We report a case of cardiac beriberi in a 76-year-old man who was hospitalized with a congestive condition of subacute onset, diagnosed as high-output heart failure associated with severe tricuspid regurgitation and indication for caval valve implantation, which, after thiamine replacement, resulted in improvement of all conditions. Thiamine deficiency usually occurs with prolonged nutritional deficiency and is almost universally identified with Wernicke's encephalopathy or beriberi. It is also, however, a rare cause of elevated lactate and gastroenterological symptoms. The authors report 3 cases of pediatric cardiac beriberi at Queen Sirikit National Institute of Child Health during the last 10-year-period. The first two cases presented classically while the third case came with an unusual presentation. Cardiac beriberi is an old disease in modern Bangkok which can present in as several different clinical patterns. The diagnosis needs a high index of suspicion. Echocardiographic findings of right heart dysfunction and dramatic response to intravenous thiamine are diagnostic.

Chickens fed with the same composition of diet as our low income beriberi polyneuropathic patients, developed clinical symptoms of thiamine deficiency in 22.3 +/- 6.3 days. A case of alcoholic beriberi is described. Left ventricular systolic function as measured by the radionuclide ejection fraction showed an abnormal fall with exercise. Subsequent return to normal with the administration of thiamine suggests myocardial involvement in the condition.

A 30 year old man with a 15 year history of alcohol abuse presented with symptoms and signs of circulatory shock, severe disturbances of renal and liver functions, and metabolic acidosis. The cardiovascular and metabolic features were attributable to Shoshin beriberi. He recovered completely after treatment with thiamine.

A case of beriberi based on short bowel is reported. A 4 year old girl, who underwent a resection of the intestine when a neonate and has had a short bowel, manifested beriberi despite eating normal Japanese food. Considering that she has had no dislikes of foods, and her family members, who have eaten meals similar to hers have been healthy, beriberi is considered in this case to have developed mainly through hypoabsorption of vitamin B1. Four cases of beriberi were diagnosed on clinical grounds and 3 were confirmed biochemically. The cases cover a spectrum of beriberi heart disease ranging from a hyperdynamic circulation to circulatory shock. They are Shoshin beriberi, beriberi with high output failure, beriberi with signs of a hyperdynamic circulation alone and beriberi with underlying cardiomyopathy.
