Pathological characteristics of the different stages of Acanthamoeba keratitis.

AIMS: To classify the clinical stages of Acanthamoeba keratitis (AK), and clarify the relationship between pathological changes and clinical features. METHODS AND RESULTS: Between January 2007 and May 2012, AK was diagnosed in 11 eyes by pathological examination and confocal laser scanning microscopy. Pathological investigation of all cornea samples from keratoplasty was done with periodic acid-Schiff and haematoxylin and eosin stains. AK clinical stage, pathological features and postoperative treatment were studied retrospectively. The 11 cases were classified into development stage, convale…

Partial characterization of the proteolytic secretions of Acanthamoeba polyphaga.

The ocular pathology associated with acanthamoebiasis may result, at least in part, from the excretory and secretory (E&S) products of the active Acanthamoeba trophozoites. To test this hypothesis, the ability of A. polyphaga (ATCC Strain 30461) trophozoite E&S products to digest collagen, the major constituent of the corneal stroma, was evaluated. The secreted proteinases of A. polyphaga were identified using in vitro azocoll degradation, activity-PAGE, radiolabeled extracellular matrix (ECM) degradation, and collagen degradation assays. Inhibitors of serine (phenylmethylsulfonyl fluoride, di…

Infection of the central nervous system due to Acanthamoeba.

It is well established that Acanthamoeba castellanii, Acanthamoeba culbertsoni, Acanthamoeba polyphaga, and probably other species of free-living amebas are virulent opportunists capable of producing disease in humans and animals. Human infections involving brain, eyes, skin, and lungs have been reported from all continents. Central nervous system (CNS) infection due to Acanthamoeba species usually occurs in chronically ill, debilitated individuals, some of them receiving immunosuppressive therapy or taking broad-spectrum antibiotics. The disease runs a protracted, insidious clinical course an…

Peptide maps of the myosin isoenzymes of Acanthamoeba castellanii.

Extracts of Acanthamoeba castellanii contain four myosin-like ATPases (Maruta, H., Gadasi, H., Collins, J.H., and Korn, E.D. (1979) J. Biol. Chem. 254, 3624-3630): double-headed Acanthamoeba myosin II and single-headed Acanthamoeba myosins IA, IB, and IC, which have heavy chains of 170,000, 130,000, 125,000, and 130,000 daltons, respectively, as well as different light chains. In the accompanying paper, evidence is presented that suggests that Acanthamoeba myosin IC is the same molecule as Acanthamoeba myosin IA plus a regulatory 20,000-dalton peptide. This conclusion is confirmed by the ident…

Iris setosa Pall. ex Link Extract Reveals Amoebicidal Activity against Acanthamoeba castellanii and Acanthamoeba polyphaga with Low Toxicity to Human Corneal Cells.

Acanthamoeba keratitis (AK) is a sight-threatening and difficult-to-treat ocular infection. The significant side effects of current AK treatments highlight the urgent need to develop a safe and effective AK medication. In this study, the amoebicidal activity of Iris setosa Pall. ex Link extract (ISE) against Acanthamoeba was examined and its specific amoebicidal mechanism was explored. ISE induced significant morphological changes in Acanthamoeba trophozoites and exhibited amoebicidal activity against A. castellanii and A. polyphaga. ISE was further fractionated into five subfractions by seque…

Efficacy of phosphonium amphiphilic salts against Acanthamoeba genotype T4.

Acanthamoebae are opportunistic pathogens causing serious human infections, including granulomatous amoebic encephalitis (GAE) and Acanthamoeba keratitis (AK). The treatment of those infections is limited and difficult to date. Recent research demonstrated high antimicrobial activity of phosphonium amphiphilic salts. In the present work we aimed to investigate the anti-Acanthamoeba effect of a series of 16 phosphonium salts. The structure of these synthesized cationic amphiphiles was modified in both the polar and nonpolar parts of the surfactant molecule. The compounds have different alkyl ch…

Corynebacterium bovis Keratitis Mimicking Acanthamoeba Infection Following Ocular Trauma.

