BACKGROUND: Double-balloon enteroscopy (DBE) is a new endoscopic tool that not only allows diagnostic workup of small bowel diseases, but also makes it possible to carry out therapeutic interventions. However, for a variety of reasons, endoscopic therapy appears to be more difficult to carry out deep in the small bowel than in the upper or lower gastrointestinal tract.
AIM: The purpose of this study was to evaluate the acute technical success and acute complication rate of DBE.
PATIENTS: Between June 2003 and July 2006, 353 patients (152 women, 201 men; mean age 60.3 +/- 17.1 yr) with suspec…

Background Pediatric Crohn's disease is frequently characterized by extensive small-bowel involvement and a dynamic disease course. Double-balloon enteroscopy (DBE) allows direct evaluation of proximal small-bowel lesions that may not be detected by conventional upper gastrointestinal endoscopy and ileocolonoscopy; however, the long-term clinical implications of baseline DBE findings remain poorly defined. Methods We conducted a retrospective, single-center longitudinal cohort study including pediatric patients initially diagnosed with Crohn's disease who underwent baseline DBE. Clinical and …

INTRODUCTION: An association has been found between human-gut microbiota and various diseases (e.g., metabolic disease) by analyzing fecal or colonic microbiota. Despite the importance of the small intestinal microbiota, sampling difficulties prevent its full analysis. We investigated the composition and metagenomic functions of microbiota along the small intestine and compared them with the microbiota from feces and from other gastrointestinal (GI) sites.
METHODS: Mucosal samples from the six GI sites (stomach, duodenum, distal jejunum, proximal ileum, terminal ileum, and rectum) were collec

BACKGROUND & AIMS: Complicated celiac disease (CCD) is a rare but severe condition with a poor prognosis. Guidelines recommend use of capsule endoscopy (CE) to explore the small bowel (SB), followed by a double-balloon enteroscopy (DBE) in selected cases with suspected CCD. Our study aimed to evaluate the diagnostic yield (DY) of CE and DBE in identifying and monitoring CCD.
METHODS: Consecutive suspected CCD patients were enrolled prospectively to undergo CE and/or DBE in the presence of: persistent symptoms despite a correct gluten-free diet (GFD), increased anti-transglutaminase antibodies…

BACKGROUND: Peutz-Jeghers syndrome (PJS) is a very rare disease that often causes severe complications such as bowel obstruction or gastrointestinal tract bleeding. In the past, it was usually treated by using surgical intervention despite the associated complications. Balloon-assisted enteroscopy (BAE) has been documented as an effective and safe method for the diagnosis and treatment of small bowel lesions. Hence, we conducted this study to verify whether BAE is useful for patients with PJS.
AIM: To evaluate the safety and efficacy of BAE with prophylactic polypectomy in patients with PJS.

The Fujinon intelligent color enhancement (FICE) system is a new, virtual chromoendoscopy technique that enhances mucosal visibility. The aim of this study was to assess the utility of double-balloon enteroscopy (DBE) with FICE technology (EPX-4400 processor, Japan) for the characterization of various small-bowel diseases. Overall, a total of 574 endoscopic pictures were obtained and analyzed. FICE was found to be a helpful method for the evaluation of adenomatous small-bowel polyps and angiodysplasias. Its use for the characterization of celiac and Crohn's disease appears to be limited. Overa…

Iliac artery pseudoaneurysm is a rare complication after pancreas transplantation that can present as obscure gastrointestinal bleeding. A 56-year-old gentleman with failed pancreas transplant developed hematochezia and anemia with bidirectional endoscopy and computed tomography angiography being inconclusive. Video capsule endoscopy suggested distal small bowel bleeding, and retrograde double-balloon enteroscopy identified a solitary ulcer with a protruding pulsatile vessel. A right external iliac pseudoaneurysm was confirmed and treated with endovascular stenting that was complicated by limb…

INTRODUCTION AND AIM: Double balloon enteroscopy is used for the diagnosis and treatment of diseases of the small bowel. It is a less invasive means for carrying out a therapeutic intervention, compared with surgical alternatives. Experience with double balloon enteroscopy in pediatrics is in the early stages of development. We believe the performance of this procedure in children has a significant clinical impact on therapeutic decision-making in diseases of the small bowel. Our aim was to evaluate double balloon enteroscopy in children for the diagnosis and treatment of midgut pathologies.

We present a case of Cronkhite-Canada syndrome (CCS) in which the entire intestine was observed using a prototype of magnifying single-balloon enteroscope (SIF Y-0007, Olympus). CCS is a rare, non-familial gastrointestinal polyposis with ectodermal abnormalities. To our knowledge, this is the first report showing magnified intestinal lesions of CCS. A 73-year-old female visited our hospital with complaints of diarrhea and dysgeusia. The blood test showed mild anemia and hypoalbuminemia. The esophagogastroduodenoscopy and colonoscopy revealed diffuse and reddened sessile to semi-pedunculated po

The diagnosis of the rare neoplasm histiocytic sarcoma (HS) relies on morphology and the presence of immunophenotypic features of histiocyti … ntent. The present report describes how HS can be diagnosed at a stage of favorable prognosis using balloon enteroscopy (BE), thereby enabling surgical resection before the development of metastases. This strategy is reviewed in the setting of a patient with jejunal HS, followed by a discussion of data from 16 other reported cases of GI HS.
