous lumbar arachnoiditis mimicking conus cauda tumor: A case report and review of literature.

Tuberculous spinal arachnoiditis involving cauda equina is rare. A patient with lumbar tuberculous arachnoiditis in the absence of both vertebral and meningeal tuberculosis, which was mimicking spinal intradural extramedullary tumor is described here. Diagnosis was made based on intraoperative findings and was confirmed by histopathology. Surgical decompression along with a combination of steroid and antitubercular therapy resulted in a good outcome. At 3 months follow-up, the patient regained bladder control and was able to walk with support. Clinical features, magnetic resonance imaging, and