Corneal isolates of Corynebacterium species are frequently dismissed as contaminants, yet they are capable of causing severe, vision-threatening keratitis. A 45-year-old man with no history of contact lens use developed acute left eye pain and visual loss after a stone fragment struck his eye during work, with immediate contamination by garden hose water. Slit-lamp examination revealed a ring-shaped stromal infiltrate with visual acuity reduced to light perception, a presentation consistent with Acanthamoeba keratitis. Empiric therapy with fortified topical vancomycin and ceftazidime, antifung…

Cytomegalovirus Corneal Endotheliitis After Acanthamoeba Keratitis: A Case Report.

OBJECTIVE: This study aims to present a unique case of acanthamoeba keratitis (AK) with a delayed diagnosis and prolonged usage of glucocorticoid eye drops. Subsequently, the patient developed cytomegalovirus corneal endotheliitis (CMVCE), leading to severe ocular complications. To the best of our knowledge, this is the first documented case of confirmed CMVCE subsequent to AK. METHODS: A 32-year-old male patient with a history of contact lens usage and extended administration of glucocorticoid eye drops for keratitis underwent deep anterior lamellar keratoplasty (DALK) in the left eye. The d…

Chitosan nanoparticles improve the effectivity of miltefosine against Acanthamoeba.

BACKGROUND: Acanthamoeba keratitis (AK) is a corneal sight-threatening infection caused by the free-living amoebae of the genus Acanthamoeba. Early and appropriate treatment significantly impacts visual outcomes. Mucoadhesive polymers such as chitosan are a potential strategy to prolong the residence time and bioavailability of the encapsulated drugs in the cornea. Regarding the recent administration of miltefosine (MF) for treating resistant AK, in the present study, we synthesized miltefosine-loaded chitosan nanoparticles (MF-CS-NPs) and evaluated them against Acanthamoeba. METHODOLOGY/PRIN…

Human Conjunctival Transcriptome in Acanthamoeba Keratitis: An Exploratory Study.

PURPOSE: The purpose of this study was to identify conjunctival transcriptome differences in patients with Acanthamoeba keratitis compared with keratitis with no known associated pathogen. METHODS: The host conjunctival transcriptome of 9 patients with Acanthamoeba keratitis (AK) is compared with the host conjunctival transcriptome of 13 patients with pathogen-free keratitis. Culture and/or confocal confirmed Acanthamoeba in 8 of 9 participants with AK who underwent metagenomic RNA sequencing as the likely pathogen. Cultures were negative in all 13 cases where metagenomic RNA sequencing did n…

Elliptical deep anterior lamellar keratoplasty in severe Acanthamoeba keratitis.

Acanthamoeba keratitis (AK) is an intractable infection of the cornea. Penetrating keratoplasty is widely used for the management of severe AK but suffers from complications like graft rejection, endophthalmitis, and glaucoma. Herein, we aimed to describe the surgical technique and the results of elliptical deep anterior lamellar keratoplasty (eDALK) for the management of severe AK. In this retrospective case series, records of consecutive patients with AK poorly responsive to medical treatment who underwent eDALK from January 2012 to May 2020 were reviewed. The largest diameter of infiltratio…

Acanthamoeba royreba: morphological features and in vitro cytopathic effect.

Observations on cultured Acanthamoeba royreba trophozoites and in vitro cytopathogenicity of this amoeba are described. In culture, amoebae were active, pleomorphic and moved on the substrate by producing endocytic structures and emitting slight cytoplasmic microprojections from the cell surface. These projections were formed by hyaline cytoplasm and they were related to motion structures such as acanthopodia and lamellipodia, in which actin provides a framework that allows rapid changes in morphology. In the cytoplasm abundant vacuoles of different size and content were seen. By means of elec…

Listeria monocytogenes does not survive ingestion by Acanthamoeba polyphaga.

Listeria monocytogenes is a ubiquitous bacterium capable of infecting humans, particularly pregnant women and immunocompromised individuals. Although the intracellular invasion and pathogenesis of listeriosis in mammalian tissues has been well studied, little is known about the ecology of L. monocytogenes , and in particular the environmental reservoir for this bacterium has not been identified. This study used short-term co-culture at 15, 22 and 37 degrees C to examine the interaction of L. monocytogenes strains with Acanthamoeba polyphaga ACO12. Survival of L. monocytogenes cells phagocytose…
