CASE DESCRIPTION: An 8-week-old female domestic shorthair cat was treated for patent ductus arteriosus (PDA) with surgical ligation. Seven weeks postoperatively, the cat was evaluated because of increased upper respiratory noise, inspiratory stridor, wheezing, and episodes of intermittent open-mouth breathing that had developed 1 week following the surgical ligation.
CLINICAL FINDINGS: The cat was sedated, and examination of the larynx revealed left-sided laryngeal paralysis.
TREATMENT AND OUTCOME: At the time left-sided laryngeal paralysis was diagnosed, the clinical signs of laryngeal dysf

HYPOTHESIS: The optimal degree of arytenoid rotation for arytenoid adduction (AA) can be determined using quantitative real-time voice analysis.
STUDY DESIGN: Repeated measures with each larynx serving as its own control.
METHODS: Unilateral vocal fold paralysis (VFP) was modeled in five excised canine larynges. Medialization laryngoplasty (ML) was performed, followed by AA. The optimal degree of arytenoid rotation was determined using real-time measurements of vocal efficiency (V(E) ), percent jitter, and percent shimmer. After the optimal degree of rotation was determined, the arytenoid wa

The role of the laryngeal reflex in glottal movement has been reported, but its mechanism remains unclear. To further investigate the neurophysiological characteristics of glottal movement, we recorded the laryngeal reflexomyographic responses (LRMRs) to electrical stimulation of the superior laryngeal nerve (SLN) in rabbits. The procedure involved simultaneous recording of the LRMRs from the thyroarytenoid muscles by means of bipolar hooked wire electrodes after electrical stimulation to the SLN. The results demonstrated characteristic patterns of the responses, consisting of R1 and R2, simil

In this study, we developed a rat model for Teflon granuloma and used this model to evaluate the removal of the granuloma at laser wavelengths at which Teflon has a maximal absorption. Twenty-four Teflon granulomas were created in 12 rats, and the gross and histologic effects from laser incision at four different wavelengths (8.25, 8.5, 8.75, and 10.6 microm) were evaluated acutely and at 7 and 14 days postoperatively. Polytetrafluoroethylene, or Teflon, is a relatively inert substance that has been used over the past 4 decades for endoscopic injection into the thyroarytenoid muscle of the lar

Clinical, morphologic, and morphometric findings are reported in 14 young Dalmatians with laryngeal paralysis. Neurologic signs, including megaesophagus, were observed in 13 of 14 dogs. Electromyographic abnormalities included fibrillation potentials and positive sharp waves in laryngeal, esophageal, facial, and distal appendicular muscles. Neurogenic atrophy was detected in intrinsic laryngeal and appendicular skeletal muscles. A diffuse, generalized polyneuropathy, dominated by axonal degeneration, was observed in recurrent laryngeal and appendicular peripheral nerves. Results of quantitativ

An 8-year-old Quarter Horse gelding was anesthetized for surgical exploration and debridement of a chronic draining wound in the intermandibular space. Anesthesia was without complication other than persistently low PaO2. Severe airway obstruction was evident immediately after extubation, requiring tracheostomy. Endoscopic diagnosis was bilateral arytenoid paralysis, which gradually resolved over the next 7 days. Compression, trauma, or tension of the recurrent laryngeal nerves are the postulated causes of idiopathic laryngeal hemiplegia in horses. The extremely extended position of the head a

A new technique is described that enables discrete activation of individual laryngeal muscles by electrical stimulation across overlying mucosa. In 15 dogs, we defined six distinct motor points by transmucosal stimulation at 3 mA while observing the resulting characteristic position of the arytenoid and true vocal cord. Five dogs were then paralyzed with succinylcholine in order to simulate bilateral vocal cord paralysis. Application of a 3-mA stimulus at each motor point yielded no motion of the cords, but when the current was increased to 20 mA, characteristic responses were elicited. In fiv

Laryngeal reinnervation procedures were performed on 15 dogs. An anastomosis was performed between a selected motor branch of the ansa hypoglossi and the posterior cricoarytenoid nerve. The results were analyzed endoscopically, electromyographically, and histologically. Results were satisfactory in 12 cases. The methods and results have been compared with those obtained using other reinnervation techniques.

With the use of a temperature sensor that would detect temperature changes during the respiratory cycle in the pharynx, electrical stimulation of the posterior cricoarytenoid muscle was achieved in dogs whose recurrent laryngeal nerves were artificially paralyzed. Due to the temperature difference between inspiratory and expiratory air flow, a trigger pulse was generated at the beginning of inspiration. The stimulation pulse following the trigger pulse stimulated the electrodes inserted into the posterior cricoarytenoid muscles. In all five canines, obvious abduction of the vocal folds synchro

The effect of left laryngeal hemiplegia on airway flow mechanics in 5 exercising horses was examined, and the efficacy of surgical repair by prosthetic laryngoplasty was evaluated. Measurements of the upper airway flow mechanics were made with horses on a treadmill (incline 6.38 degrees) while standing (period A); walking at 1.3 m/s (period B); trotting at 2.6 m/s (period C); trotting at 4.3 m/s (period D); and standing after exercise (period E). Experiments were done on healthy horses before any surgical manipulation (control), at 10 days after left recurrent laryngeal neurectomy, and at leas

A fully reliable means of rehabilitating patients with bilateral vocal cord paralysis has not yet been developed. In order to improve upon existing solutions to this problem, the authors have recently described a laryngeal pacemaker, initially tested through stimulation of a cross-over nerve-muscle pedicle from one sternohyoid muscle to the other in the canine. Afferent stimuli, initiated through elongation of the airway during inspiration, were detected by a linear strain gauge sutured to the tracheal rings and appropriately amplified. The current report deals with the application of this con

Arterial blood samples were collected during maximal exercise over 1.6 km in a thoroughbred horse with left laryngeal hemiplegia. Acid-base and blood gas measurements were performed on each sample and compared to the results from samples which were similarly collected 48 hours after laryngoplasty surgery was performed. Before surgery, the PaO2 was 53.2 mm Hg and the PaCO2 was 58.1 mm Hg after 1.6 km. After surgery, the corresponding results were 83.6 mm Hg (PaO2) and 39.0 mm Hg (PaCO2). There was no significant difference in the times taken for each gallop. The exercise intolerance associated

Contraction of the cricothyroideus muscles (CTMs), innervated by the superior laryngeal nerves (SLNs), modulates the voice by tilting the thyroid cartilage anteriorly onto the top of the cricoid and tensing the vocal cords. Either unilateral or bilateral paralysis of the SLNs is disabling for individuals with above-average voice demands. Some patients never compensate for this paralysis; there is no surgical procedure recognized to correct it. This study tested the hypothesis that surgical fusion of the thyroid and cricoid cartilages anteriorly can correct the problems of SLN injury by duplica

A three year old Thoroughbred filly was examined because of bilateral nasal discharge and external swelling of the left laryngeal area. Endoscopy revealed an enlarged left arytenoid cartilage, left laryngeal hemiplegia and drainage of purulent material into the lumen of the larynx. Radiographs showed a large fluid and gas filled cavity overlying the caudal larynx and cranial trachea. Surgical drainage and debridement of the abscess led to complete healing by secondary intention. Laryngeal ventriculectomy was performed as a treatment for left laryngeal hemiplegia, but a grave prognosis for resp
