Central aortic versus axillary artery cannulation for aortic arch surgery.

OBJECTIVE: Central aortic cannulation for aortic arch surgery has become more popular over the last decade; however, evidence comparing it with axillary artery cannulation remains equivocal. This study compares outcomes of patients who underwent axillary artery and central aortic cannulation for cardiopulmonary bypass during arch surgery. METHODS: A retrospective review of 764 patients who underwent aortic arch surgery at our institution between 2005 and 2020 was performed. The primary outcome was failure to achieve uneventful recovery, defined as having experienced at least 1 of the followin…

[Surgical treatment of giant false aneurysm of the axillary artery].

Vascular damage is an important aspect in the structure of injuries. Untimely medical care in patients with peripheral artery damage is often followed by development of false aneurysm. This event may be complicated by aneurysm rupture and bleeding, peripheral arterial thromboembolism, hematoma infection and neuropathy following adjacent nerved compression. We report surgical treatment of a patient with giant false aneurysm of the left axillary artery following a knife wound 1 year ago. The patient did not appeal for qualified medical care and ensured haemostasis at home. The patient underwent …

Bilateral axillary artery aneurysms after Bentall procedure in Marfan syndrome.

A man with Marfan syndrome underwent a Bentall procedure for annuloaortic ectasia and severe aortic regurgitation at 43 years of age. Twenty-eight years after the Bentall procedure, he developed bilateral axillary artery aneurysms (length × diameter: right: 80 × 39 mm; left: 103 × 45 mm). Aneurysmectomy and reconstruction of the axillary artery were performed using an artificial vascular graft. Histological examination revealed cystic medial necrosis. The postoperative course was uneventful, but long-term follow-up is necessary.

Angiofollicular lymphoid hyperplasia--an unusual cause of axillary artery pseudoaneurysm.

BACKGROUND: Angiofollicular lymphoid hyperplasia (Castleman's disease) is a rare inflammatory condition. Its cause is unknown and it can be both localized and general. Eighty-six percent of lesions are found in the mediastinum or hilum, and 91% are of the hyaline vascular type. Although Castleman's disease is primarily involving the chest and retroperitoneum, it may also involve neck and axilla in 2-4% of cases. In this article, we present a very rare case of Castleman's disease causing axillary artery pseudoaneurysm. METHODS AND RESULTS: A 30-year-old woman patient presented with pulsating t…

Endovascular treatment of axillary artery dissection following anterior shoulder dislocation.

Injury to the axillary artery is a rare complication of anterior shoulder dislocation. Open surgical repair is technically demanding because of the anatomical position of the vessel and the propensity for concomitant injuries. Standard surgical exposure techniques involve extensive dissection, including a combination of supraclavicular or infraclavicular incision, median sternotomy, and thoracotomy causing significant morbidity and mortality rates. Endovascular techniques may offer an alternative to these surgically demanding procedures. We present a patient with a traumatic dissection of the …

Bilateral axillary and suprascapular neuropathy after coronary artery bypass surgery.

Neuropathies after coronary artery bypass (CABG) and cardiac surgery are usually transient and have been described fairly frequently. Our patient presented with an acute onset of a surprisingly symmetrical bilateral axillary and suprascapular neuropathy during the immediate CABG period (which to our knowledge has not been reported on before). The patient, aged 64 years, suffered from diabetes mellitus type 2 and hypertension, and underwent a triple CABG. Electroneuronographic and electromyographic examination confirmed the presence of the bilateral axillary and suprascapular neuropathy, which

Aortic reoperations: experience with 23 patients using axillary artery cannulation.

The increase in the number of cardiac operations has brought about an increase in aortic pathology that requires reoperation. The aim of the present study was to evaluate axillary artery cannulation in aortic reoperations. We operated on 23 patients diagnosed with acute type A aortic dissection (13 patients), chronic type A aortic dissection (7 patients), aortic pseudoaneursym (2 patients), and arcus aorta aneurysm (1 patient). The right axillary artery was cannulated directly or through a side graft which was anastomosed to the artery. Antegrade cerebral perfusion with moderate degree hypothe…

Axillary artery aneurysm in tuberous sclerosis: cross-sectional imaging findings.

A 10-month-old boy with a known diagnosis of tuberous sclerosis presented with axillary and brachial masses. Cross-sectional imaging with computed tomography, magnetic resonance, and ultrasound demonstrated unsuspected aneurysms of the axillary and brachial arteries. The results of these studies significantly changed the preoperative planning for this patient, who was scheduled for a biopsy of a presumed soft-tissue tumor. To our knowledge, this is the first description of the cross-sectional imaging findings in the extremity peripheral vascular manifestation of tuberous sclerosis; it has only

Axillary artery injury in closed fracture of the humeral neck.

