Levosimendan as an Antidote in Experimental Calcium Channel Blocker Intoxication.

The effects of the calcium sensitizer levosimendan on hemodynamics and survival in guinea pigs intoxicated with the calcium blockers verapamil or diltiazem were evaluated in a randomized controlled study. One hundred four animals were randomized to be intoxicated with either verapamil (2.0 mg/kg) or diltiazem (4.5 mg/kg) and thereafter further randomized into 6 groups which received either saline (control), 3 different regimes of levosimendan, calcium chloride, and levosimendan combined with calcium chloride. The hemodynamics and survival of the animals were followed for 60 minutes after intox

Commentary: Teaching expertise is the best antidote for educational inequities.

This article discusses teaching expertise as the best antidote for educational inequities. Economically disadvantaged and minority students are over-represented in the population of those experiencing reading difficulties, and therefore the societal scourges of systemic racism and persistent economic inequalities are to blame. Closer examination of these variables, however, suggests that minority students attend under-resourced schools in greater numbers than white students and schooling itself predicts outcomes more than race alone. In other words, students who attend poor schools in unsuppor

Liposome-encapsulated methemoglobin as an antidote against cyanide poisoning.

Cyanide induces acute lethal poisoning resulting from inhibition of cytochrome c oxidase located in the complex IV (Complex IV) of mitochondria. However, current therapies for cyanide poisoning using hydroxocobalamin and nitrous acid compounds remain a clinical issue. Here, we show that liposome-encapsulated methemoglobin (metHb@Lipo), nanosized biomimetic red blood cells, replicate the antidotal mechanism of nitrous acid compounds against cyanide poisoning, achieving superior efficacy and fast action with no adverse effects. The structure of metHb@Lipo, which consists of concentrated methemog

That's a Phat Antidote: Intravenous Fat Emulsions and Toxicological Emergencies.

Health care providers in the emergency department (ED) frequently find themselves caring for patients who may have overdosed on a medication(s) or other toxic substance. These patients can prove to be a challenge, as providers must try to determine the substance(s) involved so that the appropriate treatment can be initiated. For those patients who are hemodynamically unstable upon presentation, it is important to note that supportive care is of the utmost importance, as there are few substances that have antidotes available. In these situations, lipid emulsion can be considered. This is especi

Intralipid emulsion treatment as an antidote in lipophilic drug intoxications.

Intravenous lipid emulsion (ILE) is a lifesaving treatment of lipophilic drug intoxications. Not only does ILE have demonstrable efficacy as an antidote to local anesthetic toxicity, it is also effective in lipophilic drug intoxications. Our case series involved 10 patients with ingestion of different types of lipophilic drugs. Intravenous lipid emulsion treatment improved Glasgow Coma Scale or blood pressure and pulse rate or both according to the drug type. Complications were observed in 2 patients (minimal change pancreatitis and probable ILE treatment-related fat infiltration in lungs). In

Time-resolved luminescence in conducting polymer/antidot nanocomposites.

We report modification of the structure and properties of conjugated polymers through controlled embedding wide-gap nanocrystals (antidots) within the polymer matrix. Investigations were carried out by means of stationary and time-resolved photoluminescence. Antidots strongly modify the luminescence spectrum of poly(p-phenylene vinylene) but have almost no influence on the spectrum of poly(2-(6-cyano-6'-methylheptyloxy)-1, 4-phenylene. We explain this observation with respect to the different chain structure and electron density distribution in these two materials. The temporal evolution of lu

[A specific antidote for the treatment of digitalis poisoning in uremic patients].

Two chronic haemodialyzed patients with digitalis intoxication are reported. One of them took digoxin 0.25 mg three times daily for an unknown period and the other took digitoxin 0.1 mg twice daily for two weeks. The symptoms of intoxication were mainly concealed by uremic syndrome. The diagnosis was established by noticed sinus bradycardia, first- and second-degree atrioventricular block in ECG and the determination of sera levels of glycosides (serum digoxin concentration was 7.36 ng/ml, serum digitoxin concentration was 46.5 ng/ml) in both cases. Modification of cyanide toxicodynamics: mechanistic based antidote development.

