The effect of obstructive sleep apnea surgery on laryngopharyngeal reflux with obstructive sleep apnea.

Background: Obstructive sleep apnea (OSA) may play a role in the occurrence of laryngopharyngeal reflux (LPR). OSA surgery also may have a significant curative effect on LPR with OSA.Objectives: To analyze the role of OSA in LPR and the effect of OSA surgery on LPR with OSA.Methods: Forty-five OSA patients with LPR symptoms were enrolled and were divided into the LPR positive group (RSI > 13) and the LPR suspicious group (1 ≤ RSI ≤ 13) based on reflux symptom index (RSI). All the patients underwent OSA surgery. The RSI scores at three different time points postoperatively were compared with th…

A fatty meal aggravates apnea and increases sleep in patients with obstructive sleep apnea.

PURPOSE: The purpose of this study was to investigate the role of a fatty meal before bedtime, on sleep characteristics and blood pressure in patients with obstructive sleep apnea (OSA). METHODS: Recently diagnosed, by full polysomnography (PSG), patients with OSA (n=19) were included. These underwent PSG for additional two consecutive nights. Two hours before the PSG examination, a ham and cheese sandwich of 360 kcal was served to all patients, at first night, while a fatty meal of 1,800 kcal was served before the second PSG examination. Comparisons were performed between the last two examin…

[Comparison of portable apnea monitors in the detection of apnea episode during daytime rest].

Although polysomnography (PSG) is the golden standard for the diagnosis of sleep apnea syndrome (SAS), access to this procedure is limited because it requires special institution and trained technicians. Therefore, many portable recording devices have been developed for detection of SAS including home monitoring. The present study evaluated the usefulness of four portable devices in detecting apneic events. The four devices are, (1) FM-500 thermister sensor type III device, (2) LS-300 pressure sensor type III device, (3) Morpheus pressure sensor type III device, and (4) SD-101, a sheet-type ty…

Signs and symptoms of sleep apnea and acute stroke severity: is sleep apnea neuroprotective?

BACKGROUND: In animal models, brief periods of hypoxemia render the brain tolerant to subsequent ischemic insults. Sleep apnea leads to frequent episodes of nocturnal hypoxemia and may induce ischemic tolerance. Snoring and daytime sleepiness are cardinal symptoms of sleep apnea. We undertook this study to determine differences in stroke severity and early neurologic course in patients at risk for sleep apnea as determined by a sleep questionnaire. METHODS: Patients admitted with acute ischemic stroke completed the Berlin questionnaire. The Berlin questionnaire examines habitual snoring, dayt…

The mean apnea-hypopnea duration in children with obstructive sleep apnea is closely associated with the apnea-hypopnea index rather than anatomical severity.

OBJECTIVE: To investigate correlations between mean apnea-hypopnea duration (MAD) and clinical/polysomnographic parameters in pediatric obstructive sleep apnea (OSA). METHODS: We analyzed 568 children diagnosed with OSA (2017-2024) using polysomnography. Participants were categorized into long MAD (≥11 s) and short MAD (<11 s) groups for comparative analysis. RESULTS: No significant differences existed in age, gender, or BMI. The long MAD group demonstrated significantly higher adenoid grades and elevated arousal indices. All respiratory event indices were increased in the long MAD group. Mi…

[Life-threatening apnea after midazolam administration in a patient with obstructive sleep apnea syndrome].

In a 27-year-old patient with a severe obstructive sleep apnea syndrome documented by a polysomnographic study, an oral dose of 15 mg midazolam caused life-threatening obstructive apnea and excessive sedation. Therapy with nasal CPAP eliminated the pharyngeal obstruction, but marked central hypoventilation persisted. Benzodiazepines are known to aggravate the obstructive sleep apnea syndrome. Dangerous central hypoventilation occurring after this medication during treatment with nasal CPAP has not yet been reported. Excessive susceptibility to depression of chemoresponsiveness to pCO2 is suspe…

Exponential Relationship Between Maximal Apnea Duration and Exercise Intensity in Non-apnea Trained Individuals.

