Early hemi-diaphragmatic plication through a video assisted mini-thoracotomy in postcardiotomy phrenic nerve paresis.

New symptom onset of respiratory distress without other cause, and new hemi-diaphragmatic elevation on chest radiography postcardiotomy, are usually adequate for the diagnosis of phrenic nerve paresis. The symptom severity varies (asymptomatic state to severe respiratory failure) depending on the degree of the lesion (paresis vs. paralysis), the laterality (unilateral or bilateral), the age, and the co-morbidity (respiratory, cardiac disease, morbid obesity, etc). Surgical treatment (hemi-diaphragmatic plication) is indicated only in the presence of symptoms. The established surgical treatment

Videoscope-Assisted Cleft Palate Surgery: Rationale, Experience, and Technical Implementation.

Limited visibility characteristic of cleft palate repair presents both ergonomic and educational challenges to cleft surgeons. Despite widespread recognition and reporting, posture-related spine disorders continue to represent a significant and potentially career-limiting problem for cleft/craniofacial surgeons. In addition, education and participation during palate repairs is difficult because of visual field constraints. At the authors' institution, a novel videoscope system was designed and implemented to (1) provide visualization for all surgical team members during palate operations, (2)

Management of bleeding complications during thoracoscopic thymectomy.

Video-assisted thoracoscopic thymectomy is increasingly performed for thymic disorders. However, intraoperative bleeding, especially from the innominate vein, is often inevitable, and is the major reason for conversion. Successful control under thoracoscopic thymectomy is crucial for the safety of the patient. We hereby report two cases of intraoperative bleeding and strategies for safe and effective bleeding control during the conventional three-port video-assisted thoracoscopic thymectomy. The first case was an 80-year-old male patient diagnosed with cystic thymoma, and injury to a thymic ve

Structured light for touchless 3D registration in video-based surgical navigation.

PURPOSE: Arthroscopic surgery, with its inherent difficulties on visibility and maneuverability inside the joint, poses significant challenges to surgeons. Video-based surgical navigation (VBSN) has proven to have clinical benefits in arthroscopy but relies on a time-consuming and challenging surface digitization using a touch probe to accomplish registration of intraoperative data with preoperative anatomical models. This paper presents an off-the-shelf laser scanner for noninvasive registration that enables an increased area of reachable region. METHODS: Our solution uses a standard arthros

A Novel, Small-Incision, Gasless Videoscope-Assisted Necrosectomy for Infected Walled-Off Necrosis: A Single-Center Experience.

When endoscopic drainage is challenging for infected walled-off necrosis with solid-predominant debris, gasless videoscope-assisted necrosectomy (VANE) offers a complementary surgical step-up option. With accompanying video, Sasanuma and colleagues demonstrate how its wide-bore corridor enables effective, single-session bulk removal of extensive solid debris while preserving vital abdominal vessels through a small incision.

Evolution of ergonomics in thoracic surgery: a comparative analysis of the surgeon's physical and cognitive load during VATS and RATS.

INTRODUCTION: Minimizing patient trauma in thoracic surgery introduces hidden ergonomic and cognitive burdens for the surgeon. This review aims to comparatively analyze these loads during the transition from thoracoscopy (video-assisted thoracoscopic surgery - VATS) to robotic-assisted surgery (robotic-assisted thoracic surgery - RATS). METHODS: A literature review was conducted focusing on objective (surface electromyography, RULA kinematic analysis, eye blink rate) and subjective (NASA--TLX) ergonomic assessments in multi-port VATS, uniportal VATS (uVATS), and RATS during pulmonary resectio

GEN-Guard: correcting generalization failures for deployable federated surgical AI.

PURPOSE: Federated Learning (FL) in surgical video AI enables collaborative model training without sharing sensitive data. However, standard evaluation practices-selecting the "best" global model based only on validation data from participating hospitals-can lead to suboptimal deployment choices. We identify this critical failure mode as performance leakage, where the selected model overfits internal federation data and fails to generalize to unseen institutions, thereby undermining the core goal of FL: robust real-world generalization. METHOD: We propose GEN-Guard, a practical post-hoc frame

Clinical utility of lung biopsy in fibrotic interstitial lung disease.

Lung biopsy has traditionally played a fundamental role in addressing diagnostic uncertainty and guiding management of fibrotic interstitial lung disease (fILD). Multiple lung biopsy procedures are available for evaluating fILD, including bronchoscopic techniques such as transbronchial biopsy and transbronchial lung cryobiopsy, as well as surgical lung biopsy, which is now almost exclusively conducted using minimally invasive video-assisted thoracoscopic surgery. The various evolving -considerations for and against lung biopsy have led to substantial ambiguity regarding the optimal timing and

Surgical Treatment of Pulmonary Nontuberculous Mycobacterial Infections.

