The Brain Network of Naming: A Lesson from Primary Progressive Aphasia.

OBJECTIVE: Word finding depends on the processing of semantic and lexical information, and it involves an intermediate level for mapping semantic-to-lexical information which also subserves lexical-to-semantic mapping during word comprehension. However, the brain regions implementing these components are still controversial and have not been clarified via a comprehensive lesion model encompassing the whole range of language-related cortices. Primary progressive aphasia (PPA), for which anomia is thought to be the most common sign, provides such a model, but the exploration of cortical areas im…

Color Vision Deficits.

PURPOSE OF REVIEW: Color provides important information about the identity of the objects we encounter. After early processing stages in the … ntral occipito-temporal cortex. Evidence from the performance of a brain-damaged patient with color anomia but preserved color categorization demonstrated the independence of color categorization from color naming in the adult brain. Evidence from patients with brain damage suggests that high-level color processing may be divided into at least three func …

A case series comparison of semantically focused vs. phonologically focused cued naming treatment in aphasia.

There is a lack of clarity in the field regarding how to best predict which naming treatment will be most beneficial for a particular indivi … fferential effects across participants. This seemed to be influenced by factors such as severity of anomia, order of treatment presentation, and capacity limits. Clinical implications of these findings highlight the importance of expanding our picture of a participant's behaviours to consider what other important factors can inform intervention decisions …

[A 6-year-old girl who exhibited residual reading disorder during the course of acute encephalopathy].

We assessed a 6-year-old girl who developed status epilepticus and exhibited transient aphasia during the course of acute encephalopathy with late reduced diffusion, and who had a residual reading disorder in the recovery period. The aphasia appeared to be fluent aphasia and anomia, suggesting that the reading disorder during the recovery process was due to impairment of the phonological process. There were no biphasic seizures during the course of the patient's illness, but this case was acute encephalopathy with febrile convulsive status epilepticus (AEFCSE) from the standpoint of the charac…

Superadditive effects of multiple lesions in a connectionist architecture: implications for the neuropsychology of optic aphasia.

Neuropsychological disorders have traditionally been understood in terms of a focal lesion to a single component of a cognitive architecture. Optic aphasia (OA) defies explanation in this way. In OA, naming of visual stimuli is impaired in the absence of general visual agnosia or anomia. OA has been explained by positing multiple semantic systems or multiple functional pathways to visual naming. M. J. Farah (1990) instead sketched a parsimonious account based on multiple lesions--to pathways mapping visual input to semantic and semantics to naming responses--and the assumption that the effects…

Paroxystic neuropsychological symptoms as the early expression of hepatic encephalopathy. A case report.

A 52-year-old patient presented with paroxystic episodes of generalized apraxia, anomia, agraphia and acalculia. The transient character of these attacks was supported by several neuropsychological examinations. Initially a tentative diagnosis of multiple TIA's was made. Treatment consisted of antiplatelet aggregation therapy. Three years later, however, paroxystic neuropsychological symptomatology occurred more frequently with an increase of severity. The patient was again seen and the differential diagnosis included epilepsy or a metabolic disturbance, in casu hepatic encephalopathy. A thera…

Partial hemispheric disconnection syndrome of traumatic origin.

A patient presented left-sided apraxia, constructional disturbances with the right hand and left tactile anomia after a severe head injury. MRI showed a callosal lesion involving the middle and posterior third of the truncus. This finding supports the thesis that the posterior callosum plays a predominant role in the interhemispheric mediation for praxis, as well as in the right-left transfer of spatial information guiding constructional performance. Left-sided apraxia was evident on imitation, on verbal command and on visual presentation of an object, but not on tactual presentation and durin…

Case study of language and numerical disability: A sequential processing deficit?

