The anterior ankle impingement syndrome is a clinical pain syndrome that is characterized by anterior ankle pain on (hyper) dorsiflexion. The plain radiographs often are negative in patients who have anteromedial impingement. An oblique view is recommended in these patients. Arthroscopic excision of soft tissue overgrowths and osteophytes is an effective way of treating anterior impingement of the ankle in patients who have no narrowing of the joint space. For grade II lesions (osteophytes secondary to arthritis with joint space narrowing) arthroscopic treatment is a good option, because no ot

Ankle joint distraction is a viable alternative to ankle arthrodesis or ankle replacement. A congruent, painful, mobile, and arthritic ankle joint that is treated with this technique can achieve good to excellent results. Attention to the principles (anterior osteophyte resection, equinus contracture release, and ankle joint realignment procedures) is as important for a successful outcome as the accurate application of the hinged ankle joint distraction technique itself.

Corrective ankle osteotomies are periarticular osteotomies of the fibula, distal tibial metaphysis, or distal tibial metaphyseal diaphyseal junction. These osteotomies are indicated when angular, rotational, or translational malalignment is present. The author presents a review of the literature and in-depth surgical technique for each procedure, as well as a review of how to prevent and address the most common complications encountered.

Medial instability is suspected on the basis of a patient's ankle feeling like it is "giving way," especially medially, when walking on uneven ground, downhill, or down stairs, pain at the anteromedial aspect of the ankle, and sometimes pain in the lateral ankle, especially during dorsiflexion of the foot. A history of a chronically unstable feeling that is manifested by recurrent injuries with pain, tenderness, and sometimes bruising over the medial and lateral ligaments, is considered to indicate combined medial and lateral instability that is believed to result in rotational instability of

The term "ankle impingement" encompasses a broad range of conditions that are typically post-traumatic and often chronic. Various forms of mechanical impingement can result from synovial proliferation, bone spur formation, or ligamentous scarring and hypertrophy. Since symptoms and physical findings can mimic a variety of disorders, accurate diagnosis may remain elusive, and proper effective therapy may be delayed. The objective of this article is to define and elucidate the etiology of the various forms of ankle impingement, clarify the range of associated osseous and soft-tissue pathology, a

Until there is a total ankle implant developed that stands the test of time, ankle arthrodesis will continue to be the gold standard in the operative treatment of the arthritic ankle joint. Ankle arthrodesis techniques include minimally invasive methods that can be performed via arthroscopy or with miniarthrotomy. Extensile techniques include extra-articular fusions, the transfibular approach, removal of both malleoli, and the Blair fusion. An ankle arthrodesis can be fixated utilizing either internal or external fixation. The form of fixation chosen depends on availability and surgeon prefere

Complex ankle instability can be linked to a variety of causes. It is essential to evaluate these patients adequately, including a clinical history, evaluation of prior records and diagnostic studies (if available), and current radiographic studies. With appropriate evaluation and assessment, the underlying cause can [figure: see text] be determined and a reasonable surgical plan developed. With appropriate planning, adequate improvement usually can be expected.

PURPOSE: The ankle impingement syndrome depends on many factors (fiber or bone production changes) manifested with pain and limited range of movement of the tibiotarsal joint. We tried to classify the various causes and sites of impingement syndromes with MRI.
MATERIALS AND METHODS: A cohort of 42 selected patients underwent a 2-year orthopedic follow-up. All patients were examined with MRI using both a low field permanent dedicated magnet at 0.2 Tesla (Artoscan, Esaote Biomedica, Genoa, Italy) and a high field General Electric unit at 1.0 T; sequences and views were chosen according to the c

The ankle impingement syndrome is a frequent condition in both athletes and the normal population. We investigated this painful syndrome from both a clinical and a diagnostic viewpoint. Depending on what ankle tissue impinges on the other, it is possible to distinguish bone impingement, soft tissue impingement and peripheral nerve entrapment. For each of these pathologic conditions we investigated the diagnostic role of conventional radiography, Computed Tomography and Magnetic Resonance Imaging. The evidence of osteophytes, exostosis and presence of the os trigonum on plain films make clinica

Ankle arthropathy remains a frequent source of disability in the hemophilic patient. Bleeding into the ankle most commonly commences in the second decade, and, once established as a target joint, progressive damage leads to significant pain, stiffness, and deformity affecting general mobility and leisure and occupational pursuits. Review of cohorts of hemophilic patients at the Royal Free Hospital has monitored this deterioration and there is no evidence that, despite improvements in medical treatment, the pattern of insidious decline in hindfoot function has changed during the past 4 decades.

We present a case of soft tissue impingement in the posteromedial ankle, which has not been previously reported. When a patient presents with chronic posteromedial ankle pain and swelling, clinicians should be suspicious of soft tissue impingement in the posteromedial ankle. Pertinent history and physical examinations are most helpful in the diagnosis of this lesion. Arthroscopic debridement of the hypertrophic soft tissue is successful in returning the patient to his or her previous activities.

Six consecutive patients, four with failed infected ankle fusions and two with posttraumatic degenerative ankle arthritis, were treated by monofocal compression arthrodesis using the Ilizarov external fixator. The average age was 48 years (range, 33-74 years). The average preoperative duration after failed infected ankle fusion was 18 months and for posttraumatic arthritis after fracture, 23 months. Infected failed pseudarthroses had significant fixed valgus deformity of the foot, bilateral draining sinuses, and near complete destruction and loss of the talus. Compression was dynamically appli

Many choices are available to athletes seeking an ankle support. The time-honored tradition of ankle taping with adhesive tape does offer protection against ankle sprains during activity. Laced stabilizers offer an equal or possibly greater amount of support, are less costly and easier to apply, and can be retightened frequently during activity. The physician should become familiar with one of these two methods and choose one based on availability and feasibility in the community. The air stirrup may be indicated for patients with a history of ankle injury who are undergoing a graduated rehabi

BACKGROUND: Total ankle arthroplasty (TAA) and ankle arthrodesis (AAD) are the established surgical options for end-stage ankle arthritis.
PURPOSE: This study aimed to compare the two procedures and to investigate factors associated with postoperative outcomes, including ankle range of motion (ROM) measured on lateral radiographs. ROM was assessed as the motion of the foot relative to the lower leg and was defined as the apparent ankle ROM (AROM).
METHODS: We retrospectively analyzed data from 38 patients who underwent TAA (n=18) or AAD (n=20) between 2015 and 2023. Patients with a minimum o

Osteoarthritis is the most common joint disease-causing pain and disability, and its management keeps creating a debate. So, we aimed to compare the safety and efficacy of total ankle arthroplasty and ankle arthrodesis for ankle osteoarthritis. We searched PubMed, Cochrane, Scopus, and Web of Science till August 2021. The outcomes were pooled as Mean difference (MD) or Risk Ratio (RR), and 95% confidence interval. We included 36 studies. The results showed a significantly lower risk of infections in total ankle arthroplasty (TAA) than ankle arthrodesis (AA) (RR= 0.63, 95% CI [0.57, 0.70], p <

BACKGROUND: Joint-preserving procedures of the ankle may postpone the need for ankle arthrodesis (AA) or total ankle replacement (TAR). The challenge for the surgeon is to determine which patients may benefit from these joint-preserving procedures. We hypothesized that patents with less than 2 mm of ankle joint space on preoperative radiographs would report inferior outcomes following joint-preserving surgery compared with those with 2 mm or greater joint space.
METHODS: Patients 18 years of age or older treated with joint-preserving ankle surgery with a minimum of 2 years of follow-up were c