Adjuvant surgical resection in the setting of pulmonary nontuberculous mycobacterial infection removes focal parenchymal disease thought to serve as a poorly perfused reservoir for organisms, thus relatively resistant to standard antimicrobial therapy. Removal of these areas of damaged lung is felt to enhance the effectiveness of the medical treatment. In general, these operations are associated with low morbidity and mortality, although resections that are more extensive carry higher risk. Many of the planned operations may be performed with minimally invasive techniques. More data are needed

[Thymic Lymphoepithelial Carcinoma:Report of a Case].

Thymic lymphoepithelial carcinoma (LEC) is a rare subtype of thymic cancer and reported in 6% of thymic carcinoma. In terms of histopathology, it closely mimics undifferentiated tumors that originate in the nasopharynx, and in half of the cases, it has been associated to the Epstein-Barr (EB) virus infection. Seven reports have been published on the result of surgical resection, particularly in the early stages of thymic LEC. Recurrence was reported in three patients, and one patient died. In our case, the patient underwent video-assisted thoracoscopic surgery for a partial thymectomy and thym

Causes, management and treatment of empyema.

Infections in the pleural space have been a significant problem since ancient times and continue to be so today, with an incidence of 52% in patients with post-pneumonia syndrome. Typically, these effusions require a combination of medical treatment and surgical drainage, including debridement and decortication. Researchers have been studying the use of intrapleural fibrinolytics in managing complicated pleural effusions and empyema, but there is still ongoing debate and controversy among clinicians. Empyema has traditionally been considered a surgical disease, with antibiotics and chest tube

Preservation vs. dissection of inferior pulmonary ligament for thoracoscopic upper lobectomy: a prospective randomized controlled trial.

OBJECTIVES: The proper procedure for inferior pulmonary ligament (IPL) during upper lobectomy remains a topic of debate. To address this matter, we carried out a trial comparing the clinical outcomes of IPL preservation versus IPL dissection during thoracoscopic upper lobectomy (TUL). METHODS: Patients undergoing thoracoscopic left/right upper lobectomy (TLUL/TRUL) were assigned to either the dissection group (Group D) or the preservation group (Group P). Our primary objective was to quantify and compare the alterations in postoperative residual bronchial angle and lung volume changes between

A comparative study of robot-assisted thoracoscopic surgery and conventional approaches for short-term outcomes of anatomical segmentectomy.

OBJECTIVES: Since anatomical segmentectomy requires meticulous dissection of the segmental pulmonary vessels and bronchus, robot-assisted thoracoscopic surgery (RATS) has been widely adopted in recent years. We investigated the usefulness of RATS segmentectomy by comparing perioperative outcomes with conventional approaches including open thoracotomy or video-assisted thoracoscopic surgery (VATS). We compared perioperative outcomes of segmentectomy between RATS and conventional approaches including open thoracotomy or video-assisted thoracoscopic surgery (VATS). METHODS: This single-instituti

[Robot-assisted Extended Thymectomy via a Bilateral Approach for Patients with Myasthenia Gravis].

Extended thymectomy is a procedure to remove the thymus gland and surrounding adipose tissue, while the traditional approach via a median sternotomy, minimally invasive approaches such as video-assisted thoracoscopic surgery (VATS) and robot-assisted thoracoscopic surgery (RATS) have been adopted. This report described the technique of bilateral approach for extended thymectomy in patients with myasthenia gravis (MG) by robot-assisted thoracoscopic surgery, and also showed the perioperative outcomes and postoperative exacerbation rates of 11 patients. In most patients, score of MG symptom were

Sub-axillary cosmetic incision versus single-incision thoracoscopic surgery for primary spontaneous pneumothorax.

BACKGROUND: In recent years, single-incision thoracoscopic surgery (SITS) has been increasingly applied as an optimal treatment option for primary spontaneous pneumothorax (PSP). However, most SITS techniques are used in the fourth to sixth intercostal space between the anterior axillary and mid axillary lines. To find out more concealed incisions, this study performed PSP surgery via the sub-axillary cosmetic incision (SACI) technique. METHODS: A total of 128 PSP patients were subjected to video-assisted thoracoscopic surgery (VATS) between January 2017 and January 2019 at our institution. T

Which pneumothorax volume/size measurement method can best predict surgical indication in primary spontaneous pneumothorax patients? A comparison of six different methods.

OBJECTIVES: We determined whether patients requiring surgery could be identified using pneumothorax volume/size in the first episode of primary spontaneous pneumothorax (PSP) and confirmed which of six different measurement methods of pneumothorax volume/size best predicted surgical indication (prolonged air leak, PAL, or ipsilateral recurrence). METHODS: Between 2013 and 2017, 282 patients with PSP were analysed. The volume of the pneumothorax (%) was calculated using volumetric methods (Collins, Rhea, Kircher, Light), while its size (large or small) was determined using the British Thoracic
