Demographic and substance use factors related to violent and accidental injuries: results from an emergency room study.

OBJECTIVE: The primary goal of this study was to identify demographic and substance use factors associated with violent injuries, accidental injuries, and medical conditions or illnesses (non-injured). METHOD: Data were examined from a sample of 1701 admissions to emergency rooms at two Canadian hospitals. These patients were interviewed and provided urine samples to detect the presence of drug metabolites for alcohol, THC, benzodiazepines, barbiturates, morphine, and codeine. RESULTS: Those with violent injuries were significantly (P<0.0001) more likely to be male and have lower incomes com…

Accidental injury, self-injury, and assault among children of women with schizophrenia: a population-based cohort study.

OBJECTIVE: We aimed to compare the risk for injury overall and by intent (accidental injury, self-injury, and assault) among children born to women with versus without schizophrenia. METHODS: Using health administrative data from Ontario, Canada, children born from 2003 to 2017 to mothers with (n = 3769) and without (n = 1,830,054) schizophrenia diagnosed prior to their birth were compared on their risk for child injury, captured via emergency department, hospitalization, and vital statistics databases up to age 15 years. Cox proportional hazard models generated hazard ratios for time to firs

Blunt, Penetrating and Strangulating; Suicidal, Homicidal, Accidental and Sport-Related Neck Injuries: a Case Series and Literature Review.

INTRODUCTION: Presentation of neck injuries in ER can be with or without neurological deficit. Trauma victims with multiple injuries should be examined for neck injuries as these injuries are potentially life threatening. Further neck movement should be restricted by applying the cervical collar until further radiological investigations rule out the spine injury. Early identification and treatment of neck injuries whether spine, vascular, or muscular injury improve the morbidity and mortality in polytrauma patients. CASE PRESENTATION: In a series of case presentations of neck injuries through

Differentiating injury patterns and outcomes in accidental, suicidal and occupational falls from heights.

BACKGROUND: Falls from significant heights are a leading cause of severe injuries and can result from accidents, suicide attempts, or occupational incidents. While previous studies have explored general injury patterns associated with falls, little is known about the specific differences between accidental, suicidal, and occupational falls. This study aims to identify distinct injury patterns and outcomes among these fall types, thereby enhancing primary care and decision-making algorithms. METHODS: We conducted a retrospective analysis using data from the German TraumaRegister DGU® (2014-202

Fatal accidental wooden branch impalement mimicking homicidal sharp-force injury.

BACKGROUND: Penetrating neck injuries are usually caused by sharp-edged weapons, and their morphology often raises suspicion of homicidal sharp-force trauma. Accidental impalement by a rigid natural object, such as a wooden branch, is rare but may closely simulate a stab injury, complicating the determination of the manner of death. CASE PRESENTATION: A 30-year-old man was brought dead to the emergency department following a road traffic accident. External examination revealed a solitary spindle-shaped wound over the left supraclavicular region, with sharply split, gaping margins resembling a

Death Due to Accidental Crossbow Arrow Injury: Case Report and Review of the Literature.

Crossbows are weapons that shoot arrow-like projectiles using an elastic launching mechanism activated by a gun-like trigger. Accidental deaths due to crossbow injuries have been rarely reported in forensic practice. In this case report, a 40-year-old man was found dead in the woods with a wound to the upper torso. The wound was observed to have a corresponding slit-like defect in each layer of his clothing. A blood-stained crossbow with the safety features off was found at the scene, along with a nearby partially bent arrow. At autopsy, a hemorrhagic sharp-edged penetrating track was found in

Fatal craniofacial circular-saw-related injury: a rare case of accidental death.

Fatal injuries caused by circular saws are rare and mainly self-inflicted. To the best of our knowledge this is the first recorded case of accidental death of a woman caused by an injury to the head region inflicted by a saw blade. The autopsy showed a 35 cm long wound, stretching from the right half of the jaw along the right side of the face and the right temporal region up to the parietal region. The cut went through the right hemisphere of the brain in the temporal and parietal lobe regions, damaging the sagittal sinus. Air embolism was considered to be the cause of death. Fatalities invol

Accidental Transorbital Penetrating Brainstem and Carotid Injury with CB Antenna: A Case Report.

