Improving aftercare with technology for anorexia nervosa after intensive inpatient treatment: A pilot randomized controlled trial with a therapist-guided smartphone app.

OBJECTIVE: To evaluate the feasibility, acceptability, and preliminary efficacy of an innovative therapist-guided smartphone-based aftercare intervention following inpatient treatment of patients with severe anorexia nervosa (AN). METHOD: Forty female patients with AN (ICD-10: F50.0/F50.1) (aged: 15-36 years) were randomized either to an 8-week smartphone-based aftercare intervention (German version of "Recovery Record") with therapist feedback as an adjunct to treatment as usual (intervention group [IG]) or to treatment as usual alone (control group [CG]). Body mass index (BMI) and eating di…

Intensified return to work clinical aftercare for sick-listed employees with diagnosed mental disorders in Germany: effects of a multicentre randomized controlled trial.

BACKGROUND: Mental disorders are a leading cause of sickness absence and disability in Europe. They often follow chronic or recurrent patterns, hindering a sustainable return to work (RTW). In Germany, RTW aftercare interventions focusing on maintaining recovery and minimizing relapse among employees transitioning from psychiatric treatment to the workplace are scarce. We therefore developed an intensified RTW clinical aftercare program (RTW-PIA). This study aimed to (i) evaluate the effectiveness of RTW-PIA in addition to Care as Usual (CAU) on achieving sustainable RTW (SRTW) compared to CAU…

Beyond surviving: The mental and physical impairment of health-related quality of life after polytrauma emphasizes the need for interprofessional long-term aftercare.

BACKGROUND: Major trauma, particularly from road traffic accidents, is a leading cause of death and long-term disability in young adults worldwide. Advances in trauma center management, especially in Germany, have substantially improved survival rates to approximately 90%, which can be considered excellent. They often experience a reduced health-related quality of life (HrQoL) due to persistent physical, emotional, and mental challenges, underscoring the need for comprehensive long-term aftercare. This study focuses on the traumatic aftermath two years after polytrauma beyond the individual pa…

Impact of telephone prompts on the adherence to an Internet-based aftercare program for women with bulimia nervosa: A secondary analysis of data from a randomized controlled trial.

INTRODUCTION: Poor adherence is a common challenge in self-directed mental health interventions. Research findings indicate that telephone prompts may be useful to increase adherence. METHOD: Due to poor adherence in a randomized controlled trial evaluating an Internet-based aftercare program for women with bulimia nervosa we implemented regular short telephone prompts into the study protocol halfway through the trial period. Of the 126 women in the intervention group, the first 63 women were not prompted by telephone (unprompted group) and compared with 63 women who subsequently enrolled int…

Health-Related Complications during Follow-Up and Their Impact on Blood Cancer Survivors: Results from the "Aftercare in Blood Cancer Survivors" (ABC) Study.

INTRODUCTION: Blood cancer survivors are at increased risk for medical complications. METHODS: Our questionnaire-based study involved 1,551 blood cancer survivors with a ≥3-year interval since the last intense treatment. Its goal was to quantify health-related complications during follow-up and assess their impact on the patients' lives. RESULTS: A total of 20.4% of the responding survivors reported a disease relapse, most often in indolent lymphomas. Second primary malignancies occurred in 14.1%, primarily in lymphoma and allogeneic transplantation survivors. The most frequent malignancy wa

Evaluation of an internet-based aftercare program to improve vocational reintegration after inpatient medical rehabilitation: study protocol for a cluster-randomized controlled trial.

BACKGROUND: Mental disorders are the main reasons for rising proportions of premature pension in most high-income countries. Although inpatient medical rehabilitation has increasingly targeted work-related stress, there is still a lack of studies on the transfer of work-specific interventions into work contexts. Therefore, we plan to evaluate an online aftercare program aiming to improve vocational reintegration after medical rehabilitation. METHODS: Vocationally strained patients (n = 800) aged between 18 and 59 years with private internet access are recruited in psychosomatic, orthopedic an…

Statement of the AGO and AG-CPC on the Aftercare/Follow-up for Surgical Procedures of the Lower Genital Tract after the Introduction of a New Cancer Screening Guideline.

The new guideline on organized cancer screening programs has been in force in Germany since January 1st, 2020. The guideline has amended earlier recommendations on cytological examinations, which were previously carried out annually during screening. The guidelines-based recommendations on the appropriate follow-up for preinvasive and invasive lesions of the uterine cervix and endometrium are briefly outlined and differentiated from screening cytology and Pap/HPV co-testing as described in the guideline on organized cancer screening programs (oKFE-RL).

How Specialist Aftercare Impacts Long-Term Readmission Risks in Elderly Patients With Metabolic, Cardiac, and Chronic Obstructive Pulmonary Diseases: Cohort Study Using Administrative Data.

BACKGROUND: The health state of elderly patients is typically characterized by multiple co-occurring diseases requiring the involvement of several types of health care providers. OBJECTIVE: We aimed to quantify the benefit for multimorbid patients from seeking specialist care in terms of long-term readmission risks. METHODS: From an administrative database, we identified 225,238 elderly patients with 97 different diagnosis (ICD-10 codes) from hospital stays and contact with 13 medical specialties. For each diagnosis associated with the first hospital stay, we used multiple logistic regressio

Is Radiographic Aftercare Obsolete? How Testing Positive for ctDNA Can Be a Precedent for Late Relapse, Even in Low-Risk Hormone-Receptor-Positive Breast Cancer.

Current aftercare after early breast cancer overlooks recent evidence on circulating free tumor DNA (ctDNA). In this case report, we present a patient with low-risk hormone-receptor-positive breast cancer. ctDNA was first detected using a tumor-informed approach 12 months after the initial diagnosis and remained positive throughout adjuvant therapy with letrozole, while routine staging examinations showed no signs of relapse. Clinical relapse was ultimately identified nearly six years after the initial diagnosis, resulting in a lead time of four years and nine months. Current studies on ctDNA …

First experience in employing a complex digital support system accompanied by personal assistance to improve aftercare in patients with stroke or transient ischemic attack - results of the PostStroke-Manager feasibility study.

Stroke aftercare is widely acknowledged as crucial yet challenging. Digital tools offer a promising strategy to improve aftercare and, thus, patients' quality of life. However, it has not yet been investigated whether digital tools addressing different aspects of aftercare at the same time can be utilized in stroke patients. This study was intended to gain first experience in employing a complex patient-centered digital support system (i.e., the PostStroke-Manager) in individuals with stroke or transient ischemic attack (TIA) and thus examined its feasibility accompanied by personal assistance…

Making connections that work: partnerships between vocational rehabilitation and chemical dependency treatment programs.

One of the most exciting and frustrating times in the treatment process or a client in recovery is the period when that individual moves into aftercare. There are many challenges and obstacles to maintaining his or her clean/sober status, and support systems are key to a client s success in aftercare. Unfortunately, a group of professionals who can have a very strong impact on that success-- those in Vocational Rehabilitation (VR)-- are often left out of the system. Treatment and aftercare counselors may have a good understanding of many of the social services needed by clients who are transit…
