Aphakia and further malformations of both eyes were diagnosed in a female German Holstein calf. Besides aphakia of the left eye, the calf exhibited microphthalmia, glaucoma and a hypoplastic uveoscleral tissue. Additional findings in the right eye were buphthalmus and glaucoma. Instead of aphakia, pathohistological and investigations revealed a very small (microphakia) and luxated lens. Neither the clinical nor the pathological examination revealed further malformations of other organs. A BVD infection could be excluded as cause for the ocular malformations observed. A deficiency or excess of …

We reviewed the cases of 21 patients (22 eyes) who underwent epikeratophakia for aphakia with at least 4 months' follow-up. Thirteen had traumatic cataracts, six had relative contraindications for implantation of an intraocular lens (IOL), one had a dislocated IOL and one had bilateral congenital cataracts. The mean best corrected visual acuity was 20/32.0 (range 20/20 to 20/70) before surgery and 20/38.6 (range 20/20 to 20/70) after surgery. The uncorrected visual acuity improved in all eyes. Twenty eyes (91%) were within two lines of the preoperative best corrected acuity; all eyes were with…

Epikeratoplasty is a form of refractive corneal surgery in which a lens made of human corneal tissue is sutured onto the deepithelialized surface of the patient's cornea. The indications of epikeratoplasty for the correction of aphakia and keratoconus are discussed. We also report the results of epikeratophakia for the correction of aphakia in eight eyes and for the treatment of keratoconus in five eyes.

Epikeratophakia is a new refractive corneal surgery since 1980. The authors treated 30 cases of unilateral aphakia with corneal lenses prepared by a self-made cryolathe. After a mean postoperative follow-up of 161 days, all patients obtained visual improvement. 19 eyes (63.3%) achieved uncorrected visual acuity over 0.5, and 5 eyes (16.7%) were 1.0 or over. 8 eyes (26.7%) were corrected to within 1 diopter by the surgery, and the mean postoperative corneal astigmatism was 2.53 diopters. The complications included over or under correction of the refractive error, interlamellar epithelial ingrow…

Examination of the contrast sensitivity in aphakic eyes corrected by glasses and in artephakias provides information which cannot be obtained by measurement of visual acuity and is thus a suitable supplement of the basic test of visual functions. Contrast sensitivity declines significantly with advancing age. Contrast sensitivity of aphakic eyes corrected by glasses corresponds to values of contrast sensitivity of phakic eyes of the same age group. Artephakias with an iris clip lens have a contrast sensitivity reduced by cca one half as compared with a control group, while in artephakias with

A series of 67 cases of epikeratophakia is presented with an average time from surgery of 12.2 months. For aphakia there was a delay in the recovery of vision, but by nine months 83% of 57 patients achieved an acuity equal to, or within 1 line of, the preoperative value. 57% were corrected to within 3 dioptres of emmetropia, but in the latter part of the series 75% were within this range. Astigmatism and reduced contrast sensitivity, especially in the presence of glare, were important complications. For keratoconus, 86% of seven patients with over two months of follow-up achieved a spectacle c…

The frequency and polarity of secondary strabismus was related retrospectively to the onset of deprivation in 131 children with mono- and bilateral aphakia after congenital cataract or perforating injury. The frequency was highest in patients up to 2 years of age at the onset of deprivation. From the 3rd year on, it declined to 50% or less. Esotropia was predominant in the first 2 years of life. At the end of the first decade of life exotropia was up to 80%.

Although the majority of aphakic patients will have intraocular lens implants (IOLs), there are several groups of patients for whom IOLs are not indicated. Synthetic keratophakia for the correction of aphakia has several inherent advantages over alternative forms of refractive surgery which use lathed corneal tissue. For example, the ability to produce a synthetic lens to precise specifications before surgery is a dramatic advantage over the use of lathed tissue lenses whose refractive properties cannot be determined until weeks or months after the lens is in place on the recipient cornea. Syn…

Contrast sensitivity was measured in pseudophakic and aphakic patients. No difference could be demonstrated between the contrast sensitivities of patients with anterior chamber/iris-supported and posterior chamber intraocular lenses (IOLs). Little difference was found between pseudophakics and aphakics. Interocular contrast ratios in five pseudophakic patients with normal fellow eyes revealed a range of contrast attenuation characteristics which may be attributed to the IOL and corneal and posterior capsule transmission.

Epikeratophakia is a simple and reversible form of refractive surgery for the correction of aphakia and myopia and for the treatment of keratoconus, perforation and pterygium. The tissue lenses are attached to the de-epithelialised cornea; no invasion of the central optical zone is required. The safety of this procedure makes it a potential solution to the problem of optical correction in infants and children with congenital and traumatic cataracts. With the advent of commercially prepared tissue lenses, this surgery should become accessible to many ophthalmic surgeons. In the future, the comb…
