[Cognitive neuropsychological investigation of apraxia].

Apraxia is a higher-order motor disturbance of goal-directed behavior characterized by an inability to perform skilled, previously learned movements in the absence of weakness and sensory defects. Clinical findings associated apraxia are divided into two large domains, production and conceptual. In this study, we reports two severely apraxic patients, case TT and UH, whose performances suggests that production and conceptual domains may be dissociable. Case TT is a 67-year-old, right-handed man who showed content errors on actual tool use, gesture production to command and gesture to sight of

The neurological bases of apraxia of speech.

Which site(s) of brain damage are associated with apraxia of speech (AOS)? There appears to be little agreement. The article first considers some reasons why not. Even allowing for factors that may have influenced findings, a definitive answer to the question of the neurological bases of AOS is not currently possible. The article goes on to look briefly at developments in the field of motor control, and limb and buccofacial apraxia in particular, that may hold clues to an answer or at least to asking the right questions. In particular, if AOS is to be understood as a motor disorder, then model

How comparable are tests of apraxia?

OBJECTIVE: To compare commonly used tests for the diagnosis of apraxia. MEASURES: People with unilateral left hemisphere lesions, diagnosed with apraxia, were compared in their scoring on commonly used tests for apraxia (n = 17). SETTING: Rivermead Rehabilitation Centre, Oxford, England, a specialist inpatient neurological unit. RESULTS: Production moment correlations were low, or did not reach significance level between three tests for ideomotor apraxia (r = 0.46 p < 0.05 one-tailed test, r = 0.005 NS, r = -0.23 NS, df = 15). Six of the seventeen apraxic patients scored above cut-off point

Limb apraxia: cortical or subcortical.

Ideomotor apraxia is defined as a disturbance in timing, sequencing, and spatial organization of gestural movements. Left hemisphere motor dominance reflected by ideomotor apraxia mainly refers to spatially and temporally complex movements performed outside the natural context. While clinicoanatomical studies have failed to unveil a specific lesion correlating with apraxia, white matter damage-interrupting corticocortical and corticosubcortical connections-seems crucial for the deficit to be persistent and severe. Patients with basal ganglia lesions and disorders, such as Parkinson's disease a

[Automatic-voluntary dissociation in apraxia].

Automatic-voluntary dissociation in apraxia was discussed and the following two points were noted 1) This phenomenon is observed for praxis with procedural memory, i.e., learned or skilled movements such as brushing teeth and dressing, but not observed for praxis without procedural memory such as tasks used for testing constructional apraxia. 2) This phenomenon may be understood by dual motor act systems, i.e. implicit and explicit motor systems. Presumably, the former is mainly mediated by medial premotor system that involitionally drives motor procedural memory, while the latter is mainly me

Apraxia during Wada testing.

Apraxia is the loss of the ability to perform learned skilled movements correctly. In right-handers, apraxia and aphasia are most frequently associated with left-hemisphere lesions. When they are dissociated, however, aphasia is more common in the absence of apraxia than vice versa. There are two hypotheses that can account for this discrepancy: (1) in right-handers, praxis is more likely than language to be mediated by the right hemisphere, or (2) the left-hemisphere network that mediates language is either more widely distributed than the network that mediates praxis or is more likely to be

Sensorimotor learning in ideomotor apraxia.

Sensorimotor learning ability in patients with ideomotor apraxia was investigated, using as subjects, eight patients with ideomotor apraxia, 8 with aphasia without ideomotor apraxia, and 8 normal controls. The aphasia, apraxia, and normal control groups were matched for age, sex, and education. The aphasia and apraxia groups were chosen to control for lesion size and scores on Kimura's recurring figure test, the Token test, and intelligence. The mirror-aiming test was performed and the learning effect in terms of decreases in total time, the number of errors, and the times for errors were exam

Ideomotor apraxia in Alzheimer's disease.

Ideomotor apraxia, tested on verbal command and by imitation, was checked in 23 patients suffering from dementia of Alzheimer's type of different severity and in 17 age-matched controls. A significant deterioration of ideomotor praxis could be shown even in mild dementia. Correlations of ideomotor apraxia and aphasia, tested by the Token test were found to be significant.

Analysis of task demands in apraxia.

Several task demands were examined in a battery of praxis tests: the movement system (limb versus axial), input modality (command versus imitation), movement complexity (single gestures versus a sequence of gestures), type of limb gesture (transitive versus intransitive), and the representational nature of the gestures. Performance accuracy for a group of left hemisphere patients was significantly lower than for two other groups of patients with either right hemisphere damage or no brain damage on all gestures. The right hemisphere patients were significantly different from the normals only fo

[Ideational apraxia in confusional psychosis].

Traditionally, Pick's ideational apraxia has been considered a neurologist's realm since it is one of the permanent manifestations of brain lesions located in the left hemisphere posterior territory. Besides, however, the ideational apraxia is likely to appear--altogether with misidentifications and spatial disorientation--as a part of the transient manifestations of confusional psychosis. It is thus quite possible that a reversible alteration of the posterior brain mechanisms could be the basis for the confusional psychosis. It seems also reasonable to include the ideational apraxia within th

Vertical ocular motor apraxia.

We report a case of vertical ocular motor apraxia in a 4-year old boy whom we have followed for 2 years. The patient had no upward and downward voluntary movements of the eyes since birth, but horizontal movements were normal. He achieved gaze changes in the vertical plane by combined blinks and head movements. Magnetic resonance imaging demonstrated bilateral lesions at the mesencephalic-diencephalic junction.

Apraxia of lid opening in blepharospasm.

Apraxia of lid opening is a nonparalytic motor abnormality characterized by difficulty in initiating the act of lid elevation. It has been reported with extrapyramidal disorders, including Parkinson's disease, Huntington's chorea, progressive supranuclear palsy, and Shy-Drager syndrome. We found seven cases (7%) of functionally disabling apraxia of lid opening in 100 consecutive blepharospasm patients studied. It is important for physicians treating blepharospasm to be aware of the association between these two visually debilitating disorders.

On the cerebral localization of constructional apraxia.

Twenty patients with constructional apraxia, their only neuropsychological impairment, were examined. On the basis of CT-findings it could be shown that the syndrome is very close connected with lesions of the right hemisphere of the brain (parietal brain-cortical and subcortical-and parieto-occipital border areas).

The executive and ideational components of apraxia.

Fifty six left brain-damaged (LBD) patients and 38 right brain-damaged (RBD) patients were requested to perform two measures of praxis using the hand ipsilateral to the side of lesion. One task required the accurate imitation of actions made by the examiner and taxed executive abilities; the other task required the patient to pantomime the use of objects that were shown, but not handed, and it also involved an ideational component, i.e., the evocation of gestures. The cut-off point discriminating a normal from a pathological performance was set at the level of the poorest score found in 60 con
