Should we offer antibiotic prophylaxis post sexual assault?

Our objective was to assess whether antibiotic prophylaxis should be offered to women post sexual assault by considering acceptability of prophylaxis, follow up attendance rates and the prevalence of sexually transmitted infections (STIs) in these women. Retrospective case notes review of female survivors of rape or sexual assault attending the Rose Clinic, Ambrose King Centre, Royal London Hospital between 1 January 1997 and 31 May 1999 was carried out. The following selection criteria were applied: age greater than 16 years; attending within two weeks of assault; having experienced vaginal a…

Antibiotic prophylaxis and treatment of surgical abdominal sepsis.

The incidence of surgical abdominal sepsis is related to the operation, patient and skill of the surgeon, ranging from <2-3% for laparoscopic cholecystectomy to >35-40% in overt peritonitis. Aged, obese, diabetic, neoplastic, acute patients have the highest incidence of sepsis. Antibiotic prophylaxis significantly reduces the incidence of postoperative infections for Class II and II operations. The proper timing (30-60 min before incision), choice of antibiotic (related to possible pathogens) and correct duration are essential. Ultra-short prophylaxis (only one administration) may be effective…

Antibiotic prophylaxis for appendectomy in children: critical appraisal.

Prophylactic antibiotics are widely used during surgery, above all for clean-contaminated or contaminated procedures. Because factors other than the degree of contamination play a critical role in regard to postoperative infectious complications, it is not evident that recommendations for antibiotic prophylaxis for an adult population can be transferred to children. The aim of the study was to analyze the incidence of postoperative infections in children undergoing appendectomy for suspected appendicitis to evaluate critically the effectiveness of prophylaxis. The charts of all children underg…

[Antibiotic prophylaxis. Single-dose piperacillin in abdominal hysterectomy].

Several studies have confirmed the effectiveness of antibiotic prophylaxis in gynecological surgery. We prospectively evaluated 181 women undergoing abdominal hysterectomy, randomly divided in two groups. One of them received 2 g of piperacillin 30 minutes before the operation and the other 4 g of the same group with the same interval. The effectiveness of both prophylactic piperacillin regimens for the prevention of infective postoperative morbidity was compared. Statistically significant differences between the two prophylactic regimens were not found in the analysis of the results.

Antibiotic prophylaxis of experimental endocarditis after dental extractions.

In rats with catheter-induced sterile aortic valve vegetations we studied the efficacy of single-dose amoxicillin and single-dose erythromycin prophylaxis for the prevention of bacterial endocarditis after extractions of periodontally diseased teeth. Endocarditis after extractions occurred in 89% of control animals and was due to group G streptococci, to Staphylococcus aureus, or to both organisms. A single-dose of amoxicillin or erythromycin successfully prevented endocarditis due to these bacterial species. The analysis of the bacteremia (by culturing blood drawn 1 min after extraction on pe

Survey of antibiotic prophylaxis in British urological practice.

Guidelines for the use of prophylactic antibiotics to cover operative procedures on the urinary tract have been suggested, but the extent to which these have been adopted into routine practice has not been assessed. This survey documents the use of prophylactic antibiotics in urological surgery in Britain. It demonstrates that awareness of the potential role of antibiotic prophylaxis is increasing. Most urologists now adopt a selective approach in the use of prophylactic agents.

To give or not give antibiotic prophylaxis for planned caesarean section: An assessment of risk of postpartum prescription of antibiotics depending on prophylaxis regimen.

INTRODUCTION: No national recommendation for antibiotic prophylaxis for planned caesarean section in Sweden exists. Swedish delivery units have chosen different strategies. Some units routinely administer antibiotic prophylaxis to all women for whom a caesarean section is planned, while other units give prophylactic antibiotics to risk groups only. We took advantage of this "natural experiment" to compare the risk of antibiotic prescription postpartum after planned caesarean section, by antibiotic prophylaxis strategy. MATERIALS AND METHODS: An observational study based on data from Swedish r…

Multicentre randomised double-blind placebo controlled trial of combination vancomycin and cefazolin surgical antibiotic prophylaxis: the Australian surgical antibiotic prophylaxis (ASAP) trial.

INTRODUCTION: Resistant Gram-positive organisms, such as methicillin-resistant staphylococci, account for a significant proportion of infections following joint replacement surgery. Current surgical antimicrobial prophylaxis guidelines recommend the use of first-generation or second-generation cephalosporin antibiotics, such as cefazolin. Cefazolin, however, does not prevent infections due to these resistant organisms; therefore, new prevention strategies need to be examined. One proposed strategy is to combine a glycopeptide antibiotic with cefazolin for prophylaxis. The clinical benefit and

Antibiotic prophylaxis in orthopaedic surgery: difficult decisions in an era of evolving antibiotic resistance.

Prophylactic antibiotics can decrease the risk of wound infection and have been routinely employed in orthopaedic surgery for decades. Despite their widespread use, questions still surround the selection of antibiotics for prophylaxis, timing and duration of administration. The health economic costs associated with wound infections are significant, and the judicious but appropriate use of antibiotics can reduce this risk. This review examines the evidence behind commonly debated topics in antibiotic prophylaxis and highlights the uses and advantages of some commonly used antibiotics. Cite this…

Antibiotic prophylaxis in surgery for closed fracture of the hand.

PURPOSE: Preoperative antibiotic prophylaxis is associated to internal fixation for closed phalangeal and metacarpal fracture, but its effectiveness is not known. METHODS: In a consecutive series of 119 adult patients undergoing Kirschner-wire fixation for phalangeal or metacarpal fracture, 56.3% (n = 67) received antibiotic prophylaxis and 43.7% (n = 52) did not. RESULTS: The rate of deep surgical site infection was 1.5% (n = 1) in the group with antibiotic prophylaxis and 1.9% (n = 1) in the group without. Minor skin irritation or infection of the pin tract occurred in 13.4% of cases (n = …

Duration of Antibiotic Prophylaxis for Ballistic Fractures to the Extremities.

BACKGROUND: In the U.S. there are thousands of ballistic firearm injuries to the extremities and subsequent infections, yearly. There is a lack of consensus regarding the ideal duration of antibiotic treatment to prevent infection of these wounds. Our study investigated infection rate among ballistic extremity fracture patients based on antibiotic prophylaxis duration, operative management, wound severity, and fracture location. MATERIALS AND METHODS: Retrospective chart review of ballistic extremity fracture patients from a single trauma center from 01/01/2010 to 12/31/2020. RESULTS: Of 161…

Ocular toxoplasmosis: background and evidence for an antibiotic prophylaxis.

PURPOSE OF REVIEW: The purpose of this review was to provide an overview of current data on antibiotic prophylaxis in ocular toxoplasmosis. RECENT FINDINGS: Studies showing the prophylactic effect of long-term antibiotics are discussed. Prophylaxis seems to be justified in patients with a high risk of recurrence because of antibiotic's potential side-effects. Therefore, predisposing factors leading to a higher risk of recurrence and the time period during which an antibiotic prophylaxis is most appropriate are reviewed. Finally, a patient-individualized treatment recommendation is summarized.…

Antibiotic prophylaxis in obstetric and gynaecological procedures: a review.

Surgical site infections are a common complication of obstetric and gynaecological surgeries; up to 10% of gynaecological patients undergoing an operative procedure will develop a surgical site infection. In surgeries with high rates of post-operative infection, antibiotic prophylaxis (using an antibiotic with an appropriate microbiological spectrum and administered in a timely manner) can play a major role in improving outcomes. This review examines the medical literature to assess the indications and appropriate antibiotic choices for prophylaxis to prevent surgical site infection in obstetr…
