Top Ten Tips Palliative Care Clinicians Should Know when Caring for Seriously Ill Patients with Stimulant Use Disorders.

Stimulant use disorder (StUD) is common in the hospice and palliative care (PC) setting, though guidance for clinicians remains limited. Stigma around stimulant use is widespread and can lead to impaired clinician/patient communication, poor symptom control, and lower quality of care. PC clinicians should be able to diagnose StUD, understand the potential pharmacologic and nonpharmacologic management options, engage with patients in a trauma-informed way, and refer to addiction medicine colleagues when appropriate. When utilizing opioids for pain management, PC clinicians should be aware of th…

Specialized training on addictions for physicians in the United States.

In the United States accredited residency programs in addiction exist only for psychiatrists specializing in addiction psychiatry (ADP); nonpsychiatrists seeking training in addiction medicine (ADM) can train in nonaccredited "fellowships," or can receive training in some ADP programs, only to not be granted a certificate of completion of accredited training. Information about ADP residency programs has been tabulated, but it is not available for ADM fellowships. The authors conducted a national survey to compile information about the location, structure, curriculum, and other characteristics …

An Unexpected Outcome of an Ischemic Stroke on the Reward Pathways in the Human Brain.

BACKGROUND: Addiction affects millions of people, often resulting from a complex interplay between genetic and environmental factors, and is frequently linked to mental health disorders. Many experts agree there is no cure for addiction, but there are effective treatments available. Many patients continue to succumb to addiction despite treatment. Therefore, more research is needed to develop new therapies in addiction medicine. CASE PRESENTATION: This is the case of a 63-year-old woman with an extensive smoking history who suffered an acute stroke affecting her left-sided basal ganglia, incl…

Ultra-high dilution medicines can modulate dopaminergic molecular targets and ΔFosB in the Nucleus accumbens preventing morphine reacquisition in rats.

Addiction is a recurring disease that constitutes a significant public health issue, particularly evident in the global opioid crisis involving substances like morphine (MORPH). Reestablishing to opioid use poses a major obstacle to successful detoxification treatment, as many individuals return to drug use after withdrawal. Therefore, given that drug addiction involves emotional, social, and physical factors, there is a necessity to explore integrative medicine approaches, including ultra-high dilution medicines such as isotherapic (ISO) and organotherapic (ORG) treatments. This integrative a

Recovery Capital Correlates With Less Methamphetamine Use and Crime in the Community.

OBJECTIVES: Reentry postcorrectional involvement is a high-risk time for patients with a history of addiction. We investigated whether participation in an addiction medicine clinic with active case management led to improvements in patients' recovery capital and whether there were associated changes in criminal activity and co-occurring methamphetamine or alcohol use. METHODS: Participants (n = 136) were patients with an opioid or stimulant use disorder who had Department of Corrections involvement in the preceding year, who completed the Assessment of Recovery Capital (ARC) and reported crim…

Comorbid pain and opioid addiction: psychosocial and pharmacological treatments.

Treating comorbid pain (nonmalignant) and opioid addiction is a complex endeavor that requires cooperation of multi-modal treatment teams incorporating pharmacological, psychological, and social components. There are multiple barriers for patients, mental health practitioners, and physical health practitioners to provide complete treatment for this difficult treatment population. In this article, we will review which treatments have been empirically validated in this treatment population, where further research is required, and considerations for potential "best approaches" to use for patient

Consequences of Opioid Abuse and their Treatments in Persian Medicine: A Review Study.

Since then, opium use and abuse has spread to all corners of the world, specifically the Eastern countries. This study aimed to investigate the consequences of opium use and their treatments according to Persian medicine. METHODS: In this narrative review, primary sources of Persian medicine and modern medicine databases of PubMed, Google Scholar, PsycINFO, the American Academy of Medical Sciences,

Precision Psychiatry and Dual Disorders.

OBJECTIVE: Addiction co-occurs with distinct pathological personality traits, other psychiatric disorders or symptoms and cognitive impairment, which are known as dual disorders or co-occurring disorders. This symptomatic high concurrency suggests that both conditions are in some ways causally linked. Research is ongoing to identify distinctive neurobehavioral mechanisms and endophenotypes that predispose individuals to compulsive drug use and other mental disorders. Research is also providing new revelations about the diverse effects of substances on individuals, including differences accordi

Outreach has emerged as an essential response to reach marginalised populations and remove barriers to health care. In the field of addiction medicine, this approach has been rethought to address the intertwined vulnerabilities and complex needs of people experiencing marginalisation and drug use. In this context, effective outreach is possible within a strong collaboration between health, social and security actors, and by fully integrating peer support, opening up important and innovative collaborations between these actors to adapt addiction policies and services to the realities on the gro…

The role and uptake of drug and alcohol services in patients with alcohol-associated acute pancreatitis at a large multisite health service.

BACKGROUND: Alcohol consumption remains an independent risk factor for recurrent presentations of alcohol-associated acute pancreatitis (AAP). Although there are recognised guidelines for alcohol use disorder (AUD), there are no recommendations on targeting alcohol cessation to reduce recurrence of AAP in guidelines for acute pancreatitis. AIMS: This study describes the uptake of the Addiction Medicine Unit (AMU), a specialist consultative service, in patients with AAP presenting to a tertiary hospital. METHODS: Retrospective cohort with all inpatients with AAP at a large multisite tertiary …