A case of atherosclerotic axillary artery rupture in closed fracture of the proximal part of the humerus is described. It demonstrates a possible serious arterial injury in elderly persons even in case the fracture of the humeral neck is not dislocated. If there are signs of disturbed circulation in these injuries, angiography is indicated irrespective of the type of fracture and mechanism of injury.

Central venous and pulmonary artery catheterizations via the axillary vein.

We report the results of over 300 central venous and pulmonary artery catheterizations via the axillary vein. We found that the success rate with this method compares well with those of other catheterization routes, whilst the risk of mechanical injury, even for artificially ventilated patients, was virtually eliminated.

Axillary artery disruption secondary to anterior dislocation of the shoulder.

We report a 13-year-old male who sustained a segmental injury to the third part of the left axillary artery following a subcoracoid shoulder dislocation while wrestling. The artery was repaired with an autogenous saphenous vein interposition graft. The patient's postoperative course was uneventful, and, in a 3-year followup, he has complete range of motion of the left shoulder without neurovascular compromise.

Axillary Artery Variant: A Cadaveric Case Report and Clinical Significance.

Vasculature variations are commonly studied and used as a tool to further understand embryology, pathology, and clinical significance. In this case report, a rare axillary artery variant (AA) in the right upper limb of an 84-year-old Caucasian male donor was analyzed (including its relationship with the brachial plexus) and differentiated from the canonical AA branching pattern seen in the left upper limb. The variant was observed as a bifurcation of the second part of the AA into the superficial axillary branch (SAB) and deep axillary branch (DAB). The SAB gave off the lateral thoracic artery…

Axillary artery and brachial plexus injury secondary to blunt trauma.

Rupture of the axillary artery in the absence of a fracture of dislocation is a rare traumatic event. An associated injury to the brachial plexus may accompany an axillary artery injury but has rarely been reported in the literature. We present the case of an elderly female, who fell onto an outstretched arm and sustained an axillary artery rupture, combined with a brachial plexus injury. The patient in this case did well post-operatively. The challenge in these cases is early recognition and diagnosis of a vascular injury. A significant mechanism of injury needs to alert the clinician to the …

Rescue axillary artery covered stent during transvenous cardiac device implantation.

Injury to axillary and subclavian arteries during cardiac rhythm device implantation might lead to significant mortality and morbidity espec … t amount of arterial bleed was suddenly encountered without a clear source. Traumatic injury to the axillary artery caused by the suture needle was suspected. An immediate angiography of the left axillary artery via femoral approach showed a significant axillary artery side branch leakage adjacent to the sleeve suture site. A covered stent was deployed to the axi …

Left Axillary Artery Cannulation Facilitates Reoperative Total Aortic Arch Replacement.

Total aortic arch replacement remains a technically formidable procedure, particularly in patients with previous proximal aortic dissection repair. Our case discussion highlights a useful strategy for extracorporeal support and circulation management to facilitate total arch reconstruction in the reoperative setting, based on cannulation of the left axillary artery. Our preference is to use a left axillary artery approach to initiate cardiopulmonary bypass and to ultimately revascularize the left arm via an extra-anatomic graft. Our technique, as described, affords the option to initiate cardi…

Endovascular management of radiation-induced subclavian and axillary artery aneurysms.

Subclavian and axillary artery aneurysms are rare occurrences and are associated with serious sequelae if they are untreated. Little is known about these aneurysms, and best practice guidelines are lacking. We describe an 87-year-old man with a history of chest irradiation who presented with radiation-induced subclavian and axillary aneurysms and acute upper extremity ischemia. An endovascular stent graft procedure was undertaken because of his prohibitive high risk for open surgical treatment. Follow-up duplex ultrasound revealed patent stent grafts with complete exclusion of aneurysm sacs. E…

Traumatic pseudoaneurysm of axillary artery combined with brachial plexus injury.

Traumatic pseudoaneurysm of the axillary artery combined with brachial plexus injury is extremely rare. The factors that influence the symptoms and functional recovery related to this condition are unclear. Nine patients who had sustained this trauma were surgically treated at our unit between June 1999 and November 2010. The cause of trauma, symptoms, signs and examinations of neurological and vascular deficits, and the surgical findings of the involved nerves and vessels were recorded in detail. The functional recovery of vessels and nerves, as well as the extent of pain, were evaluated, res…