It is well known that the duration of apnea is longer in static than in dynamic conditions, but the impact of exercise intensity on the apnea duration needs to be investigated. The aim of this study was to determine the relationship between apnea duration and exercise intensity, and the associated metabolic parameters. Ten healthy active young non-apnea trained (NAT) men participated in this study. During the first visit, they carried out a maximum static apnea (SA) and a maximal progressive cycle exercise to evaluate the power output achieved at peak oxygen uptake (PVO2peak). During the secon…

Emergence of Central Sleep Apnea Events After Maxillomandibular Advancement Surgery for Obstructive Sleep Apnea.

PURPOSE: Central sleep apnea (CSA) can develop after the treatment of obstructive sleep apnea (OSA) with continuous positive airway pressure (CPAP). No studies have identified whether treatment of OSA with maxillomandibular advancement surgery (MMA) can result in CSA. The purpose of our study was to determine the incidence and clinical significance of CSA emerging after MMA surgery to treat OSA. PATIENTS AND METHODS: A retrospective review was conducted of all patients who had undergone MMA surgery for OSA at the Department of Oral and Maxillofacial Surgery at the QEII Health Sciences Centre …

Central sleep apnea in children with obstructive sleep apnea syndrome and improvement following adenotonsillectomy.

BACKGROUND: Although the pathogenesis of central and obstructive events seems to be different, these two entities may somehow be related. We aimed to determine whether, as reported in previous research, the number of central sleep apnea (CSA) cases in a population of children with obstructive sleep apnea syndrome (OSAS) was greater than in patients without obstructive events, and if CSA worsens with increasing OSAS severity. As a second objective, we analyzed changes in central apnea index (CAI) after adenotonsillar surgery compared to changes when no surgery has been performed. METHODS: We r…

Recurrence of sleep apnea syndrome following tracheostomy. A shift from obstructive to central apnea.

This report describes an unusual case of severe obstructive sleep apnea and alveolar hypoventilation leading to hypersomnolence and cor pulmonale, which were corrected by tracheostomy. Four years later, after a 22.5-kg weight gain, nocturnal apneas of similar frequency, duration, and depth of desaturation reappeared but were totally central in origin. The central apneas were eliminated with home nocturnal positive-pressure ventilation via cuffed tracheostomy tube. Each time the patient's apneas were corrected (obstructive: tracheostomy; central: mechanical ventilation), daytime alveolar hypove…

Rehabilitation and therapeutic exercise in obstructive sleep apnea: a perspective beyond the apnea-hypopnea index.

INTRODUCTION: Obstructive sleep apnea (OSA) is a multisystem disease whose clinical impact extends beyond nocturnal respiratory events. Although the apnea hypopnea index (AHI) is widely used to classify disease severity, this parameter does not adequately reflect the functional, symptomatic, and quality-of-life burden experienced by individuals with OSA. In this context, rehabilitative interventions have emerged as relevant therapeutic strategies within the comprehensive management of the disease. OBJECTIVE: To synthesize the available scientific evidence on the role of rehabilitation in indi…

Dynamics of Preparatory Apneas and Their Influence on Maximal Dry Static Apnea in Breath-Hold Divers.

Background: Physiological and psychological factors are important for determining static breath-hold duration. Preconditioning, such as preparatory apneas at functional residual capacity, is a potentially valuable method for prolonging breath-hold duration at total lung capacity. We investigated the physiological influence of preparatory apneas to determine the possible association with maximal apnea duration via diaphragmatic pressure and electromyographic measurements. Methods: Fourteen male breath-hold divers (39 ± 10 years; body mass, 87.2 ± 8.5 kg; body fat, 14.4 ± 3.8%; body height, 186.…

Increase in Body Mass Index Decreases Duration of Apneas and Hypopneas in Obstructive Sleep Apnea.

BACKGROUND: Obesity is one of the most prominent risk factors for obstructive sleep apnea (OSA). Weight loss decreases the number of shorter respiratory events (<40 s), whereas the number of longer events remains almost unchanged. However, it is unknown how body mass index (BMI) affects individual obstruction event severity within OSA severity categories when age, sex, smoking, daytime sleepiness, snoring, hypertension, heart failure, and sleeping posture are considered. Therefore, we investigated whether individual obstruction event severity varies with BMI within the OSA severity categories …
