AIMS: To study atrial tachycardia-induced electrical remodeling in a canine model of sinus node dysfunction (SND).
MATERIALS AND METHODS: A canine model of SND was established by contacting a cotton patch with 20% formaldehyde on the sinus node. Atrial effective refractory period (ERP), ERP dispersion, and inducibility of atrial fibrillation (AF) were recorded at multiple sites in the atrium, before and after SND induction as well as after rapid atrial pacing. The recovery of atrial ERP in the left and right atrium (LA and RA) after cessation of atrial pacing was also recorded.
RESULTS: Comp

Atrial tachycardia (AT) downregulates L-type Ca(2+) current (I(CaL)) and causes atrial fibrillation-promoting electric remodeling. This study assessed potential underlying signal transduction. Cultured adult canine atrial cardiomyocytes were paced at 0, 1, or 3 Hz (P0, P1, P3) for up to 24 hours. Cellular tachypacing (P3) mimicked effects of in vivo AT: decreased I(CaL) and transient outward current (I(to)), unchanged I(CaT), I(Kr), and I(Ks), and reduced action potential duration (APD). I(CaL) was unchanged in P3 at 2 and 8 hours but decreased by 55+/-6% at 24 hours. Tachypacing caused Ca(2+)

We report the case of a 24-year-old man who underwent surgical repair of a sinus venosus atrial septal defect. Transthoracic echocardiograms performed on Day 3 and 6 after surgery demonstrated a very early cardiac remodeling with dramatic reduction of the right ventricular and atrial dimensions paralleled by an increase in the size of the left ventricle. This case provides the first demonstration of a very early cardiac remodeling after repair of sinus venosus atrial septal defect.

Atrial fibrillation (AF) is the most frequent arrhythmia found in clinical practice. In recent studies, a decrease in the development or recurrence of AF was found in hypertensive patients treated with angiotensin-converting enzyme inhibitors or angiotensin receptor-blocking agents. Hypertension is related to an increased wall tension in the atria, resulting in increased stretch of the individual myocyte, which is one of the major stimuli for the remodeling process. In the present study, we used a model of cultured atrial neonatal rat cardiomyocytes under conditions of stretch to provide insig

INTRODUCTION AND OBJECTIVES: Recent clinical studies suggest a potential antiarrhythmic role of angiotensin-converting enzyme inhibitors in preventing atrial fibrillation. Studies in an animal model suggested that these drugs may prevent sustained atrial fibrillation by avoiding the occurrence of detrimental atrial electrical remodeling secondary to temporary episodes of fibrillation or atrial tachycardia. We sought to determine whether intravenous enalaprilat, administered at doses habitually used in clinical practice, prevented pacing-induced acute atrial remodeling.
PATIENTS AND METHOD: We…

The failing ventricular myocardium is characterized by reduction of high-energy phosphates and reduced activity of the phosphotransfer enzymes creatine kinase (CK) and adenylate kinase (AK), which are responsible for transfer of high-energy phosphoryls from sites of production to sites of utilization, thereby compromising excitation-contraction coupling. In humans with chronic atrial fibrillation (AF) unassociated with congestive heart failure (CHF), impairment of atrial myofibrillar energetics linked to oxidative modification of myofibrillar CK has been observed. However, the bioenergetic sta

BACKGROUND: The aim of this study was to examine the reduction in the atrial conduction time during the follow-up phase of catheter ablation in persistent atrial fibrillation (PsAF) patients, which would suggest atrial reverse remodeling, and to investigate its impact on predicting AF recurrence.
METHODS: Fifty-seven PsAF patients underwent radiofrequency catheter ablation. The P-wave to left atrial appendage (LAA) conduction time was measured by transthoracic echocardiography using tissue Doppler imaging (P-LAA TDI), within 24 h and 6 months after ablation.
RESULTS: The accuracy of the P-LA

BACKGROUND: Obstructive sleep apnoea (OSA) can cause left atrial (LA) and left ventricular (LV) remodelling, which is linked to atrial fibrillation (AF). Whether continuous positive airway pressure (CPAP) can reverse LA and LV remodelling in patients with OSA and paroxysmal AF (PAF) has yet to be studied. We assessed the impact of CPAP treatment on LA and LV size and function in patients with OSA and PAF before and after catheter ablation.
METHODS: In a randomised controlled trial, we screened patients with PAF for OSA. We enrolled patients with an Apnoea-Hypopnoea Index ≥15/hour. The burden