Cyanide's actions are complex and cannot be attributed solely to inhibition of oxygen utilization. Recent mechanistic studies show that cyanide inhibits multiple enzymes and alters several vital intracellular processes to produce the intoxication syndrome. By understanding the intracellular targets and the mechanisms underlying the toxicity, it is proposed that more effective antidotal regimens can be achieved. A mechanistic based, multi-step in vitro model was developed for screening potential cyanide antidotes. A series of compounds was screened for their ability to reverse the effect of cya

Time to discontinue the use of solutions A and B as a cyanide 'antidote'.

Solutions A and B (15.8% ferrous sulfate in 0.3% citric acid and 6% sodium carbonate, respectively) have been available as a first-aid treatment for cyanide ingestion for many decades. Controversy surrounding the efficacy of solutions A and B has existed for much of that time, the main protagonists being in the UK. The current opinion in the UK is that solutions A and B should no longer be used as a first-aid measure in the management of cyanide poisoning. Similarly, oral sodium thiosulfate or activated charcoal should not be used. The recommended first-aid treatment of symptomatic cyanide poi

CAR-T and checkpoint inhibitors: toxicities and antidotes in the emergency department.

INTRODUCTION: New cancer treatments with immunotherapy have led to unique toxicities affecting cancer patients. As cancer-related visits to the emergency department increase, the emergency physician and the medical toxicologist should be aware of immunotherapy-related toxicities. In this review we discuss immune related adverse events (irAEs) from chimeric antigen receptor T (CAR-T) cell therapy and immune checkpoint inhibitors (ICI). DISCUSSION: While presentation of the irAEs may mimic common conditions, it is important to recognize them as they may be life-threatening. A thorough history a

Antidepressant-related erectile dysfunction: management via avoidance, switching antidepressants, antidotes, and adaptation.

The ideal antidepressant would control depression with no adverse effect on sexual function. Erectile dysfunction and other sexual dysfunction associated with antidepressant medication treatment are problems with many antidepressants and can lead to patient dissatisfaction and decreased compliance with treatment. A computerized MEDLINE search (English language, 1966-2003) was performed using the terms antidepressive agents, erectile dysfunction, and sexual dysfunction. Emphasis was placed on studies with specific sexual function measurements taken before and after treatment and placebo control

[Do antidotes have a future? The example of 4-methylpyrazole and hydroxocobalamin].

In all probability the future progress in the "molecular" approach of therapy in acute poisoning within the next 20 years or so will be more rapid than the "mechanistic" approach, which undoubtedly has brought considerable improvements in the overall survival of these patients in the past. Principally molecular antidotes can act on three different levels, a) prior to the cellular targets (e.g. through modifications of toxokinetics), b) at specific cellular receptors through competition for binding (e.g. naloxone and flumazenil), and c) after the receptor sites through unspecific counteraction

Effect of cyanide antidotes on the metabolic conversion of cyanide to thiocyanate.

The excretion of thiocyanate following the administration of equitoxic doses of cyanide to unprotected mice and to animals pretreated with various cyanide antidotes has been studied. The results demonstrate that cyanide given alone or to animals pretreated with thiosulfate is extensively converted to thiocyanate. Animals pretreated with sodium nitrite or a combination of nitrite and sodium thiosulfate excreted even higher amounts of thiocyanate. This demonstrates that cyanide originally detoxified by combination with methemoglobin is ultimately converted to thiocyanate in the animal body. Pret

High dose insulin is an inodilator, not an antidote in the poisoned patient!

High dose insulin therapy has become an increasingly popular treatment for poisonings over the last two decades. It is often mistakenly considered an antidote for poisonings of multiple drug classes, including beta-blocker and calcium channel blocker overdose. This misconception has encouraged overzealous and, at times, inappropriate use, particularly in patients who have vasoplegic shock following poisonings from drugs such as dihydropyridine calcium channel blockers. High dose insulin is not an antidote, but rather an inodilator. Its relatively slow onset of action, compared to catecholamine

Liposomal methemoglobin as a potent antidote for hydrogen sulfide poisoning.

Hydrogen sulfide (H2S) induces acute and lethal toxicity at high concentrations. However, no specific antidotes for H2S poisoning have been approved. Liposomal methemoglobin (metHb@Lipo) was developed as an antidote for cyanide poisoning. As the toxic mechanism of H2S poisoning is the same as that of cyanide poisoning, metHb@Lipo could potentially be used as an antidote for H2S poisoning. In this study, we evaluated the antidotal efficacy of metHb@Lipo against H2S poisoning. Stopped-flow rapid-scan spectrophotometry clearly showed that metHb@Lipo scavenged H2S rapidly. Additionally, metHb@Lipo
