[Height-induced vertigo and its medical interpretation: Goethe and the Strassburger Münster].

An analysis combining medicine and literature challenges the methodology of both disciplines. This essay on the vertigo Goethe suffered on the tower of the Strasbourg Minster attempts to trace the vicissitudes of interpreting an emblem, like vertigo, burdened by cultural meaning and implications. Thus, Goethe's own report of this event 40 years after the fact, in his "Dichtung und Wahrheit", has to be related to another, hidden chronology of vertigo and fear in his account which, at first glance, conveys quite different implications. The first part of this paper refers to a medical interpretat…

Transtympanic dexamethasone application in Ménière's disease: an alternative treatment for intractable vertigo.

The aetiology of Ménière's disease still remains unknown and its therapy is therefore empirical. As a result of immunological abnormalities demonstrated, steroids are commonly used in Ménière's disease. The place of topical steroids is still controversial. In this investigation topical dexamethasone is applied for three months through a ventilation tube in patients with intractable vertigo. The results showed that this treatment controls vertigo in 72 per cent of cases. No patient was worse than before treatment. Only in 17 per cent of the patients was there an increase in hearing level. When …

Success of the modified Epley maneuver in treating benign paroxysmal positional vertigo.

OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) is a common condition seen by otolaryngologists. The purpose of this study is to determine the ability of the modified Epley maneuver to treat BPPV. STUDY DESIGN: Retrospective review. METHODS: A retrospective chart review of 107 patients diagnosed with BPPV at our institution between March of 1993 and June of 1995. Each patient was diagnosed with isolated BPPV by history and Hallpike-Dix maneuver. There were no other vestibular symptoms or electronystagmogram abnormalities. Patients diagnosed with BPPV received modified Epley maneuvers,…

[Study of the vestibule in patients with dizziness and vertigo after 60 years of age].

Aging of the vestibular apparatus is shown to consist of processes similar to those occurring in other parts of the nervous system. The recognition of these findings is a first step in the clinical differentiation of presbyastasis from symptoms related to specific disease processes. Authors had reviewed results of otoneurological investigations in the material of 8328 patients and realised that 1034 of them (12.4%) were over-60-year-old. These patients suffered from pathology of peripheral as well as central part of vestibular system.

Technique and results of singular neurectomy for the management of benign paroxysmal positional vertigo.

One hundred thirty-seven patients with chronic disabling paroxysmal positional vertigo were treated surgically with singular neurectomy (SN) during the years 1972-1994. One hundred forty-six SN were performed on these patients. One hundred twenty-eight patients underwent unilateral SN while 9 patients underwent bilateral sequential SN. The patients ranged in age from 25 to 86 years with a mean of 57 years and a female to male ratio of 92: 45. One hundred thirty-eight patients (94%) experienced complete relief of vertigo following SN. Three patients (2%) experienced partial relief of positional…

A practical approach to dizziness. Questions to bring vertigo and other causes into focus.

Evaluation of a patient presenting with dizziness begins with and largely depends on the patient's history. The diagnosis often can be accurately determined in a primary care setting when a stepwise algorithmic approach is used. The first step is getting a detailed account of precisely what the patient means by "dizziness." This helps determine whether the cause is vertigo or another condition, such as orthostatic hypotension. Establishing whether the vertigo is central or peripheral in origin and, if peripheral, how long episodes last further focuses the investigation. Certain clues on physic…

[Diagnostic and therapeutic problems in vestibular neuronitis: clinical implications for sudden vertigo].

We defined sudden vertigo as a sudden, unilateral peripheral vestibular dysfunction. The criterion for its diagnosis is a single episode of vertigo without cochlear and central symptoms. Among 20 patients with sudden vertigo there was no difference in clinical aspects between those with CP (canal paresis) (CP% > or = 25%) and those without CP (CP% < 25%). This suggests that sudden vertigo with CP is due to sudden vestibular dysfunction with predominant involvement of the lateral semicircular canal. Basically, vestibular neuronitis is considered to be due to acute unilateral neuropathy of the v…

A longitudinal study of symptoms, anxiety and subjective well-being in patients with vertigo.

In a prospective longitudinal study of the relationship between symptoms and anxiety in people with vertigo, 101 patients were evaluated on two occasions separated by a 7 month interval. At Time 1 the age, gender, vertigo type, duration of illness, medication, and audiovestibular test results of patients were recorded, and they completed questionnaires assessing handicap, emotional distress, predisposition to anxiety, and symptoms indicative of vertigo and of somatic anxiety. These questionnaires were re-administered at Time 2, and patients also indicated whether their vertigo was more or less…

[Paget's disease with temporal bone involvement, hypoacusis and vertigo. Apropos a case].

The Paget disease is a chronic osteodystrophy of unknown etiology, very frequent in 50 years of age and above. It's an anatomo-radiologic syndrome that sometimes, is accompanied by clinical symptoms. May be monostotic or polyostotic but never generalized. Occasionally it affects the temporal bone producing deafness, tinnitus and vertigo. We report a case with important temporal bone demineralization, deafness and vertigo.

[Isolated vertigo disclosing infarction in the area of the posterior and inferior cerebellar arteries].

We report three cases of small cerebellar infarcts mimicking labyrinthine dysfunction. A sudden rotatory vertigo might be the only presenting symptom of a cerebellar infarct. In these cases, the clinical features may closely mimick an acute peripheral labyrinthine disorder. However, the absence of nystagmus or a direction changing nystagmus with different eye position and the normality of caloric responses may be suggestive of a cerebellar infarct. This syndrome may be explained by the involvement of the nodulus, part of the flocculo-nodular complex, that has primary vestibular connections. Ce…

The effects of two anti-vertigo drugs (betahistine and prochlorperazine) on driving skills.

1. The effects of betahistine 72 mg three times daily, prochlorperazine 5 mg three times daily and placebo taken for 3 days before testing were compared on two actual driving tasks (weaving and gap estimation) and two psychomotor tasks (reaction time and kinetic visual acuity) in normal subjects in a double-blind prospectively randomised cross-over study. 2. The psychomotor effects of betahistine could not be distinguished from those of placebo. 3. Prochlorperazine impaired driving performance causing increased carelessness and slowing on the weaving test. 4. There was little subjective apprec

Results of microvascular decompression of the eighth nerve as treatment for disabling positional vertigo.

A total of 41 patients who underwent microvascular decompression to relieve disabling positional vertigo in one 12-month period were studied. At follow-up examinations 2 1/2 to 3 1/2 years after the operation, 30 patients had total relief of their symptoms or were considerably improved, to the point that they could resume normal work or other activities in which they were not able to engage before the operation. Two of the 41 patients had mild improvement, and the final 9 patients had no change in their symptoms. One patient, who had had a previous vestibular nerve section without improvement,…

Vestibulospinal function in two syndromes with spontaneous attacks of vertigo: evaluation by posturography.

Two vestibular syndromes comprising attacks of spontaneous vertigo were studied and disturbance of the vestibulospinal reflex was evaluated. Patients with Menière's disease were examined between attacks and patients with a sudden unilateral vestibular loss syndrome at least 3-4 weeks after the initial attack. Posturography, (recording and analysing the postural sway) was used to evaluate the influence of these disorders upon the vestibulospinal reflex. In 50% of the patients, the postural sway was disturbed compared with the performance of a normal control group. Menière's disease had a more p…