The authors present an unusual case of a transorbital penetrating injury of the internal carotid artery and brainstem. A young man accidentally ran into a 10-foot long citizens band (CB) antenna, and presented to the emergency department with nausea, dizziness and right periorbital ecchymosis. The nature and full extent of the injury were not appreciated at the time of initial examination and computed tomography scan. Magnetic resonance imaging subsequently demonstrated an unusual brainstem lesion. Neurology consultation was obtained. The neurologist and neuroradiologists reviewed the neurolog

Profile of non-accidental childhood injury at a tertiary hospital in south-west Nigeria.

BACKGROUND: Injury threatens children lives worldwide. Most studies from Nigeria have been on accidental injuries in children. OBJECTIVES: To study the profile of non-accidental injuries among children receiving care at a tertiary hospital in Nigeria. METHODS: Children with non-accidental injuries were serially recruited and studied. The Paediatrics Trauma Score (PTS) was tested against the outcome. RESULTS: Non-accidental injuries accounted for 0.84% of all 5264 patients and 21.3% of the 207 injured. The case fatality rate was 6.8%. Physical abuse, sexual assault, drowning/near drowning, g…

Heterotopic ossification of the hip after non-accidental injury in a child: case report.

Heterotopic ossification is bone formation that occurs in an abnormal soft tissue location, most commonly after trauma, head injury, and surgical procedures such as joint arthroplasty. We report a rare case of severe heterotopic ossification caused by nonaccidental injury in a 3-year-old child. To our knowledge, heterotopic ossification as a result of nonaccidental injury in children has been reported in only 2 other patients. Physicians should be aware of this rare presentation of nonaccidental injury, and nonaccidental injury should be considered in the differential diagnosis in children who

Identifying non-accidental injury in children presenting to A&E departments: an overview of the literature.

BACKGROUND: The need for an effective system of child protection, which is able to detect children at risk of physical abuse, has never been more urgent. The level of reporting of non-accidental injury (NAI) in young children is lower than would be expected from official statistics, which means that Accident and Emergency (A and E) nurses and clinicians have a special responsibility to improve detection of possible abuse, in what has become a very challenging area of medicine. AIMS: In this paper, we review the main issues facing clinicians and nurses in the identification of children at risk

Blunt carotid artery injury after accidental neck compression: report of a case.

Almost all cases of carotid artery injury are precipitated by a high-energy impact such as motor vehicle accidents or gunshot wounds, and are usually diagnosed using angiography. We report herein a case of carotid artery injury induced by a low-energy insult with rare clinical signs which was diagnosed using ultrasonography as well as angiography. A 37-year-old man sustained an accidental compression of the neck and was transferred to our emergency room. Horner's syndrome and phrenic nerve palsy were detected on the left side. Ultrasonography demonstrated two sites of injury with an intimal fl

Accidental contact burns of the upper limb in children with obstetric brachial plexus injury.

The denervated limb of the child with obstetrical brachial plexus injury is theoretically at risk of contact burns. A prospective study was designed to document the incidence and clinical presentation of these burn injuries in a series of 127 birth palsy cases. The study group included 90 Erb's and 37 total plexus palsy cases. Accidental burns occurred in 11% of total palsy cases and 1% of the Erb's palsy group. All injuries were contact burns and confined to the denervated limb. When the circumstances of the burn injury where investigated, it was concluded that injury to the denervated limb w

The long-term psychiatric consequences of accidental injury. A longitudinal study of 107 adults.

One hundred and seven accidentally injured adults were studied while in hospital and assessed prospectively twice more in a mean period of 28 months. The patients were studied by means of taped clinical interviews, including the Comprehensive Psychopathological Rating Scale (which includes the Montgomery-Asberg Depression Rating Scale), and several self-report measures of distress (Schedule of Recent Life Events, General Health Questionnaire, Impact of Event Scale and State Anxiety Inventory) at the three assessments. The total incidence of psychiatric disorders considered to be caused by the

Ehlers-Danlos syndrome type IV mimicking non-accidental injury in a child.

This case report describes a child in whom non-accidental injury was initially suspected, but who was later shown to have autosomal dominant Ehlers-Danlos syndrome (EDS) type IV. Her mother and brother were also type III collagen deficient. This syndrome may be commoner than the small number of reported cases suggested and may sometimes explain previously undiagnosed easy bruising. The importance of recognizing this syndrome is stressed as it may save a great deal of embarrassment, frustration and anger for the family wrongly suspected of causing injury to their child.
