The changing surgical management of juvenile nasopharyngeal angiofibroma.

The management of juvenile nasopharyngeal angiofibroma (JNA) has changed during the last decades but it still continues to be a challenge for the multidisciplinary head and neck surgical team. The aim of this study was to review the used treatment approach and outcome of JNA in a single institution series of 27 patients diagnosed and treated during the years 1970-2009. All patients were male, with the median age of 17 years (range 11-33 years). Surgery was used as the primary treatment in every case. Surgical approaches varied, transpalatal approach (N = 14) being the most common approach used…

An unusual case of epistaxis-juvenile nasopharyngeal angiofibroma.

BACKGROUND: Epistaxis is a common complaint seen in the younger population. Its etiology is typically from the anterior nares, although we present a case of epistaxis from an uncommon source-juvenile nasopharyngeal angiofibroma. Juvenile nasopharyngeal angiofibroma (JNA) is a highly vascular, benign, yet locally invasive tumor that occurs in preadolescent males. OBJECTIVES: To illustrate a case of epistaxis from an uncommon source. CASE REPORT: A 21-year-old man with a past medical history of allergic rhinitis presented to the Emergency Department (ED) after 6 months of intermittent yet wors…

[Clinical case of diagnosis and treatment of extranasopharyngeal angiofibroma].

Extranasopharyngeal angiofibromas (ENGA) are benign fibrous vascular tumors that grow aggressively and are accompanied by destruction of the bones of the base of the skull and damage to nearby anatomical structures. The clinical manifestations of ENGA are rather nonspecific and largely depend on the localization (nasal septum, inferior nasal concha, upper jaw, etc.). The article presents a clinical case of endonasal endoscopic resection of ENGA with simultaneous X-ray-associated endovascular embolization of the tumor's supply vessels. The data of computed tomography with contrast, histological…

Extranasopharyngeal angiofibroma of the sinonasal tract: A systematic review.

PURPOSE: Extranasopharyngeal angiofibromas of the sinonasal tract (ENA-SNT) are exceptionally rare tumors that remain poorly characterized in the literature. We aim to systematically review and synthesize the clinical characteristics, management, and outcomes of ENA-SNT. METHODS: This review adhered to PRISMA guidelines. A literature search was conducted across major databases, including PubMed, EMBASE, Web of Science, and Google Scholar. Articles that reported ENA-SNT cases in humans were eligible for inclusion. Data extraction and quality assessment were independently performed by two revie…

The use of coblation technique for the debulking of juvenile angiofibroma.

In this paper, we describe how we successfully debulked a large juvenile nasopharyngeal angiofibroma (JNA) in a 15-year-old boy using the coblation technique. The child had been complaining about recurrent epistaxis and persistent nasal obstruction for several months. JNA is a rare benign tumor that predominantly affects adolescent and young adult males. JNAs exhibit gradual progression, local expansion, high vascularity, and a notable tendency for persistence and recurrence. Due to their high vascularity, JNAs require surgical removal, and treatment commonly includes preoperative embolization…

Massive cellular angiofibroma of the vulva: a case report.

Cellular angiofibroma is a rare benign mesenchymal tumor, typically occurring in the vulvar region of middle-aged women. This report highlights the importance of histological analysis in diagnosing this uncommon condition and emphasizes its benign nature and straightforward management. We present a case of a 58-year-old North African woman who had a large, well-defined mass in the left labia majora, which had been evolving over 2 years. MRI confirmed the resectability of the tumor by delineating its boundaries. The tumor, despite its slow growth leading to delayed diagnosis, was effectively tr…

Angiofibroma of the Nasal Cavity in a Dog: Case Report.

Nasopharyngeal angiofibroma is a histologic benign but locally aggressive vascular tumor described in humans. The description of this disease in dogs is limited. A mixed-breed female dog was presented with a history of epistaxis and nasal discharge. Rhinoscopy and computed tomography revealed a soft-tissue mass obstructing the entire nasal cavity, choana, maxillary recess, and right frontal sinus, but there was no bone or lymph node involvement. Histopathologic evaluation revealed irregular groups of blood vessels within a stroma of connective tissue, with secondary inflammation. Immunohistoch…

Endoscopic Endonasal and Transmaxillary Resection of a Nasopharyngeal Angiofibroma.

Juvenile nasopharyngeal angiofibromas (JNAs) are highly vascularized benign lesions that mainly arise in adolescent male patients. Current treatment options include observation, radiotherapy, or surgical resection. Surgical resection is the treatment of choice as it achieves immediate and complete results, thereby minimizing the risk of recurrence. Here we have reported on a transnasal-transmaxillary endoscopic approach for resection of a JNA in a 16-year-old male patient. The patient presented with 1 year of progressive nasal obstruction and intermittent episodes of right-sided epistaxis. Bec…

Reassessing the Anatomic Origin of the Juvenile Nasopharyngeal Angiofibroma.