We report a case study of Christine, an intelligent 30-year-old woman with a developmental learning disability. Psychometric evaluation and extensive interviews revealed several findings: most notably, evidence of anomia, auditory processing problems, difficulty acquiring reading and spelling skills, and an extremely poor sense of number. In addition, Christine showed all four of the Gerstmann symptoms (left-right confusion, finger agnosia, dysgraphia, and dyscalculia). We suggest that most, if not all, of these impairments may be explained in terms of a sequential processing deficit (e.g., Ba…

Dissociation between semantic and autobiographic memory: a case report.

We report a patient who showed a dissociation between impaired semantic memory and preserved autobiographic memory. M.N., a 56-year-old right-handed woman, developed a supra sellar meningioma and underwent multiple operations. Following Lineac radiation, necrosis occurred in the left temporal lobe. Magnetic Resonance Imaging revealed bilateral lesions in the temporal lobes and in the right basal frontal lobe. The patient's intellectual deterioration and anterograde amnesia were mild. Language was preserved, but for a subtle anomia. Autobiographic memory remained intact, while semantic memory f…

The continuum of deep/surface dyslexia.

A right-handed male sustained traumatic brain injury which resulted in anomia, dyslexia and agraphia. The most severe CT (computed tomography)-identified brain damage was located in the right parieto-temporal lobe. In the first months following the injury, the pattern of reading errors was similar to that associated with deep dyslexia. However, nonlexical derivation of phonology from print was not abolished. As the patient's ability to associate letter patterns with sounds improved, oral reading also improved. Although he no longer produced semantic errors in oral reading, he continued to prod…

Sniffing behaviour, or recognizing a lily by smell, but not recognizing a sock on sight.

We report a 65-year-old man with a post-anoxic encephalopathy who showed compulsive sniffing at available objects. This stereotyped environment-driven behaviour has not been previously described. Other compulsive environment-driven responses, such as manipulation and utilization of tools and hyperlexia, were also present. The disorder shared several features with the Klüver-Bucy syndrome where mouthing of objects, rather than smelling them, is common. The patient had a severe dementia, with amnesia, anomia, apraxia, and visual agnosia. Whereas he could not recognize very familiar objects on si…

Loss of visual imagery and loss of visual knowledge--a case study.

As a sequel of a left posterior cerebral artery infarction a patient had severely defective mental imagery of shapes and colours of objects. Imagery of faces, letters, and topological relationships was preserved. The impairment of imagery of object colours was associated with colour agnosia and colour anomia. For colours, there was no difference between performance on tasks calling for imagery of object colours and tasks affording a distinction between correctly and incorrectly coloured objects. For shapes of objects, imagery appeared to be below the level of the patient's knowledge about the …

[The localizing value of speech disorders in transitory cerebral ischemia].

Clinical and angiographic findings in 30 patients who had suffered from a dysphasic disorder in the course of transient cerebral ischaemia w … ent to decide whether the dysphasia is due to aphasic syndrome of middle cerebral artery origin, to anomia or to dysarthria. Thus, transient dysphasia appears to be a symptom of little localizing value.

Severe dysphagia in lower cranial nerve involvement as the initial symptom of Wegener's granulomatosis.

We observed a 42-year-old woman presenting with severe dysphagia secondary to paralysis of the lower cranial nerves and right phrenic nerve involvement, followed by respiratory failure. An EMG confirmed bilateral denervation of the 9th, 10th, 11th and 12th cranial nerves and right phrenic nerve. Videolaryngoscopy showed bilateral vocal fold immobility. Anemia, elevated ESR, microhematuria and C-ANCA (PR-3) antibodies were detected. Brain MRI and CSF were normal. A chest CT showed bilateral, irregular pulmonary lesions. An 18F-FDG total body scan showed diffuse hypermetabolic regions in both pu

Naming disorders anomic aphasia.

Naming disorders, as a symptom, is always found in all aphasic patient. The patient although has a clear understanding of what he is trying to name or what he is told to write, but needs longer time or can not articulate it. Sometimes he substitutes with other words or even produces jargon words. Assessment and intervention are complicate and delicated the most important functions of the clinician. The goal of word-retrieval activities in treatment is to improve the patient's cognitive performance and to teach method to circumvent the vocabularly when needed.