Obstructive sleep apnoea (OSA) affects 9-24% of the adult population. OSA is associated with atrial disease, including atrial enlargement, fibrosis and arrhythmias. Despite the link between OSA and cardiac disease, the molecular changes in the heart which occur with OSA remain elusive. To study OSA-induced cardiac changes, we utilized a recently developed rat model which closely recapitulates the characteristics of OSA. Male Sprague Dawley rats, aged 50-70 days, received surgically implanted tracheal balloons which were inflated to cause transient airway obstructions. Rats were given 60 apnoea

This review deals with the understanding of the role of key factors of ageing, oxidative stress and inflammation, in relation to development of age-related cardiovascular disease, atrial fibrillation. Increased production of reactive oxygen species and systemic inflammation promote cardiac structural and electrophysiologic remodeling that is crucial with respect to development and sustaining of atrial fibrillation. Data suggest that alterations in atrial connexin-43 and/or connexin-40 expression, phosphorylation and distribution affect cell-to-cell electrical coupling and molecular signalling

AIMS: Atrial cardiomyopathy (AtCM) is increasingly recognized as a substrate for atrial fibrillation (AF), yet its operationalization remains limited and largely marker-driven. Whether autonomic remodelling represents a longitudinally evolving domain within a multidomain framework of AtCM remains unclear.
OBJECTIVES: To determine whether autonomic remodelling, assessed through static and longitudinal heart rate variability (HRV) abnormalities, defines a distinct domain of AtCM and contributes to atrial disease burden and clinical risk.
METHODS AND RESULTS: We studied 670 individuals aged 65

BACKGROUND: FGF23 mediates cardiac fibrosis through the activation of pro-fibrotic factors in in vitro models and is markedly elevated in kidney disease. Left atrial global longitudinal strain (LA GLS) derived by echocardiographic speckle-tracking measures longitudinal shortening of the LA walls, quantifies atrial performance and may enable detection of early LA remodeling in the setting of normal ventricular function. We hypothesized that LA GLS is abnormal in children on hemodialysis (HD) compared to healthy controls of comparable age/sex distribution and that, among HD patients, greater FGF

BACKGROUND: Artificial intelligence-derived electrocardiographic (AI-ECG) age is conventionally treated as a static marker. Its diurnal dynamics and structural correlates in individuals without atrial fibrillation (AF) remain unknown.
OBJECTIVE: To characterize diurnal AI-ECG age patterns, their reproducibility, and associations with left atrial (LA) remodeling.
METHODS: We analyzed continuous wearable ECGs from two prospective registries (NCT05119725, NCT05355948). In 499 AF-naïve participants with ≥48-hour recordings, AI-ECG age was estimated every 10 minutes. Unsupervised clustering ident

BACKGROUND: Long-term dynamic changes in the left atrium (LA) following radiofrequency catheter ablation (RFCA) for atrial fibrillation (AF) remain unclear. We aimed to unveil the temporary dynamics of LA reverse remodeling and the optimal echocardiographic indicator and timing for evaluating RFCA outcomes in this 3-year longitudinal study.
METHODS: Data were retrospectively collected from 400 consecutive patients undergoing RFCA for AF (paroxysmal 71%). Echocardiographic examinations were performed at 1 day, and 1, 3, 6, 12, 18, 24, 30, and 36 months after RFCA. Patients were classified into

BACKGROUND: Left bundle branch area pacing may reduce new-onset atrial fibrillation. However, its mechanism and the comparison of atrial reverse remodeling effect between left bundle branch pacing (LBBP) and left ventricular septal pacing (LVSP) has not been elucidated. We aim to investigate reverse atrial remodeling by LBBP and LVSP in a canine model of rapid pacing-induced dyssynchronous heart failure (DHF).
METHODS: Twenty-six beagles followed a 6-week protocol: controls (n=4), rapid atrial pacing (n=4), DHF (n=6), DHF+LBBP and DHF+LVSP (3 weeks DHF followed by 3 weeks LBBP or LVSP, both n…