OBJECTIVE: A modern imaging review is necessary to further define the anatomic origin of the juvenile nasopharyngeal angiofibroma. METHODS: After institutional review board approval, a search from January 1998 to January 2013 yielded 33 male patients (aged 10-23 years) with pathologically proven juvenile nasopharyngeal angiofibroma lesions, as well as pretreatment computed tomography/magnetic resonance imaging. Juvenile nasopharyngeal angiofibroma involvement was assessed in the following regions: sphenopalatine foramen, pterygopalatine fossa, vidian canal, nasopharynx, nasal cavity, sphenoid…

Topical sirolimus for the treatment of angiofibromas in tuberous sclerosis.

BACKGROUND: The skin is one of the most affected organs in tuberous sclerosis complex and angiofibromas are seen in almost 80% of such patients. These benign tumors impose a great psycho-social burden on patients. OBJECTIVE: The aim of the study was to evaluate the effectiveness and tolerability of topical sirolimus for facial angiofibromas in patients with tuberous sclerosis complex. METHODS: This was a prospective, single-blinded, cross-over study which involved twelve patients. We investigated the effect and safety of topical 0.1% sirolimus, which was obtained by crushing sirolimus tablet…

Microspheres embolization of juvenile nasopharyngeal angiofibroma in an adult.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma (JNA) is a benign though locally aggressive, highly vascular tumor primarily affecting adolescent males which has traditionally posed a significant intraoperative challenge during its resection due to the high risk of uncontrollable hemorrhage. Pre-operative angiographic embolization of the major feeding vessels to the tumor has become a valuable, even necessary, tool in the surgical treatment of these lesions. PRESENTATION OF CASE: Our patient was a 32-year-old man with a chief complaint of recurrent left-sided epistaxis for one year, brisk …

Bilateral vascular supply in juvenile nasopharyngeal angiofibromas.

OBJECTIVES: Juvenile nasopharyngeal angiofibroma (JNA) is a rare, benign tumor of the nasopharynx, exclusively affecting males in their teens and twenties. Historically, it has been thought that JNAs primarily receive their blood supply from the ipsilateral external carotid system. We demonstrate in our case series and literature review that bilateral vascular supply in these tumors is, in fact, very common. STUDY DESIGN: Case series and literature review. METHODS: We present four consecutive cases of JNA in a three-year period from a major tertiary care center. A literature review on the to…

[Extranasopharyngeal angiofibroma of the nasal cavity and paranasal sinuses].

BACKGROUND: Angiofibromas commonly arise in the nasopharynx in young male patients. Diagnosis is widely based on radiographic imaging to avoid ill-advised biopsy which may result in brisk bleeding. This study was undertaken to evaluate the incidence, clinical features and complications that may occur during the process of diagnosis and surgical therapy of angiofibromas outside the nasopharynx. METHODS AND PATIENTS: Case report of a 13-year-old female patient and review of the literature. RESULTS: Our patient received multi-agent chemotherapy elsewhere due to a misdiagnosed angiofibroma. Comp…

Extranasopharyngeal Angiofibroma of the Natural Ostium of Maxillary Sinus.

Extranasopharyngeal angiofibroma which occurs in all head and neck regions is extremely rare. Unlike most angiofibromas which show nasal congestion and recurrent epistaxis as their symptoms, extranasopharyngeal angiofibromas (ENAF) may lead to various symptoms depending on their location. Nasal septum is the most frequent site of origin of ENAF. No study of ENAF originating in natural ostium of maxillary sinus has been reported. We present a case of 27-year-old male who has extranasophar- yngeal angiofibroma arising from the natural ostium of maxillary sinus in an adult patient whose symptom w…

Extensive nasopharyngeal angiofibromas: the maxillary swing approach.

The objective of this study was to evaluate the efficacy and outcome using the maxillary swing approach for the management of extensive nasopharyngeal angiofibromas. A retrospective analysis in a tertiary care center revealed five cases with extensive nasal angiofibromas operated using the maxillary swing approach between 2010 and 2012. All patients had tumor extension to the lateral-most portions of the infratemporal fossa with complete occupation and destruction of the lateral wall of the sphenoid sinus causing abutment to the cavernous sinus and complete involvement of the pterygopalatine f…

Extranasopharyngeal angiofibroma of the nasal septum: a case report.

Angiofibroma arising outside the nasopharynx is unusual. The nasal septum is an extremely rare site for the origin of angiofibroma, and to date only 6 such cases previously have been reported in the literature. We report here a case of a 22-year-old man with a vascular mass arising from his nasal septum. The histopathology report was consistent with angiofibroma. We also review other cases in the literature of angiofibroma arising from the nasal septum and discuss a theory of the likely origin of angiofibroma.
